Abstract
ภายหลังเกิดภาวะเศรษฐกิจประเทศตกตํ่าในปี พ.ศ.2540 ประชาชนมีการปรับเปลี่ยนพฤติกรรมการบริโภค รวมถึงการเลือกใช้บริการทางการแพทย์ สถานบริการสุขภาพต่าง ๆ โดยเฉพาะในส่วนของเอกชน ได้มีการปรับเปลี่ยนกลยุทธ์การบริหารจัดการและการตลาด เพื่อสนองความต้องการของผู้บริโภค และเพื่อคงความสามารถการดำเนินธุรกิจบริการผู้ป่วยได้อย่างยั่งยืนต่อไป การวิจัยครั้งนี้มีวัตถุประสงค์ 2 ประการ คือ (1) เพื่อศึกษาระดับปัจจัยที่มีอิทธิพลต่อการเลือกใช้บริการโรงพยาบาลของรัฐบาลหรือโรงพยาบาลเอกชนในจังหวัดเชียงใหม่ (2) เพื่อศึกษาความสัมพันธ์ระหว่างปัจจัยลักษณะส่วนบุคคล กับ การเลือกใช้บริการโรงพยาบาล กลุ่มตัวอย่างเป็นผู้ใช้บริการโรงพยาบาลในจังหวัดเชียงใหม่ 35 แห่ง ได้แก่โรงพยาบาลรัฐบาล 25 แห่งและโรงพยาบาลเอกชน 10 แห่ง มีจำนวนตัวอย่างจากโรงพยาบาลรัฐบาล 400 ตัวอย่าง และโรงพยาบาลเอกชน 400 ตัวอย่าง วิธีการวิจัย ใช้แบบสอบถามที่ได้พัฒนาและสร้างขึ้นเพื่อ
ถามถึงปัจจัยที่มีอิทธิพลต่อการเลือกใช้บริการโรงพยาบาล จากกรอบแนวคิดส่วนประสมทางการตลาดบริการที่ผ่านการทดสอบแล้ว ซึ่งมีค่าความเชื่อมั่น 0.85 สถิติที่ใช้ในการวิเคราะห์ข้อมูลคือสถิติเชิงพรรณนาได้แก่ การคำนวณร้อยละ ความถี่ ค่าเฉลี่ย และค่าเบี่ยงเบนมาตรฐาน และสถิติเชิงอนุมานได้แก่ การทดสอบไคสแคว์ โดยใช้โปรแกรมสำเร็จรูป SPSS ผลการวิจัย (1) ปัจจัยที่มีอิทธิพลต่อการเลือกใช้บริการโรงพยาบาลในระดับมากที่สุดพบในกลุ่มผู้เลือกใช้บริการโรงพยาบาลเอกชน ได้แก่ โรงพยาบาลมีแพทย์ที่มีความชำนาญเฉพาะทางปัจจัยส่วนใหญ่พบในระดับมาก ปัจจัยที่มีอิทธิพลในระดับน้อยที่สุดพบในกลุ่มผู้เลือกใช้บริการโรงพยาบาลรัฐบาล ได้แก่ ค่านิยมตามลัทธิศาสนา การเยี่ยมบ้าน การใช้บัตรเครดิต การมีรถรับส่ง และอัตราการบริการอื่นๆ (2) อายุ สถานภาพการสมรส ระดับการศึกษา อาชีพ รายได้เฉลี่ยต่อเดือน จุดประสงค์การมาโรงพยาบาลและการมีสวัสดิการรักษาพยาบาลมีความสัมพันธ์ต่อการเลือกใช้บริการ

There were many influencing factors of choosing hospital services before the economic crisis in 1997. Since then, the customer had changed their behaviors of consuming. Hospitals, especially in private sectors had also been developing their competitive strategies in order to hold clients and sustain their competitive advantages. The objectives of the study were (1) to determine the level of influencing factor of choosing hospital, and (2) to compare between the individual characteristics of clients and type of hospital they chose. The samples in this study were the clients from the 35 hospitals in Chiang Mai province. The number of sample were 400 cases from 25 government hospitals and 400 cases from 10 private hospitals. The samples were given questionnaires with reliability of 0.85. SPSS program for windows was used to calculate percentage, mean, standard deviation and chi-square test. The results were (1) The highest level of influencing factor for choosing hospital was an expert doctors available which was found among clients who chose private hospitals. The factors were mainly found in high level. Religions, using credit card, ambulance available and price of other services were classified at lowest level among clients who chose government hospitals. (2) Age, marital status, educational level, occupation, income, purpose of health service and health insurance were the characteristics which related with type of hospital they chose at a significant level of 0.05.
