Abstract
การวิจัยประเมินผลในครั้งนี้ใช้แบบจำลองซิปป์ มีวัตถุประสงค์เพื่อศึกษา (1) สภาพแวดล้อม ได้แก่ นโยบายการดำเนินงาน (2) ปัจจัยนำเข้าได้แก่ ความเหมาะสมเพียงพอของทรัพยากร บุคลากรงบประมาณ เครื่องมือ อุปกรณ์และการจัดการระบบดูแลระยะยาวด้านสาธารณสุข (3) กระบวนการ ได้แก่ การวางแผนดูแลรายบุคคลระยะยาวด้านสาธารณสุข การจัดบริการดูแลโดยบุคลากรสาธารถสุข การจัดบริการดูแลที่บ้าน/ชุมชน การจัดหาอุปกรณ์ทางการแพทย์ การประเมินสภาพผู้สูงอายุการติดตามประเมินผล (4) ผลผลิต ได้แก่ ความครอบคลุมการได้รับบริการตามชุดสิทธิประโยชน์ และความพึงพอใจของผู้สูงอายุต่อการดำเนินงานระบบดูแลระยะยาวด้านสาธารณสุขสำหรับผู้สูงอายุที่มีภาวะพึ่งพิง และ (3) ปัญหา อุปสรรคและข้อเสนอแนะการดำเนินงานระบบการดูแลระยะยาวด้านสาธารณสุขสำหรับผู้สูงอายุที่มีภาวะพึ่งพิง กลุ่มตัวอย่างประกอบด้วย 4 กลุ่ม ได้แก่ (1) คณะอนุกรรมการสนับสนุนการจัดบริการดูแลระยะยาวสำหรับผู้สูงอายุที่มีภาวะพึ่งพิงจำนวน 25 คนเลือกโดยวิธีเจาะจงตำบลละ 1 คน (2) ผู้จัดการการดูแลระยะยาวด้านสารารณสุขจำนวน 25 คน เลือกโดยวิธีเจาะจง ตำบลละ 1 คน (3) ผู้ช่วยเหลือดูแลผู้สูงอายุที่มีภาวะพึ่งพิง จำนวน 25 คน เลือกโดยวิธีเจาะจง ตำบลละ 1 คน และ (4) ผู้สูงอายุที่มีภาวะพึ่งพิงในเขตอำเภอเมืองนครราชสีมา จำนวน 170 คน โดยศึกษาทุกหน่วยประชากร เก็บรวบรวมข้อมูลโดยใช้ แบบสอบถามความคิดเห็น และแบบวัดความพึงพอใจ ที่มีค่าความเชื่อมั่นเท่ากับ 0.96 และ 0.84 ตามลำดับ สถิติที่ใช้ในการวิเคราะห์ ข้อมูล คือ ความถี่ ร้อยละ ค่าเฉลี่ย และส่วนเบี่ยงเบนมาตรฐาน ผลการวิจัย พบว่า (1) สภาพแวดล้อมมีนโยบายการดำเนินงานระบบการดูแลระยะยาวด้านสาธารณสุขและให้ความสำคัญต่อการดูแลผู้สูงอายุ ในภาพรวมอยู่ระดับมาก (2) ปัจจัยนำเข้าพบว่า มีทรัพยากร บุคลากร งบประมาณ เครื่องมือ อุปกรณ์ และการบริทารจัดกรระบบดูแลระยะยาวด้านสาธารณ สุขมีความเหมาะสมและเพียงพอมีบุคคลากรที่มีความรู้ความเข้าใจและมีความสามารถเอื้อต่อการจัดระบบการดูแลระยะยาวสำหรับผู้สูงอายุที่มีภาวะพึ่งพิง ในภาพรวมอยู่ระดับปานกลาง (3) กระบวนการมีการวางแผนดูแลรายบุคคลระยะยาวด้านสาธารณสุข การจัดบริการดูแลโดยบุคลากรสาธารณสุข การจัดบริการดูแลที่บ้าน/ชุมชน การจัดหาอุปกรณ์ทางการแพทย์การประเมินสภาพผู้สูงอายุ การติดตามประเมินผลการดำเนินการคัดกรองและประเมินความสามารถในการทำกิจกรรมในชีวิตประจำวันของผู้สูงอายุ การวางแผนการดูแลรายบุคคลตามชุดสิทธิประโยชน์โดยทีมผู้จัดการการดูแลระยะยาวด้านสาธารณสุข ทีมหมอครอบครัว และผู้ช่วยเหลือดูแลผู้สูงอายุ ในภาพรวมอยู่ระดับมาก (4) ผลผลิต พบว่า ความครอบคลุมการได้รับบริการตามชุดสิทธิประโยชน์และความพึงพอใจของผู้สูงอายุที่มีภาวะพึ่งพิง ในภาพรวมอยู่ระดับปานกลาง และ (5) ปัญหาและอุปสรรค พบว่า บุคลากรผู้ปฏิบัติงาน ได้แก่ ผู้จัดการการดูแลระยาวด้านสาธารณสุข ผู้ช่วยเหลือดูแลผู้สูงอายุที่มีภาวะพึ่งพิง มีจำนวนไม่เพียงพอขาดความรู้ ทักษะในการปฏิบัติงานและมีภาระงานมาก ขาดการมีส่วนร่วมของครอบครัวและ ชุมชนในการดูแลผู้สูงอายุ วัสดุอุปกรณ์ทางการแพทย์ยังไม่เพียงพอกับความต้องการ ข้อเสนอแนะ ผู้บริหารควรเพิ่มจำนวนบุคลากรผู้ปฏิบัติงานตามสัดส่วนในการดูแลผู้สูงอายุที่มีภาวะพึ่งพิง จัดทำคู่มือการปฏิบัติงานจัดฝึกอบรมฟื้นฟูความรู้และทักษะในการดูแลผู้สูงอายุที่มีภาวะพึ่งพิงอย่างต่อเนื่อง จัดตั้งกองทุนผู้สูงอายุระดับทั้องถิ่นทุกพื้นที่เพื่อผลักดันให้ชุมชนมีส่วนร่วมในการดูแลผู้สูงอายุและสนับสนุนให้มีการจัดตั้งศูนย์กายอุปกรณ์ระดับตำบล/อำเภอ เพื่อรองรับภาวะพึ่งพิงที่มีความจำเป็นต้องใช้อุปกรณ์ทางการแพทย์ต่างๆ
This evaluation research used the CIPP model and aimed to identify: (1) contexts such as implementation policy; (2) inputs such as appropriate resources, personnel, budget, tools, equipment and management of long-term health care (LTC) system; (3) processes such as individual LTC plans, services by health personnel, home/community-based care, medical equipment provision, older persons assessment, and monitoring/evaluation; (4) products such as service coverage according to the benefit package and satisfaction with LTC for dependent older persons; and (5) problems/ obstacles of and make recommendations for improving the LTC system for dependent older persons. The study samples involved four groups of respondents, including (1) 25 members, one purposively selected from each of the sub-district committees on LTC for dependent older persons, (2) 25 LTC managers, one purposively selected from each subdistrict, (3) 25 dependent older-person caregivers, one purposively selected from each subdistrict, and (4) all 170 dependent older persons who lived in Mueang Nakhon Ratchasima district. Data were collected using a questionnaire about the opinion and satisfaction with the reliability values of 0.96 and 0.84, respectively; and statistics used for data analysis were frequency, percentage, mean, and standard deviation. The results showed that, concerning LTC for dependent older persons: (1) the contexts, including overall policies on LTC system implementation focusing on dependent older persons, were at the high level; (2) the inputs including suitable and adequate resources, personnel, budget, equipment, and LTC system management as well as knowledgeable and capable personnel for elderly LTC management were at the moderate level; (3) the overall processes, including older persons’ individual LTC plans, services by health personnel, home/community-based care provision, medical equipment provision, general conditions’ assessment, monitoring/evaluation, daily living activity assessment, individual care plan as per their benefit package by a team of LTC managers, family care team members, and elderly caregivers, were at the high level; (4) the overall products, including service coverage as per their benefit package and satisfaction, were at the moderate level; and (5) major problems/obstacles were inadequate personnel, especially LTC managers and caregivers, insufficient knowledge and working skills among such personnel, high workloads, lack of family and community participation, and inadequate medical equipment/supplies. It is thus suggested that relevant executives should allocate additional staff in proportion to the number of dependent older persons, develop a practical manual, set up a refresher training program on LTC on a continual basis, establish a local older persons fund in all localities to encourage community participation in the LTC system, and set up a sub-district/district prosthetic/orthotic center for dependent older persons who are in need of such devices
