Abstract
การวิจัยและพัฒนานี้มีวัตถุประสงค์เพื่อ 1) ศึกษาความต้องการการพัฒนาสมรรถนะการดูแลผู้ป่วยทารกแรกเกิดของพยาบาลวิชาชีพในหน่วยการพยาบาลผู้ป่วยทารกแรกเกิดศูนย์การแพทย์ปัญญานันทภิกขุชลประทาน 2) พัฒนารูปแบบการพัฒนาสมรรถนะการดูแลผู้ป่วยทารกแรกเกิดและ 3) ศึกษาความเหมาะสมของรูปแบบการพัฒนาสมรรถนะการดูแลผู้ป่วยทารกแรกเกิดกลุ่มตัวอย่างแบ่งตามระยะการพัฒนารูปแบบ ได้แก่ ระยะที่ 1 การศึกษาความต้องการการพัฒนาสมรรถนะการดูแลผู้ป่วยทารกแรกเกิดกลุ่มตัวอย่างเป็นพยาบาลวิชาชีพที่ปฏิบัติงานในหน่วยการพยาบาลผู้ป่วยทารกแรกเกิดจำนวน 6 คนระยะที่ 2 การพัฒนารูปแบบการพัฒนาสมรรถนะการดูแลผู้ป่วยทารกแรกเกิดกลุ่มตัวอย่างเป็นพยาบาลวิชาชีพและกุมารแพทย์ที่ปฏิบัติงานในหน่วยการพยาบาลผู้ป่วยทารกแรกเกิดจำนวน 5 คนและระยะที่ 3 การศึกษาความเหมาะสมของรูปแบบการพัฒนาสมรรถนะการดูแลผู้ป่วยทารกแรกเกิดกลุ่มตัวอย่างเป็นพยาบาลวิชาชีพที่ปฏิบัติงานในหน่วยการพยาบาลผู้ป่วยทารกแรกเกิดจำนวน 13 คนกลุ่มตัวอย่างทั้ง 3 ระยะถูกเลือกแบบเจาะจงเครื่องมือการวิจัย ได้แก่ 1) แนวทางสนทนากลุ่มเพื่อศึกษาความต้องการการพัฒนาสมรรถนะการดูแลผู้ป่วยทารกแรกเกิด 2) แนวทางสนทนากลุ่มเพื่อพัฒนารูปแบบฯ 3) แบบสอบถามความเหมาะสมของรูปแบบฯ มีค่าสัมประสิทธิ์ครอนบาคอัลฟา 96 วิเคราะห์ข้อมูลเชิงปริมาณโดยใช้สถิติพรรณนาและข้อมูลเชิงคุณภาพโดยการวิเคราะห์เนื้อหาผลการวิจัยพบว่า 1) พยาบาลวิชาชีพมีความต้องการและเห็นว่ามีความจำเป็นในการพัฒนาสมรรถนะการดูแลผู้ป่วยทารกแรกเกิดของพยาบาลวิชาชีพ 2) รูปแบบกิจกรรมการพัฒนาสมรรถนะประกอบด้วยการพัฒนาสมรรถนะหลัก ได้แก่ ความรักในวิชาชีพพยาบาลการทำงานเป็นทีมการประสานงานการสื่อสารทักษะการตัดสินใจบุคลิกภาพในการบริการความสามารถในการบริหารจัดการและการคิดสร้างสรรค์นวัตกรรมและสมรรถนะเฉพาะ ได้แก่ ความรู้ความสามารถที่ทันสมัยการปฏิบัติการพยาบาลและหัตถการสำคัญการพยาบาลผู้ป่วยทารกแรกเกิดในการใช้เครื่องช่วยหายใจการจัดสิ่งแวดล้อมและป้องกันการติดเชื้อการให้สารน้ำสารอาหารทางหลอดเลือดดำการบริหารยาการวางแผนจำหน่ายการใช้อุปกรณ์และเครื่องมือทางการแพทย์ที่สำคัญและความสามารถในการสอนและ 3) รูปแบบการพัฒนาสมรรถนะการดูแลผู้ป่วยทารกแรกเกิดมีความเหมาะสมอยู่ในระดับมาก
This research and development study aimed: l)to identify the needs of neonatal nursing competency development of professional nurses at Neonatal Intensive Care Unit, Panyananthaphikkhu Chonprathan Medical Center, Nonthaburi province, 2) to develop a model of neonatal nursing competency development, and 3) to study the appropriateness of neonatal nursing competency development model. The sample was divided according to the study phases. 1) The study of the needs of neonatal nursing competency development, the sample was 6 neonatal professional nurses at Neonatal Intensive Care Unit. 2) The model development of neonatal nursing competency development, the sample was 4 professional nurses and a pediatrician, and 3) the study of the appropriateness of neonatal nursing competency development model, the sample was 13 professional nurses. The sample in all phases was selected by purposive sampling. The research tools comprised: 1) a focus group guideline for a study of needs of neonatal nursing competency development, 2) a focus group guideline for model development, 3) questionnaires of the appropriateness of development model. The reliability coefficients were .96. Quantitative data were analyzed by descriptive statistics. Qualitative data were analyzed by content analysis. The results revealed that 1) neonatal nurse practitioners confirmed the needs of neonatal nursing competency development. 2) The model activities comprised of core competencies development: loving of the nursing profession, team working, coordination, communication, decision-making skills, service personality, management competency, and innovation creative idea. The specific competencies include update knowledge, significant nursing care and procedures, nursing care for neonatal with the respirator, environmental arrangement and infection prevention, intravenous infusion, drug administration, discharge planning, using of medical equipment and tools, and teaching ability. 3) The appropriateness of neonatal nursing competency development model was rated at the high level.
