Abstract
การศึกษาครั้งนี้มีวัตถุประสงค์เพื่อทบทวนงานวิจัยอย่างเป็นระบบเกี่ยวกับรูปแบบและวิธีการให้ข้อมูลในผู้ป่วยก่อนเข้ารับการผ่าตัดศึกษาโดยสืบค้นงานวิจัยระหว่างปี ค.ศ. 2008-2012 จากฐานข้อมูลอิเล็กทรอนิกส์ และการสืบค้นด้วยมือ พบงานวิจัยที่เกี่ยวข้องกับการให้ข้อมูลในผู้ป่วยก่อนเข้ารับการผ่าตัด ตามเกณฑ์ที่กำหนด 10 เรื่อง เป็นงานวิจัยจำนวน 4 เรื่อง วิทยานิพน 2 เรื่องและการศึกษานคว้าอิสระ 4 เรื่อง สามารถแบ่งระดับเกณฑ์การประเมินคุณภาพรายงานวิจัยของราชวิทยาลัยอายุรแพทย์แห่งประเทศไทย ได้เป็นหลักฐาน ระดับ A จำนวน 1 เรื่อง ระดับ B จำนวน 8 เรื่องและระดับ C จำนวน 1 เรื่อง
ผลการศึกษา พบว่า มีการให้ข้อมูลในผู้ป่วยรับการผ่าตัด 6 กลุ่ม ได้แก่ 1) ผ่าตัดโรคทางศัลยกรรมทั่วไป 2) ผ่าตัดแผลที่เท้าในผู้ป่วยเบาหวานชนิดที่สอง 3) ผ่าตัดเปลี่ยนข้อสะโพกและข้อเข่าเทียม 4) ผ่าตัดมะเร็งหลอดอาหาร ) ผ่าตัดไขสันหลัง ๑) ผ่าตัดหัวใจแบบเปิด เนื้อหาข้อมูลสำหรับผู้ป่วยทุกกลุ่ม ได้แก่ วิธีการหรือการรักษาก่อนและ หลังผ่าตัดได้แก่ การปฏิบัติตัวขณะเข้ารับการรักษา การบรรเทาอาการปวด การเคลื่อนไหวร่างกายหลังผ่าตัด รูปแบบการให้ข้อมูล พบว่า ผู้ให้ข้อมูลได้แก่ ทีมสุขภาพ ซึ่งส่วนใหญ่คือพยาบาลวิชาชีพวิธีการให้ข้อมูลมักดำเนินการเป็นรายบุคคล โดยการบรรยาย ร่วมกับการใช้ สื่อ คู่มือ ต่าง ๆ พอกจากนี้ยังพบว่าผู้ป่วยผ่าตัดแต่ละกลุ่มได้รับข้อมูลเฉพาะตามชนิดของการผ่าตัด ดังนี้ 1) ผู้ป่วยผ่ตัดโรกทางชัลยกรรมทั่วไป มีการให้ข้อมูลวิธีการเผชิญความเครียดโดยการใช้เทคนิคผ่อนคลาย 2) ผู้ป่วยผ่าตัดแผลที่เท้าใบผู้ป่วยเบาหวานชนิดที่สอง ไห้รับข้อมูลเกี่ยวกับโรคเบาหวานและภาวะแทรกซ้อนที่เท้า 3) ผู้ป่วยผ่าตัดเปลี่ยนข้อสะไพกและข้อเข่าเทียมได้รับข้อมูลเกี่ยวกับสรีรวิทยาของข้อสะโพกและข้อเข่าการทำหน้าที่ของข้อสะโพกและข้อเข่าในสภาวะปกติ ข้อมูลเกี่ยวกับการผ่าตัด และการเปลี่ยนข้อสะ โพกและข้อเข่าใหม่ และการทำกายภาพบำบัดหลังผ่าตัด การฟื้นฟูขณะอยู่ที่บ้านและการออกกำลังกายที่เหมาะสม 4) ผู้ป่วยผ่าตัดมะเร็งหลอดอาหารได้รับข้อมูลการดูแลที่จะได้รับขณะรักษาตัวในไอซียู 5) ผู้ป่วยผ่าตัดไขสันหลัง ไห้รับข้อมูลเกี่ยวกับสรีรวิทยาพื้นฐานของไขสันหลัง การบริหารปอดไดยฝึกการใช้เครื่อง Incentive spirometry การใช้เครื่อง Sequential Compression Device (SCD) การใช้ชุดช่วยพยุงหลัง และ 6) ผู้ป่วยผ่าตัดหัวใจแบบเปิด ได้รับข้อมูลระยะเวลาการรักษาตัวใน ไอซียูหลังผ่าตัด การย้ายกลับหอผู้ป่วย การฟื้นฟูร่างกายภายหลังกลับบ้น การขอความช่วยเหลือและ ขอคำปรึกษาจากเจ้าหน้ที่ภายหลังกลับบ้าน โดยการติดต่อทางโทรศัพท์
The objective of this study was to systematically review models and methods of giving information for patients before receiving operations. The study was conducted by searching electronic databases and hand searched of published studies during 2008 - 2012. Ten studies were met the inclusion criteria, and these were four research papers, two theses, and four independent studies. According to evaluation criteria of The Royal College of Physicians of Thailand, these studied were grouped into three levels: A - level (1) s, B – level (8), and C- level (1). The results showed that preoperative information was given to six groups of surgical patients: general surgery, Diabetic foots surgery, hip and knee prosthesis surgery, CA esophagus surgery, spine surgery, and open heart surgery. The contents of this information included how to treat patients before and after operation, for examples, how the patients take care of themselves while they were treated, how to relief their pain, and how patients moved after surgery. An information given model was as follows. A health team gave information to the patient individually, and mostly nurses did. They explained and used teaching media and manuals. Patients were given information according to their specific surgery as follows. 1) How to cope with stress by using relaxation technique was given to general surgery patients. 2) Diabetes and complications of feet were given to patients with Diabetic foots surgery. 3) Four topics of health information were given to patients with hip and knee prosthesis surgery. The topics comprised physiology and functions of hip and knee, total hip replacement, knee replacement, physiotherapy, rehabilitation, and exercises. 4) How patients took care of themselves while staying at hospitals was given to patients with CA esophagus surgery. 5) Basic physiology of the spinal cord, lung exercises by using Incentive spirometry, how to use Sequential Compression Device (SCD), and using back support were given to patients with spine surgery. Finally, 6) duration of time staying in ICU after surgery, rehabilitation after discharge, asking for help and consultation from health staff after discharge by using telephone were given to patients with open heart surgery
