ผลของโปรแกรมการปรับเปลี่ยนพฤติกรรมการรับประทานยาในผู้ป่วยความดันโลหิตสูงที่ควบคุมระดับความดันโลหิตไม่ได้อำเภอนาหว้า จังหวัดนครพนม

Abstract

การวิจัยกึ่งทดลองนี้ มีวัตถุประสงค์เพื่อเปรียบเทียบ 1) พฤติกรรมการรับประทานยา ความร่วมมือในการรับประทานยา และระดับความดันโลหิตของกลุ่มทดลองระหว่างก่อนและหลังใช้โปรแกรมฯ 2) พฤติกรรมการรับประทานยา ความร่วมมือในการรับประทานยา และระดับความดันโลหิตหลังใช้โปรแกรมฯ ระหว่างกลุ่มทดลองและกลุ่มเปรียบเทียบ กลุ่มตัวอย่างเป็นผู้ป่วยโรคความดันโลหิตสูงที่ไม่สามารถควบคุมระดับความดันโลหิตได้ อายุ 30-65 ปี จำนวน 60 คน ซึ่งได้จากการเลือกอย่างเฉพาะเจาะจงตามเกณฑ์คัดเข้าศึกษาจากผู้ที่มารับบริการที่โรงพยาบาลส่งเสริมสุขภาพตำบลดอนศาลา อำเภอนาหว้า จังหวัดนครพนม เป็นกลุ่มทดลอง และจากโรงพยาบาลส่งเสริมสุขภาพตำบลท่าเรือเป็นกลุ่มเปรียบเทียบ จำนวนกลุ่มละ 30 คน เครื่องมือที่ใช้ในการทดลองเป็นโปรแกรมการปรับเปลี่ยนพฤติกรรมการรับประทานยาในผู้ป่วยความดันโลหิตสูงที่พัฒนาขึ้น โดยประยุกต์ใช้แนวคิดทฤษฎีขั้นตอนการเปลี่ยนพฤติกรรมของโปรซาสกาและคณะ เครื่องมือที่ใช้ในการเก็บรวบรวมข้อมูลเป็นแบบสอบถาม มี 3 ส่วน ได้แก่ ข้อมูลทั่วไป พฤติกรรมการรับประทานยา และความร่วมมือในการรับประทานยา มีค่าความเที่ยงของแบบสอบถามส่วนที่ 2 และ 3 เท่ากับ .83 และ .86 ตามลำดับ วิเคราะห์ข้อมูลโดยใช้สถิติพรรณนา สถิติการทดสอบไคสแควร์ การทดสอบที่ชนิดอิสระและไม่อิสระ การทดสอบวิลคอกซันซายน์แรงค์ และสถิติการทดสอบแมนวิทนีย์ยู ผลการศึกษาพบว่า (1) หลังใช้โปรแกรมฯ กลุ่มทดลองมีพฤติกรรมการรับประทานยา ความร่วมมือในการรับประทานยาสูงกว่าก่อนใช้โปรแกรม และระดับความดันโลหิตต่ำกว่าก่อนใช้โปรแกรมฯ อย่างมีนัยสำคัญทางสถิติที่ระดับ .05 (2) หลังใช้โปรแกรมฯ กลุ่มทดลองมีพฤติกรรมการรับประทานยา ความร่วมมือในการรับประทานยาสูงกว่ากลุ่มเปรียบเทียบ และมีระดับความดันโลหิตต่ำกว่ากลุ่มเปรียบเทียบ อย่างมีนัยสำคัญทางสถิติที่ระดับ .05

The objectives of this quasi-experimental research were to compare 1) drug use behaviors, medication adherence and blood pressure level before and after using the program in the experimental group; and 2) drug use behaviors, medication adherence and blood pressure level after using the program between the experimental group and the comparison group. For the samples, 60 patients with Uncontrolled hypertension, aged 30-65, were selected by specific inclusion criteria. The study area was Nawa District, Nakhon Phanom Province. The experimental group comprised 30 patients who got treatment at Donsala Health Promotion Hospital, and the comparison group comprised 30 patients who got treatment at Thareua Health Promotion Hospital. The tool of this experiment was a Behavioral Modification Program on Drug Adherence Among Uncontrolled Hypertension Patients based on the Transtheoretical Theory. Study period was 8 weeks. The data collection tool was a questionnaire composed of 3 parts: 1) general information, 2) drug use behaviors and 3) medication adherence. The reliability of parts 2 and 3 were .83 and .86, respectively. Data were analyzed by descriptive statistics, independent and dependent t-test, Wilcoxon Signed Ranks test, and Mann-Whitney U test. The results found as follows. 1) After using the program, the experimental group scored significantly higher on drug use behaviors and medication adherence, and had lower blood pressure levels than before using the program (p < .05). 2) After using the program, the experimental group had significantly higher on drug use behaviors and medication adherence, and had lower blood pressure levels than the comparison group (p < .05).
