เปรียบเทียบการเป็นองค์กรแห่งการเรียนรู้ของกลุ่มการพยาบาลตามการรับรู้ของพยาบาลวิชาชีพระหว่างโรงพยาบาลชุมชนเขตตรวจราชการสาธารณสุขที่ 1 ที่ได้รับและยังไม่ได้รับการรับรองคุณภาพโรงพยาบาล

Abstract

การวิจัยเชิงพรรณนาครั้งนี้มีวัตถุประสงค์เพื่อศึกษา (1) ระดับการเป็นองค์กรแห่งการเรียนรู้ของกลุ่มการพยาบาล (2) เปรียบเทียบการเป็นองค์กรแห่งการเรียนรู้ของกลุ่มการพยาบาล (3) ประสบการณ์ในการพัฒนาไปสู่การเป็นองค์กรแห่งการเรียนรู้ของกลุ่มการพยาบาล และ (4) ปัจจัยเอื้อ ปัจจัยที่เป็นปัญหาและอุปสรรคต่อการเป็นองค์กรแห่งการเรียนรู้ของกลุ่มการพยาบาล โรงพยาบาลชุมชน เขตตรวจราชการสาธารณสุขที่ 1 กลุ่มตัวอย่างเป็นพยาบาลวิชาชีพ ที่ปฎิบัติงานในกลุ่มการพยาบาล โรงพยาบาลชุมชน เขตตรวจราชการสาธารณสุขที่ 1 และมีประสบการณ์ในการปฏิบัติงานมากกว่า 1 ปี จำนวน 514 คน ซึ่งได้มาจากการสุ่มแบบทลายขั้นตอน และหัวหน้ากลุ่มการพยาบาล จำนวน 10 คน จากการเลือกแบบเจาะจง เครื่องมือที่ใช้ในการวิจัย มี 2 ชุด ชุดที่ 1 เป็นแบบสอบถามการเป็นองค์กรแห่งการเรียนรู้ ประกอบด้วย 2 ตอน (1) ข้อมูลส่วนบุคคลและ (2) การเป็นองค์กรแห่งการเรียนรู้ และชุดที่ 2 เป็นแบบสัมภาษณ์ประสบการณ์ในการพัฒนากลุ่มการพยาบาลให้เป็นองค์กรแห่งการเรียนรู้ ปัจจัยที่เอื้อและปัจจัยที่เป็นปัญหาและอุปสรรคต่อการพัฒนาสู่การเป็นองค์กรแห่งการเรียนรู้ ซึ่งผ่านการตรวจสอบความตรงเชิงเนื้อหา โดยผู้ทรงคุณวุฒิ จำนวน 4 คน ได้ค่าดัชนีความตรงเชิงเนื้อหาทั้งฉบับ เท่ากับ 0.83 และวิเคราะห์ค่าความเที่ยงของแบบสอบถาม โดยคำนวณค่าสัมประสิทธิ์ครอนบาคแอลฟาเท่ากับ 0.968 สถิติที่ใช้ในการวิเคราะห์ข้อมูลได้แก่ ร้อยละ ค่าเฉลี่ย ส่วนเบี่ยงเบนมาตรฐาน สถิติทดสอบที และการวิเคราะห์เชิงเนื้อหา ผลการศึกษาพบว่า (1) ระดับการเป็นองค์กรแห่งการเรียนรู้ของกลุ่มพยาบาล โรงพยาบาลชุมชน เขตตรวจราชการสาธารณสุขที่ 1 ที่ได้รับการรับรองคุณภาพอยู่ในระดับสูง ส่วนโรงพยาบาลที่ยังไม่ได้รับการรับรองคุณภาพอยู่ในระดับปานกลาง (2) คะแนนเฉลี่ยการเป็นองค์กรแห่งการเรียนรู้ของกลุ่มการพยาบาลโรงพยาบาลชุมชน ที่ได้รับและยังไม่ได้รับการรับรองคุณภาพแตกต่างกันอย่างมีนัยสำคัญทางสถิติที่ระดับ.01 (3) แนวทางการพัฒนากลุ่มการพยาบาลให้เป็นองค์กรแห่งการเรียนรู้ ประกอบด้วย 1) การจัดประชุมวิชาการในการประชุมประจำเดือน 2) การส่งเสริมให้มีการแลกเปลี่ยนความรู้จากการที่พยาบาลไปอบรมภายนอกหน่วยงาน 3) การร่วมกันกำหนดวิสัยทัศน์ ของกลุ่มการพยาบาลและกำหนดเป็นนโยบายที่ชัดเจนของกลุ่มการพยาบาล (4) ปัจจัยที่เอื้อต่อการเป็นองค์กรแห่งการเรียนรู้ประกอบด้วย 1) นโยบายที่ชัดเจน 2) มีกระบวนการพัฒนาและรับรองคุณภาพและ 3) ภาวะผู้นำของผู้นำทางการพยาบาลทุกระดับ ปัญหาและอุปสรรคต่อการเป็นองค์กรแห่งการเรียนรู้ได้แก่ 1) บุคลากรไม่เห็นความสำคัญของการพัฒนา 2) พยาบาลอายุมาก ภาระงานมาก และมีปัญหาสุขภาพ และ 3) การสื่อสารไม่มีประสิทธิภาพ

The purposes of this descriptive research were: (1) to study the level of learning organization in Nursing Departments, (2) to compare learning organization of nursing departments between accredited and non-accredited community hospitals in public health inspection region 1, (3) to explore experiences in developing learning organization of nursing departments, and (4) to investigate successful factors and factors which caused problems and obstacles in building learning organization of nursing departments, public health inspection region 1. The sample comprised two groups. The first included 514 professional nurses who had worked for at least 1 year in community hospitals in public health inspection region 1. They were selected by the multi-stage random sampling technique. The second included 10 head nurses of nursing departments. They were selected by the purposive sampling technique. The research tools used for collecting the data were questionnaires comprising 2 sections: (1) personal data and (2) learning organization. These tools were tested for content validity and reliability. The content validity index (CVI) was 0.83, and the Cronbach alpha coefficient of the second section was 0.968. Research data were analyzed by descriptive statistics (percentage, mean, and standard deviation), independent t-test, and content analysis. The research findings were as follows. (1) Nurses rated learning organization of nursing departments at accredited community hospitals where at the high level; whereas, they rated learning organization of nursing departments at non-accredited community hospitals at the moderate level. (2) There was significantly different of learning organization of the nursing departments between accredited and non-accredited community hospitals in public health inspection region 1 (p < .01). (3) The guidelines for developing learning organization of the nursing departments were as follows:!) arrange monthly academic meetings, 2) promote exchanging knowledge of professional nurses who were trained outside their departments, and 3) share vision of nursing organizations and state explicitly their policy. (4) successful factors of learning organization included the following: 1) policy must be clear, 2) establish a process of development and hospital accreditation, and 3) all nursing administrators need to embrace leadership. Problems and obstacles included the followings: 1) staff did not perceive the importance of learning organization, 2) some staff were approaching retirement age but their workload was heavy and they also had their own health problems, and 3) communication is not efficient.
