Abstract
การวิจัยครั้งนี้มีวัตถุประสงค์เพื่อศึกษา (1) การกระจายอำนาจด้านสาธารณสุขให้กับ องค์กรปกครองส่วนท้องถิ่น (2) ปัญหาและอุปสรรคในการถ่ายโอนภารกิจด้านสาธารณสุขให้กับ องค์กรปกครองส่วนท้องถิ่น อำเภอหลังสวน จังหวัดชุมพร และ(3) แนวทางแก้ไขปัญหาในการ ถ่ายโอนภารกิจด้านสาธารณสุขให้กับองค์กรปกครองส่วนท้องถิ่น อำเภอหลังสวน จังหวัดชุมพร การวิจัยนี้เป็นการวิจัยเชิงคุณภาพ มีประชากร 5 กลุ่ม ประกอบด้วย ผู้บริหารฝ่าย การเมือง ข้าราชการส่วนท้องถิ่นที่ปฏิบัติงานสาธารณสุข ผู้บริหารหน่วยบริการสังกัดกระทรวงสาธารณสุข ผู้บริหารหน่วยงานราชการส่วนภูมิภาคระดับจังหวัด อำเภอ และประชาชน คัดเลือก กลุ่มตัวอย่างแบบเฉพาะเจาะจง จำนวน 20 คน สัมภาษณ์เชิงลึก วิเคราะห์ข้อมูลเชิงพรรณนา ผลการศึกษา พบว่า (1) การกระจายอำนาจด้านสาธารณสุขให้กับองค์กรปกครอง ส่วนท้องถิ่นมีความก้าวหน้าน้อยมาก แม้มีการกำหนดขั้นตอนไว้ในกฎหมายที่เกี่ยวข้อง และการปฏิบัติงานตามภารกิจที่กำหนดให้ถ่ายโอนยังมีความซ้ำซ้อนและไม่เป็นไปตามแผนการกระจาย อำนาจที่กำหนดไว้ (2) ปัญหาและอุปสรรคในการถ่ายโอนภารกิจด้านสาธารณสุข เกิดจากกระทรวงสาธารณสุข ไม่ได้กำหนดเรื่องการกระจายอำนาจเป็นนโยบายหลัก ไม่ได้ให้ความสำคัญ ขาดความจริงใจในการกระจายอำนาจ รวมทั้งไม่ส่งเสริมให้มีการกระจายอำนาจ โดยองค์กร ปกครองส่วนท้องถิ่นขนาดเล็ก ยังไม่มีความพร้อมในด้านโครงสร้างและบุคลากรด้านสาธารณสุข (3) แนวทางแก้ไขปัญหา คือ รัฐ ควรมีความจริงใจในการกระจายอำนาจด้านสาธารณสุข สร้าง ความเข้าใจกับภาคส่วนที่เกี่ยวข้อง นำเสนอตัวอย่างพื้นที่ที่ประสบความสำเร็จในการถ่ายโอน ภารกิจ มีหน่วยงานพัฒนาความรู้ด้านวิชาการด้านสาธารณสุข และควรปรับรูปแบบการจัดสรร งบประมาณตามจำนวนประชากรให้กับท้องถิ่นโดยตรง
The objectives of this research were to study: (1) the decentralization of public health administration authority to local administrative organizations; (2) obstacles to and problems involved with the transfer of public health tasks to local administrative organizations in Langsuan District, Chumphon Province; and (3) approaches to solving those problems. This was a qualitative research based on in-depth interviews with 20 key informants, chosen through purposive sampling, who represented the 5 sample groups of government administrators, local government employees involved in public health work, administrators of service units under the Ministry of Public Health, administrators of provincial-or district-level government agencies, and members of the general public. The data were analyzed through descriptive analysis. The results showed that (1) Very little progress has been made in the decentralization of public health administration authority to local administrative organizations in Langsuan District, even though the steps are elucidated in the relevant laws. In practice, there is still redundancy in many public health tasks and the tasks are not carried out as specified in the decentralization plan. (2) Some of the obstacles to the decentralization of public health authority are that the Ministry of Public Health has not set this as a priority policy, does not consider it important, and does not promote and encourage decentralization. Also, the local administrative organizations are not well prepared because they are small and lack sufficient public health personnel and infrastructure. (3) Approaches to solving the problems are to make the government more sincere in its efforts to decentralize power, to build greater understanding among stakeholders, to learn from the examples of other districts that have been more successful at public health decentralization, to set up an agency in charge of developing public health knowledge, and to change the method of budget allocation to make the budget match the population of each local area.
