การพัฒนารูปแบบการบันทึกทางการพยาบาลที่เน้นกระบวนการพยาบาลงานผู้ป่วยในโรงพยาบาลขุนตาล จังหวัดเชียงราย

Abstract

การวิจัยครั้งนี้เป็นการวิจัยและพัฒนา มีวัตถุประสงค์เพื่อ (1) พัฒนารูปแบบการบันทึกทางการพยาบาลที่เน้นกระบวนการพยาบาล งานผู้ป่วยในโรงพยาบาลขุนตาล จังหวัดเชียงราย และ (2) ศึกษาประสิทธิภาพของรูปแบบการบันทึกทางการพยาบาลที่เน้นกระบวนการพยาบาล กลุ่มตัวอย่าง คือ พยาบาลวิชาชีพงานผู้ป่วยใน จำนวน 9 ราย และ แบบบันทึกทางการพยาบาลของผู้ป่วยใน จำนวน 60 แฟ้ม เป็นบันทึกก่อนทดลอง จำนวน 30 แฟ้ม และแบบบันทึกหลังทดลอง จำนวน 30 แฟ้ม สุ่มตัวอย่างแบบเจาะจง เครื่องมือที่ใชัในการทดลอง ได้แก่ (1) รูปแบบการบันทึกทางการพยาบาลที่เน้นกระบวนการพยาบาล และ (2) แบบประเมินความรู้เกี่ยวกับการบันทึกทางการพยาบาลที่เน้นกระบวนการพยาบาล เครื่องมือที่ใช้ในการรวบรวมข้อมูลได้แก่ (1) แบบสำรวจปัญหาและอุปสรรคของการบันทึกทางการพยาบาล (2) แบบสอบถามความคิดเห็นของพยาบาลเกี่ยวกับความเหมาะสมของรูปแบบการบันทึกทางการพยาบาล และหาความเชื่อมั่นโดยใช้คำสัมประสิทธิ์ครอนบาคอัลฟาได้เท่ากับ 0.93 (3) แบบตรวจสอบคุณภาพการบันทึกทางการพยาบาล และหาความเชื่อมั่นโดยคำนวณหาดัชนีความสอดคล้อง ได้ค่าความเชื่อมั่นด้านความครอบคลุมตามกระบวนการพยาบาลเท่ากับ 0.84 ด้านความถูกต้องตามกฎหมายเท่ากับ 0.90 และด้านความต่อเนื่องของการบันทึกเท่ากับ 0.95 วิเคราะห์ข้อมูลคุณภาพการบันทึกทางการพยาบาลโดยใช้สถิติทดสอบ ทีอิสระ และวิเคราะห์ข้อมูลความคิดเห็นของพยาบาลเกี่ยวกับความเหมาะสมของรูปแบบการบันทึกทางการพยาบาลโดยหาค่าเฉลี่ยอันดับที่ค่ามัธยฐาน ทดสอบความแตกต่างด้วยสถิติ Wilcoxon matched-pair signed ranks test ผลการวิจัยสรุปได้ด้งนี้ (1) ได้รูปแบบการบันทึกทางการพยาบาลที่เน้นกระบวนการพยาบาลงานผู้ป่วยใน โรงพยาบาลขุนตาลที่มีประสิทธิภาพ ประกอบด้วย (ก) หลักการบันทึกทางการพยาบาลที่เน้นกระบวนการพยาบาล (ข) วัตถุประสงค์ของรูปแบบการบันทึก (ค) นโยบายและการควบคุมการบันทึกทางการพยาบาล (ง) การประเมินสมรรถนะการบันทึกทางการพยาบาลและการฝึกอบรม (จ) แนวทางการบันทึกและแบบบันทึกทางการพยาบาล (2) ค่าเฉลี่ยอันดับที่ค่ามัธยฐาน ของคะแนนความคิดเห็นของพยาบาลเกี่ยวกับความเหมาะสมของรูปแบบการบันทึกทางการพยาบาลหลังทดลองสูงกว่าก่อนทดลอง อย่างมีนัยสำคัญทางสถิติที่ระดับ .01 และ (3) ค่าเฉลี่ยคะแนนคุณภาพการบันทึกทางการพยาบาลโดยรวมหลังทดลองสูงกว่าก่อนทดลอง อย่างมีนัยสำคัญทางสถิติที่ระดับ .01

The purposes of this research and development were: (a) to develop the nursing documentation model focused on the nursing process of the inpatient unit at Khuntan Hospital in Chiang Rai Province; and (b) to evaluate the efficiency of the nursing documentation model. The sample consisted of nine nurses of the inpatient unit and sixty patients’ nursing documentation of which the first thirty were used before and the other thirty were used after the application of the nursing documentation model .These documents were chosen by purposive sampling. The experiment tools were (I) the nursing documentation model which was focused on the nursing process and (2) the test which measured the knowledge of nurses on nursing documentation. The data collection tools were (1) the problem of using nursing documentation form, (2) the opinions of nurses on the appropriateness of the model, and (3) the check list of the quality of nursing documentation. The Cronbach alpha coefficient of the second tool was 0.93. The reliability of the third tool was checked by the Item Objective Congruence (IOC). It was found that the structure reliability according to the nursing process was 0.84, the legal reliability was 0.90, and the continuity reliability was 0.95. Statistical devices used for data analysis were independent t-test and Wilcoxon matched- pair signed ranks test. The former was employed to explore the quality of nursing documentation, while the latter was done to investigate the opinions of nurses on the appropriateness of using the new model. The results of this research showed that the new model was efficient. Moreover, (I) the new model consisted of five components: (a) the principle of nursing documentation focused on the nursing process, (b) the objectives of model, (c) the policy and quality control of nursing documentation, (d)the assessment of staff competency and training of nursing documentation, and (e) the nursing documentation guideline and forms. (2) Nurses expressed the opinion that the new model was more significantly suitable than before (p<.01). Finally, (3) the mean scores of the quality of nursing documentation after the experiment were significantly higher than before conducting the experiment (pc.OI).
