ปัจจัยส่งผลต่อระดับกิจกรรมการจำหน่ายผู้ป่วยด้านการจัดการทางการพยาบาล ตามแนวคิดลีน ของโรงพยาบาลตติยภูมิแห่งหนึ่ง เขตกรุงเทพมหานคร

Abstract

การวิจัยนี้ มีวัตถุประสงค์เพื่อศึกษา 1) ความรู้ในการจัดการ และระดับการบริหารของหน่วยงาน 2) ระดับกิจกรรมการจำหน่ายผู้ป่วยด้านการจัดการทางการพยาบาล และ 3) ปัจจัยที่ส่งผลกับระดับกิจกรรมการจำหน่ายผู้ป่วยด้านการจัดการทางการพยาบาลตามแนวคิดลีนของโรงพยาบาลตติยภูมิแห่งหนึ่ง เขตกรุงเทพมหานครการวิจัยนี้เป็นการวิจัยเชิงสำรวจแบบภาคตัดขวางมีประชากรที่ศึกษาคือ พยาบาลวิชาชีพที่ปฏิบัติงานมากกว่า 6 เดือน ทั้งหมด 1,093 คน คำนวณจำนวนตัวอย่างด้วยวิธีของยามาเน่ได้เท่ากับ 293 คน สุ่มเก็บตัวอย่างด้วยวิธีการสุ่มแบบชั้นภูมิอย่างเป็นสัดส่วน จากนั้นทำการสุ่มเลือกตัวอย่างจากประชากรในแต่ละหอผู้ป่วยด้วยวิธีสุ่มอย่างง่าย เครื่องมือที่ใช้ในการวิจัยคือ 1) แบบสอบประเมินความรู้ในการจัดการทางการพยาบาลตามแนวคิดลีน ได้ค่าความตรงทั้งฉบับคือ 1.00 ได้ค่าความเที่ยง 0.72 2) แบบประเมินระดับการบริหารโรงพยาบาลโดยโครงสร้างตามแนวคิดของอองรี ฟาโยล ได้ค่าความตรง ทั้งฉบับคือ 0.90 ได้ค่าความเที่ยง 0.98 และ 3) แบบประเมินระดับกิจกรรมการจำหน่ายผู้ป่วยด้านการจัดการทางการพยาบาลตามแนวคิดลีน ได้ค่าความตรงทั้งหมดฉบับคือ 1.00 ได้ค่าความเที่ยง 0.92 วิเคราะห์ข้อมูลโดยใช้สถิติการแจกแจงความถี่ ร้อยละ ค่าเฉลี่ย ส่วนเบี่ยงเบนมาตรฐาน และการถดถอยพหุคูณเชิงเส้นด้วยวิธีการคัดเลือกตัวแปรตามแบบขั้นตอนผลวิจัยพบว่า 1) ความรู้ในการจัดการทางการพยาบาลตามแนวคิดลีนของพยาบาลวิชาชีพ อยู่ในระดับปานกลาง (x̄= 2.09, S.D.= 0.82) ระดับการบริหารของหน่วยงานอยู่ในระดับมาก (x̄=3.97, S.D.= 0.61) 2) ระดับกิจกรรมการจำหน่ายผู้ป่วยด้านการจัดการทางการพยาบาลตามแนวคิดลีนอยู่ในระดับมาก (x̄=18.49, S.D.= 3.89) และ 3) ระดับการบริหารของหน่วยงานด้านการวางแผน (นโยบายและแผน) ด้านการควบคุม (การประเมินผล) ส่งผลเชิงบวก และด้านการประสานงาน (บรรยากาศในการทำงาน) ส่งผลเชิงลบต่อระดับกิจกรรมการจำหน่าย อย่างมีนัยสำคัญทางสถิติที่ 0.05 โดยมีค่าพยากรณ์เท่ากับร้อยละ 19.3 (R2= 0.193)

The purposes of this study were to explore (1) the knowledge and level of nursing management, (2) the level of implementation of nursing discharge plans, and (3) the factors affecting the level of implementation of a nursing discharge plan, all based on the Lean concept at a tertiary care hospital in Bangkok.This cross-sectional survey research was conducted in a sample of 293 registered nurses (RNs) selected from all 1,093 RNs who had been working at the hospital for more than 6 months. The number of samples was calculated using the method of Yamane, which involved proportional stratified random sampling and simple random sampling. 1) The research tools used included a Lean-based nursing management assessment questionnaire with a validity of 1.00 and a reliability values of 0.72, 2) the Henri Fayol hospital management questionnaire with validity and reliability values of 0.90 and 0.98, respectively, and 3) the lean-based hospital management activity questionnaire with validity and reliability values of 1.00 and 0.92, respectively. Statistical analysis was undertaken to determine frequency distribution, percentage, mean, standard deviation and linear multiple regression using the stepwise variable selection method.The results revealed that, among the RNs respondents: (1) their knowledge of nursing management based on the Lean concept was at a moderate level (x̄ = 2.09, S.D.= 0.82); and their organization management capacity was at a high level (x̄ = 3.97, S.D. = 0.61); (2) their Lean-based implementation of the nursing discharge plans was at a high level (x̄ =18.49, S.D.=3.89); and (3) the organization management factors regarding planning (policies and plans) and controlling (evaluation) had a positive effect, but coordinating (working environment) had a negative effect, significantly on the implementation level of Lean-based nursing discharge plans (P≤ 0.05) with a predictive value of 19.3% (R2 = 0.193).
