Abstract
การวิจัยกึ่งทดลองนี้ มีวัตถุประสงค์เพื่อเปรียบเทียบความรู้ การรับรู้เกี่ยวกับโรคไตเรื้อรัง พฤติกรรมการปฏิบัติตัวเพื่อชะลอไตเสื่อม อัตราการกรองของไต ระดับความดันโลหิต และระดับน้ำตาลในเลือดของกลุ่มทดลองระหว่างก่อนและหลังใช้รูปแบบฯ และระหว่างกลุ่มทดลองและกลุ่มเปรียบเทียบในระยะหลังใช้รูปแบบฯ กลุ่มตัวอย่างเป็นผู้ป่วยโรคไตเรื้อรังระยะที่ 3 (อัตราการกรองของไต 30-59 มิลลิลิตรต่อนาทีต่อ 1.73 ตารางเมตร) อายุ 30-70 ปี มารับบริการที่คลินิกโรคเรื้อรังโรงพยาบาลส่งเสริมสุขภาพตำบลวังยาง (กลุ่มทดลอง) โรงพยาบาลส่งเสริมสุขภาพตำบลบ้านทุ่งยาว และโรงพยาบาลส่งเสริมสุขภาพตำบลวังโพรง (กลุ่มเปรียบเทียบ) อำเภอเนินมะปราง จังหวัดพิษณุโลก ซึ่งได้จากการเลือกแบบเจาะจงตามเกณฑ์ในการศึกษาโดยแบ่งเป็นกลุ่มทดลองและกลุ่มเปรียบเทียบกลุ่มละ 27 คน เครื่องมือที่ใช้ในการทดลองเป็นรูปแบบการชะลอไตเสื่อมในผู้ป่วยโรคไตเรื้อรังระยะที่ 3 ที่ผู้วิจัยพัฒนาขึ้นโดยใช้แนวคิดแบบจำลองพรีสีด-โพรสีด เครื่องมือที่ใช้ในการเก็บรวบรวมข้อมูล เป็นแบบสอบถาม 5 ส่วน ได้แก่ 1) ข้อมูลทั่วไปและข้อมูลทางคลินิก 2) ความรู้เรื่องโรคไตเรื้อรัง 3) การรับรู้โอกาสเสี่ยงเกี่ยวกับโรคไตเรื้อรัง 4) การรับรู้ความรุนแรงของการเกิดภาวะแทรกซ้อน และ 5) พฤติกรรมการปฏิบัติตัวเพื่อชะลอไตเสื่อม มีค่าความเที่ยงของแบบสอบถามส่วน 2, 3, 4 และ 5 เท่ากับ 0.715, 0.742, 0.832 และ 0.785 ตามลำดับ วิเคราะห์ข้อมูลด้วยสถิติพรรณนา สถิติทดสอบที สถิติวิลคอกซันซายน์แรงค์ และสถิติแมนวิทนีย์ยู ผลการศึกษาพบว่า 1) กลุ่มทดลองมีความรู้และการรับรู้โอกาสเสี่ยงเกี่ยวกับโรคไตเรื้อรัง พฤติกรรมการปฏิบัติตัวเพื่อชะลอไตเสื่อมหลังใช้รูปแบบฯ สูงกว่าก่อนใช้รูปแบบฯ และค่าเฉลี่ยความดันโลหิตซีสโตลิกและ ไดแอสโตลิกต่ำกว่าก่อนใช้รูปแบบอย่างมีนัยสำคัญทางสถิติที่ระดับ .05 แต่การรับรู้ความรุนแรงของโรคไตเรื้อรัง อัตราการกรองของไตและระดับน้ำตาลในเลือดของกลุ่มทดลองก่อนและหลังใช้รูปแบบฯ ไม่แตกต่างกันอย่างมีนัยสำคัญทางสถิติที่ระดับ .05 2) หลังใช้รูปแบบฯ กลุ่มทดลองมีความรู้ การรับรู้โอกาสเสี่ยงและการรับรู้ความรุนแรงเกี่ยวกับโรคไตเรื้อรัง พฤติกรรมการปฏิบัติตัวเพื่อชะลอไตเสื่อม และอัตราการกรองของไตสูงกว่ากลุ่มเปรียบเทียบอย่างมีนัยสำคัญทางสถิติที่ระดับ .05 และมีค่าเฉลี่ยความดันโลหิตซีสโตลิกและไดแอสโตลิกต่ำกว่ากลุ่มเปรียบเทียบอย่างมีนัยสำคัญทางสถิติที่ระดับ .05 แต่ระดับน้ำตาลในเลือดของกลุ่มทดลองและกลุ่มเปรียบเทียบไม่แตกต่างกันอย่างมีนัยสำคัญทางสถิติที่ระดับ .05
The objectives of this quasi-experimental research were to compare knowledge and perception of chronic kidney disease (CKD), self care behaviors to slow kidney failure, glomerular filtration rate (GFR), levels of blood pressure and fasting blood sugar (FBS) of patients with stage 3 CKD before and after experiment in the experimental group and between experimental and comparison group after experiment. The sample comprised patients with stage 3 CKD (eGFR 30 - 59 ml /min /1.73 m2) who attended at chronic disease clinic in Wangyang health promoting hospital (experimental group) and Banthungyow with Wangprong Health Promoting hospital (comparison group) Neonmaprang District, Phitsanulok Province. They were selected by the purposive sampling technique as the criteria into the experimental group (n = 27) and the comparison group (n = 27) respectively. The experimental tool was the model to slow the progression of patients with stage 3 chronic kidney disease based on the PRECEDE-PROCEED model. The data collection tool was questionnaire with five parts: 1) general data and clinical data, 2) knowledge of CKD, 3) perception about risk of CKD, 4) perception of the severity of complications, and 5) self care behaviors to slow kidney failure. The reliability of parts 2-5 were 0.715, 0.742, 0.832, and 0.785 respectively. Data were analyzed by descriptive statistics, t-test, Wilcoxon signed rank test, and Mann-Whitney U test. The research findings were as follows. 1) Knowledge and perception of risk of CKD, self care behaviors to slow kidney failure of the experimental group after experiment were significantly higher than before (p < .05) and the levels of systolic and diastolic blood pressure after the experiment were significantly lower than before (p<.05). There was no statistical significant difference of the perception in the severity of complications, GFR and the level of FBS of the experimental group between before and after the experiment (p >.05). 2) After the experiment, knowledge and perception of CKD, self care behaviors to slow kidney failure and GFR of the experimental group were significantly higher than the comparison group (p < .05) and the levels of systolic and diastolic blood pressure were significantly lower than the comparison group (p < .05). There was no statistical significant difference of the level of FBS in the experimental group and the comparison group after the experiment (p > .05).
