Abstract
การคัดกรองผู้ป่วยเด็กเป็นมาตรฐานการให้บริการพยาบาลผู้ป่วยนอกของโรงพยาบาลรามาธิบดีจักรีนฤบดินทร์ที่เป็นภารกิจที่สำคัญ ที่พยาบาลวิชาชีพผู้ทำหน้าที่คัดกรองอาการของผู้ป่วยเด็กจะต้องคัดกรองอาการได้อย่างถูกต้องเพื่อประเมินภาวะเจ็บป่วยและค้นหากาวะวิกฤติได้ทันท่วงที ก่อนส่งต่อห้องฉุกเฉินได้อย่างรวดเร็ว ทันเวลา จากข้อมูลในการคัดกรองผู้ป่วยเด็ก พบว่า มีความคลาดเคลื่อนในการคัดกรองและขังไม่มีแนวทางปฏิบัติที่ชัดเจน การศึกษาค้นคว้าอิสระนี้ มีวัตถุประสงค์เพื่อจัดทำคู่มือการคัดกรองผู้ป่วยเด็ก สำหรับพยาบาลวิชาชีพที่ปฏิบัติงาน ณ จุดคัดกรองแผนกผู้ป่วยนอก วิธีการจัดทำคู่มือนี้ได้จาก 1) ได้ศึกษาค้นคว้าจากเอกสารและงานวิจัยที่เกี่ยวข้องกับการประเมินและคัดกรองผู้ป่วยเด็ก 2) การศึกษาแนวทางการคัดกรองผู้ป่วยตามระบบ The Emergency Severiy Index version 4 รวมถึงเกณฑ์มาตรฐานสัญญาณชีพของผู้ป่วยเด็กที่ใช้ในโรงพยาบาลรามาธิบดีจักรีนฤบดินทร์ ผลการศึกษาการจัดทำ คู่มือการกัดกรองผู้ป่วยเด็ก สำหรับพยาบาลวิชาชีพที่ปฏิบัติงานบริการผู้ป่วยนอก โรงพยาบาลรามาธิบดีจักรีนฤบดินทร์ ประกอบไปด้วย 5 บท คือ 1. บทนำ 2. หลักการกัดกรองผู้ป่วยเด็ก 3. ขั้นตอนทำคู่มือ 4. เนื้อหาคู่มือคัดกรองผู้ป่วยเด็ก และ 5. ผลการนำคู่มือไปทคลองใช้ จากการนำคู่มือไปทดลองใช้พบว่าพยาบาลวิชาชีพที่จุดคัดกรองมีความรู้ ความมั่นใจ และคัดกรองผู้ป่วยเด็กได้ถูกต้องตามเกณฑ์มากขึ้น ลดความคลาดเคลื่อนในการปฏิบัติงาน และเกิดประโยชน์จากการใช้คู่มือที่ได้ทบทวนแนวทางการปฏิบัติตามนโยบายของโรงพยาบาลในการคัดกรองผู้ป่วย มีมาตรฐานในการปฏิบัติงานด้านการคัดกรองผู้ป่วยนอก
Pediatric nursing screening is an important standard for outpatient nursing services of Ramadhibodi Chakri Naruebodindra Hospital; and registered nurses responsible for such screening or triage have to find out critical or emergency conditions in pediatric patients in order to rapidly transfer them to the emergency room in time. This study aimed to develop a manual on pediatric screening or triage for registered nurses at the hospital’s outpatient department. The manual was prepared based on the reviews of (1) principles and literature related to assessment and triage in pediatric patients, and (2) the Emergency Severity Index (version 4) guidelines and the pediatric vital signs criteria in Ramadhibodi Chakri Naruebodindra Hospital. The newly developed Manual of Pediatric Triage for Outpatient Registered Nurses at Ramadhibodi Chakri Naruebodindra Hospital contains five parts: (1) Introduction, (2) Principles of Pediatric Triage; (3) Steps for Developing the Manual, (4) Content of the manual, and (5) Summary of the manual utilization. The tryout of the manual showed that triage registered nurses had gained knowledge and confidence in pediatric triage practices. So, they could decrease errors in their pediatric screening-triage practices based on the hospital’s policy, guidelines and standards on this matter.
