Abstract
การวิจัยเชิงพรรณานี้มีวัตถุประสงค์ เพี่อศึกษาต้นทุนต่อหน่วยบริการของโรงพยาบาลเปือยน้อย จังหวัดขอนแก่น ปีงบประมาณ 2550 การศึกษาต้นทุนโดยการเก็บข้อมูลการบริการทั้งหมดของโรงพยาบาลย้อนหลังตั้งแต่ 1 ตุลาคม 2549 ถึง 30 กันยายน 2550 วิธีวิเคราะห์ต้นทุนด้วยการแบ่งหน่วยต้นทุนออกเป็น 3 ประเภท ได้แก่ (1) หน่วยต้นทุนที่ไม่ก่อให้เกิดรายได้ (2) หน่วยต้นทุนที่ก่อให้เกิดรายได้ และ (3) หน่วยต้นทุนที่ให้บริการผู้ป่วย การกระจายต้นทุนรวมของหน่วยต้นทุนไม่ก่อให้เกิดรายได้และหน่วยต้นทุนที่ก่อให้เกิดรายได้มายังหน่วยต้นทุนที่ให้บริการผู้ป่วย ใช้วิธีสมการเส้นตรงต้นทุนทั้งหมดของหน่วยต้นทุนที่ให้บริการผู้ป่วยเมื่อหารด้วยจำนวนครั้งผู้ป่วยนอก ครั้งผู้ป่วยอุบัติเหตุฉุกเฉิน และวันนอนของผู้ป่วยในตานแผนกของผู้ป่วยจะได้ต้นทุน
ต่อหน่วยบริการต่อครั้งผู้ป่วยนอก ต่อครั้งผู้ป่วยอุบัติเหตุฉุกเฉิน และต่อวันนอนของผู้ป่วยใน ผลการศึกษาพบว่าต้นทุนทั้งหมดของหน่วยบริการผู้ป่วเท่ากับ 24,599,979.14 บาท ประกอบด้วย ต้นทุนรวมทางตรงเท่ากับ 12,443,207.47 บาท และต้นทุนทางอ้อมเท่ากับ 12,156,771.67 บาท ต้นทุนต่อหน่วยบริการผู้ป่วยนอกมีค่าเท่ากับ 231.11 บาทต่อครั้ง จำแนกเป็นต้นทุนบริการพื้นราน 141-75 บาท และต้นทุนรักษาพยาบาล 89.36 บาท ต้นทุนอุบัติเหตุฉุกเฉิน มีค่าเท่ากับ 445.71 บาทต่อครั้ง จำแนกเป็นต้นทุนบริการพื้นฐาน 366.50 บาท และต้นทุนรักษาพยาบาล 79.21 บาท ต้นทุนผู้ป่วยในมีค่าเท่ากับ 1,473.39 บาท ต่อวันนอนจำแนกเป็นต้นทุนบริการพื้นฐาน 1,364.45 บาท และต้นทุนรักษาพยาบาล 108.94 บาท ข้อเสนอแนะจากการศึกษาผู้บริหารควรนำผลการวิจัยไปใช้ในการจัดทำแผนการดำเนินงานเพื่อให้การปฏิบัติงานการให้บริการดูแลสุขภาพดีขึ้นการกำหนดบโยบายบประหยัด การจัดการบัญชียาและเวชภัณฑ์สำหรับยาที่ใช้และคงคลัง กำหนดเกณฑ์มาตรฐานสำหรับการสั่งยาและเวชภัณฑ์ นอกจากนี้ควรจัดตั้งคณะกรรมการการจัดการฐานข้อมูลด้านการเงินและการใช้ทรัพยากรของโรงพยาบาล เพื่อพัฒนาคุณภาพการบริการดูแลรักษาสุขภาพของทุกหน่วยงานของโรงพยาบาลอย่างมีประสิทธิภาพและประสิทธิผล
The objective of this retrospective descriptive research was to estimate the unit cost of health care service at Puainoi hospital in Khon Kean province duringy The hospital data was all collected from 1 October 2006 to 30 September 2007. The cost units of the hospital were separated into 3 types of cost centers; (1) non-revenue producing cost center (NRPCC), (2) revenue producing cost center (RPC C), and (3) patient service (PS). The total cost of non-revenue producing cost center and revenue producing cost center were allocated to patient by simultaneous equation method. The total cost of patient service- was divided by the number of patient visits, emergency visits, and hospital days for obtained the unit cost ner out patient visit, per emergency visit, and per hospital day. The findings in this study showed that the full cost of Puainoi hospital was 24,599,979.07 baht which the total direct cost and the total indirect cost were 12,443,207.47 baht and 12,156,771.67 baht respectively. The unit cost of out-patient service was 231.11 baht per visit consisted of 141.75 baht in routine service cost and 89 36 bZt in medical care cost. The unit cost of the emergency unit service was 445.71 baht per visit consisted of 366.50 baht in routine service cost and 79.21 baht in medical care cost. The unit cost of in-patient service was 1,473.39 baht per day consisted of 1,364.45 baht in routine service cost and 108.94 baht in medical care From this research it is recommended that the administrators should results to formulate operational plan for better performance in providing l^alth care service. Setting; economical public policy and defined medical and phannaceut ca supply management, defined standard criteria for medical and pharmaceutcal ordering. Appointing committee of database information to establish a financial and other resource recording system.
