ประสิทธิผลการประยุกต์ใช้ทฤษฎีลีนในระบบบริการพยาบาลผู้ป่วยนอกโรงพยาบาลประจวบคีรีขันธ์

Abstract

การศึกษานี้เป็นการวิจัยกึ่งทดลอง โดยนำทฤษฎีลีนมาประยุกต์ในระบบบริการพยาบาลผู้ป่วยนอกโรงพยาบาลประจวบคีรีขันธ์ มีวัตถุประสงค์เพื่อเปรียบเทียบระยะเวลาและขั้นตอนการให้บริการพยาบาลผู้ป่ายนอก ก่อนและหลังการประยุกต์ใช้ทฤษฎีลีน และศึกษาความพึงพอใจต่อการให้บริการของผู้รับบริการและบุคลากรทางการพยาบาลหลังการประยุกต์ใช้ทฤษฎีลีน กลุ่มตัวอย่างได้แก่ ผู้รับบริการจำนวน 441 ราย ที่ได้มาจากการสุ่มแบบบังเอิญระหว่างเดือนกรกฎาคม-ตุลาคม 2555 และบุคลากรทางการพยาบาลผู้ป่วยนอก จำนวน 12 ราย เครื่องมือที่ใช้ประเมินผล ประกอบด้วย แบบบันทึกระยะเวลาและขั้นตอนการบริการพยาบาล และแบบสอบถามความพึงพอใจของผู้รับบริการและบุคลากรทางการพยาบาลได้ผ่านการตรวจความตรงโดยผู้เชี่ยวชาญจำนวน 3 ราย นอกจากนี้แบบสอบถามความพึงพอใจยังได้ผ่านการทดสอบความเที่ยงด้วยค่าสัมประสิทธิ์อัลฟาของคอนบราค ได้ค่า .923 และ .934 ตามลำดับ สำหรับสถิติที่ ใช้วิเคราะห์ ข้อมูลการประเมินผลได้แก่ ค่าร้อยละ ค่าเฉลี่ย ค่าส่วนเบี่ยงเบนมาตรฐาน และการทดสอบที่แบบเป็นอิสระต่อกัน ผลการศึกษาพบว่า การพัฒนาระบบบริการพยาบาลผู้ป่วยนอกด้วยทฤษฎีลืนสามารถขจัดขั้นตอนที่ไม่จำเป็น และรวมขั้นตอนเข้าด้วยกัน ส่งผลให้ระยะเวลาเฉลี่ยในการให้บริการหลังการประยุกต์ใช้ทฤษฎีลีนลดลงอย่างมีนัยสำคัญทางสถิติจาก 44.37 นาทีเหลือ 35.15 นาทีในคลินิกโรคทั่วไป และ 29.00 นาทีในคลินิกศัลยกรรม สำหรับความพึงพอใจของผู้รับบริการอยู่ในระดับมากส่วนความพึงพอใจของผู้ให้บริการก็อยู่ในระดับมากที่สุด ผลการศึกษานี้แสดงให้เห็นว่าการใช้ทฤษฏลีนสามารถลดความล่าช้า ความสับสนในขั้นตอนบริการพยาบาล

The study was designed by quasi-experimental research and applying Lean theory for the out-patient nursing service system at Prachuap Khiri Khan Hospital. The objectives of this study were: to compare duration and procedures of out-patient nursing services before and after applying Lean theory as well as to study the satisfaction of patients and nurses on nursing services after applying Lean theory. The samples comprised 441 patients, selected by accidental sampling technique during July to October 2012 and 12 nurses at the out-patient department. Two research instruments were used (a) record forms for recording duration and procedures of nursing services and (b) satisfaction questionnaires. These instruments were validated by three experts. The Cronbach alpha reliability coefficients of the satisfaction questionnaires in which patients and nurses parts were .923 and .934 respectively. The data were analyzed by percentage, mean, standard deviation, and independent t-test. The results showed that applying Lean theory could reduce unnecessary steps and combine some steps together. As a result, duration time of health services statistically significantly decreased from 44.37 minutes to 35.15 minutes in general clinic and 29.00 minutes in surgery clinic. In addition, patients and nurses rated their satisfaction at the high and the highest levels. This study showed that nurses can work faster and less confusion when applying Lean theory .
