การประเมินระบบจัดการยาความเสี่ยงสูงในโรงพยาบาลชุมชนจังหวัดสุราษฎร์ธานี โดยใช้เครื่องมือประเมินหาส่วนขาดและวางแผนเพื่อความปลอดภัยในการดูแลผู้ป่วย

Abstract

การวิจัยนี้เป็นการวิจัยเชิงสำรวจ มีวัตถุประสงค์เพื่อ 1) ศึกษาปัจจัยส่วนบุคคลและปัจจัยองค์กรของโรงพยาบาลชุมชน จังหวัดสุราษฎร์ธานี และ 2) เปรียบเทียบระบบจัดการยาความเสี่ยงสูงตามมาตรฐานสถาบันพัฒนาของโรงพยาบาลชุมชนที่ผ่านการรับรองคุณภาพและรอการรับรองคุณภาพของโรงพยาบาลชุมชนจังหวัดสุราษฎร์ธานี ผลการวิจัยพบ 1) ประชากรส่วนใหญ่เป็นเพศหญิง ร้อยละ 93.2 อายุเฉลี่ย 34.12 ปี (X =34.1 สำเร็จการศึกษาในระดับปริญญาตรี ร้อยละ 94.4 มีวิชาชีพดังนี้ พยาบาลวิชาชีพ ร้อยละ 84.7เภสัชกร ร้อยละ 10.6 และแพทย์ ร้อยละ 4.7 ส่วนใหญ่ปฏิบัติงานอยู่ในโรงพยาบาลชุมชนขนาด 30เตียง ร้อยละ 43.63 และเป็นโรงพยาบาลที่ผ่านการรับรองคุณภาพสถาน พยาบาลอยู่ในระดับบันได ขั้นที่ 2 มีโรงพยาบาลชุมชนที่มีมาตรฐานระบบการจัดหาความเสี่ยงสูง ที่ผ่านเกณฑ์ จำนวน 20มาตรฐานร้อยละ 24.69 และที่ไม่ผ่านเกณฑ์ อีก 61 มาตรฐาน ร้อยละ 75.31 และ 2) เมื่อเปรียบเทียบระบบการจัดยาตามมาตรฐานพบว่า โรงพยาบาลชุมชนที่มีขนาดใหญ่กว่ามีจำนวนมาตรฐานที่ผ่านเกณฑ์มากกว่าโรงพยาบาลชุมชนที่มีขนาดเล็กกว่าและพบว่าโรงพยาบาลที่ผ่านการรับรองคุณภาพโรงพยาบาลในระดับขั้น 3 มีจำนวนมาตรฐานที่ผ่านเกณฑ์มากที่สุด

This survey research was conducted in 18 community hospitals in Surat Thani province and aimed at (1) determining staff personal factors and organizational factors of the hospitals; and (2) reviewing and comparing the high-alert drug management systems in the hospitals, based on the Gap Analysis Model of the Healthcare Accreditation Institute (Public Organization). The study population included 557 doctors, pharmacists, and registered nurses at the hospitals. Data were collected between 7 February and 20 March 2011, using a questionnaire (reliability value, 0.81) containing three parts (general information including personal/organizational factors, substandard functions, and high-alert drug monitoring systems. And statistical analyses performed included the determination of percentages, means and standard deviations. The results showed that: (1) among the respondents, the majority (93.2%) were female; their mean age was 34.1 years, 94.4% having completed a bachelor's degree; 84.7% were nurses, 10.6% were pharmacists, and 4.7% were doctors; 43.6% were working at 30-bed hospitals that had passed the Level 2 Healthcare Accreditation requirements; and 24.7% of the hospitals met 20 items of the high-risk drug monitoring standards, while the other 75.3% did not; and (2) among the community hospitals, the larger hospitals met a larger number of high-risk drug requirements compared with the smaller hospitals; and those with a Level 3 Healthcare Accreditation certification met most of the high-risk drug requirements
