ความสัมพันธ์ระหว่างการจัดการเชิงกลยุทธ์ของหัวหน้าหอผู้ป่วยและความไว้วางใจในองค์กรพยาบาลกับประสิทธิผลของหอผู้ป่วยตามการรับรู้ของพยาบาลวิชาชีพประจำการโรงพยาบาลชุมชน เขตตรวจราชการสาธารณสุขที่ 13

Abstract

การวิจัยครั้งนี้เป็นการวิจัยเชิงอรรถาธิบายมีวัตถุประสงค์เพื่อศึกษา (1) การจัดการเชิงกลยุทธ์ของหัวหน้าหอผู้ป่วย ความไวัวางใจในองค์กรพยาบาลและประสิทธิผลของหอผู้ป่วยตามการรับรู้ของพยาบาลวิชาชีพ ประจำการโรงพยาบาลชุมชน เขตตรวจราชการสาธารณสุขที่ 13 (2) ปัจจัยที่มีอิทธิพลในการพยากรณ์ประสิทธิผลของหอผู้ป่วยตามการรับรู้ของพยาบาลวิชาชีพประจำการโรงพยาบาลชุมชน เขตตรวจราชการ สาธารณสุขที่ 13 กลุ่มตัวอย่างคือ พยาบาลวิชาชีพที่มีระยะเวลาปฏิบัติงานตั้งแต่ 1 ปี ขึ้นไปในโรงพยาบาลชุมชนเขตตรวจราชการสาธารณสุขที่ 13 จำนวน 107 คน ได้จากการสุ่มตัวอย่างแบบแบ่งชั้น (Stratified random sampling) เครื่องมือที่ใช้คือแบบสอบถามแบ่งเป็น 4 ตอน คือข้อมูลส่วนบุคคลการจัดการเชิงกลยุทธ์ของหัวหน้าหอผู้ป่วยความไว้วางใจในองค์กรพยาบาล และประสิทธิผลของหอผู้ป่วย ผ่านการตรวจสอบความตรงตามเนื้อหาจากผู้ทรงคุณวุฒิ จำนวน 5 ท่าน ได้ค่า CV1 ของแบบสอบถามตอนที่ 2-4 เท่ากับ 0.97, 0.97 และ 0.91 ตามลำดับ และวิเคราะห์ความเที่ยงได้ค่าสัมประสิทธิ์อัลฟ่าของครอมบาค เท่ากับ 0.99, 0.97 และ 0.98 ตามลำดับ วิเคราะห์ข้อมูลโดยใช้สถิติ ร้อยละ ค่าเฉลี่ย ส่วนเบี่ยงเบนมาตรฐาน และวิเคราะห์การถดถอยพหุคูณแบบเพิ่มตัวแปรเป็นขั้นตอน ผลการวิจัยพบว่า (1) การจัดการเชิงกลยุทธ์ของหัวหน้าหอผู้ป่วย ความไวัวางใจในองค์กรพยาบาลอยู่ในระดับปานกลาง และประสิทธิผลของหอผู้ป่วยอยู่ในระดับสูง (2) การจัดการเชิงกลยุทธ์ของหัวหน้าหอผู้ป่วย และความไวัวางใจในองค์กรพยาบาลมีความสัมพันธ์ทางบวกกับประสิทธิผลของหอผู้ป่วย ในระดับปานกลาง (r = 0.697, 0.693, ตามลำดับ) และสามารถร่วมกันพยากรณ์ประสิทธิผลของหอผู้ป่วยได้ร้อยละ 59.40 (R2 = 0.594)

The purposes of this explanatory research were: (1) to study strategic management of head nurses, trust in nursing organizations, and the effectiveness of patient care units as perceived by staff nurses at community hospitals in Public Health Inspection Region 13 and (2) to explore factors predicting the effectiveness of patient care units as perceived by staff nurse at community hospitals in Public Health Inspection Region 13. The sample of this study included 107 staff nurses who had worked at community hospitals in Public Health Inspection Region 13 for at least one year. They were selected by the stratified random sampling technique. Questionnaires were used as research tools and comprised four sections: (1) personal data, (2) strategic management of head nurses, (3) trust in nursing organizations, and (4) the effectiveness of patient care units. The content validity of questionnaires was verified by five experts. The CVIs of the second to fourth sections were .97,.97, and .91 respectively. The Cronbach’s alpha coefficients of the second to fourth sections were .99, .97, and .98 respectively. Research data were analyzed by descriptive statistics (percentage, mean, and standard deviation) and stepwise multiple regression analysis. The results of this study were as follows. (1) Staff nurses rated strategic management of their head nurses and trust in nursing organizations at the moderate level. They rated the effectiveness of patient care units at the high level. (2) Strategic management and trust in nursing organizations positively significantly correlated to the effectiveness of patient care units at the moderate level (r = 0.697, 0.693, p<0.001) and could predict the effectiveness of patient care units. These predictors accounted for 59.40 % (R2 = 0.594).
