ผลของรูปแบบการดูแลผู้ป่วยโรคปอดอุดกั้นเรื้อรังในชุมชนหลังจำหน่ายจากโรงพยาบาลสุไหงปาดี จังหวัดนราธิวาส

Abstract

การวิจัยแบบกึ่งทดลองนี้ มีวัตถุประสงค์เพื่อศึกษาผลของการใช้รูปแบบการดูแลผู้ป่วย โรคปอดอุดกั้นเรื้อรังในชุมชน หลังจำหน่ายจากโรงพยาบาลสุไหงปาดี จังหวัดนราธิวาส ต่ออัตราการได้รับการเยี่ยมบ้าน อัตราการกำเริบของโรคเฉียบพลัน และอัตราการเข้ารักษาซ้ำก่อน 28 วัน กลุ่มตัวอย่างที่ใช้ในการศึกษาครั้งนี้ได้แก่ อาสาสมัครสาธารณสุขประจำหมู่บ้าน ใน อำเภอสุไหงปาดี หมู่บ้านละ 1 คน จำนวน 5 คน และผู้ป่วยโรคปอดอุดกั้นเรื้อรังที่เข้ารับการรักษา ในแผนกผู้ป่วยในและจำหน่ายจากโรงพยาบาลสุไหงปาดี จำนวน 41 คน เป็นกลุ่มทดลอง 17 คน ที่ได้รับรูปแบบการดูแลผู้ป่วยโรคปอดอุดกั้นเรื้อรังในชุมชน และกลุ่มควบคุม 24 คน ที่ได้รับการดูแลตามปกติ เครื่องมือที่ใช้ในการวิจัย ได้แก่ คู่มือการดูแลสุขภาพผู้ป่วยโรคปอดอุดกั้นเรื้อรังสำหรับอาสาสมัครสาธารณสุขประจำหมู่บ้าน (อสม.) แบบทดสอบความรู้เกี่ยวกับการดูแลผู้ป่วยโรคปอดอุดกั้นเรื้อรังหลังจำหน่าย และสมุดบันทึกประจำตัวผู้ป่วยโรคปอดอุดกั้นเรื้อรัง ซึ่งผู้วิจัยสร้างขึ้น ผ่านการตรวจสอบความตรงตามเนื้อหาโดยผู้ทรงคุณวุฒิ 5 ท่าน วิเคราะห์ข้อมูลด้วยสถิติเชิงพรรณนา ได้แก่ จำนวน ร้อยละ ค่าเฉลี่ย ส่วนเบี่ยงเบนมาตรฐาน และ Z- test ผลการวิจัยพบว่า 1) อัตราการได้รับการเยี่ยมบ้านของผู้ป่วยโรคปอดอุดกั้นเรื้อรังในกลุ่มทดลองที่สูงกว่า ผู้ป่วยในกลุ่มควบคุม อย่างมีนัยสำคัญทางสถิติ (P<0.001) 2) อัตราการกำเริบของโรคเฉียบพลันของผู้ป่วยโรคปอดอุดกั้นเรื้อรัง ในกลุ่มทดลอง และในกลุ่มควบคุมไม่แตกต่างกันอย่างมีนัยสำคัญทางสถิติ (P<0.05) และ 3) อัตราการเข้ารักษาซ้ำก่อน 28 วัน ของผู้ป่วยโรคปอดอุดกั้นเรื้อรัง ในกลุ่มทดลองต่ำกว่าผู้ป่วยในกลุ่มควบคุมอย่างมีนัยสำคัญทางสถิติ (P<0.001)

The purposes of this quasi-experimental research were: to examine the effects of a caring model for patients with chronic obstructive pulmonary disease (COPD) in communities after discharged from Sungaipadi Hospital, Narathiwat Province on home health care rate, acute exacerbations rate, and readmission rate. The sample consisted of fifty Village Health Volunteers of Sungai padi district, Narathiwat Province and forty-one chronic obstructive pulmonary disease patients who were discharged from Sungaipadi Hospital. The 24 patients were assigned into two groups: the control group (24) and the experimental group (17). The research instruments, developed by the researcher, were a guideline of home health care for COPD patients, and this guideline was used by health volunteers, a knowledge test about caring for patients with COPD, and a personal health record for each COPD patient. Content validity of the instruments was examined by five experts. Data were analyzed by percentage, mean, standard deviation, and Z- test. The results showed as follows. 1) The home health care rate of COPD patients in the experimental group was significantly higher than those in the control group 2) There was no significantly different in the acute exacerbations rate of the patients in both experimental group and control group 3) The readmission rate of the patients in the experimental group was significantly lower than those in the control group.
