การพัฒนารูปแบบการนิเทศทางการพยาบาลโรงพยาบาลเชียงคำ จังหวัดพะเยา

Abstract

การวิจัยครั้งนี้เป็นการวิจัยและพัฒนา มีวัตถุประสงค์เพื่อ (1) พัฒนารูปแบบการนิเทศ ทางการพยาบาล (2) ศึกษาประสิทธิภาพของรูปแบบการนิเทศทางการพยาบาล โรงพยาบาลเชียงคำ จังหวัดพะเยา กลุ่มตัวอย่างคือ หัวหน้าหอผู้ป่วยและรองหัวหน้าหอผู้ป่วยที่ปฏิบัติงานอยู่ในหอผู้ป่วยสูติ-นรีเวชกรรม ศัลยกรรม อายุรกรรม และกุมารเวชกรรม จำนวน 8 คน พยาบาลวิชาชีพประจำการที่ปฏิบัติงานในหอผู้ป่วยทั้ง 4 หอ จำนวน 34 คน เครื่องมือที่ใช้ในการวิจัยมี 4 ชุด คือ (1) แบบสำรวจสภาพการณ์การนิเทศทางการพยาบาล (2) แบบทดสอบความรู้การนิเทศทางการพยาบาล (3) แบบสอบถามความคิดเห็นของผู้รับการนิเทศเกี่ยวกับประสิทธิภาพของรูปแบบ การนิเทศทางการพยาบาล และ (4) แบบสอบถามความคิดเห็นของผู้นิเทศทางการพยาบาลเกี่ยวกับประสิทธิภาพของรูปแบบการนิเทศทางการพยาบาล เครื่องมือผ่านการตรวจสอบความตรงตามเนี้อหาจากผู้ทรงคุณวุฒิ จำนวน 5 คน และเครื่องมือชุดที่ 2 ,3 และ 4 มีค่าความเที่ยงเท่ากับ 0.75 0.98 และ 0.97 ตามลำดับ วิเคราะห์ข้อมูลโดยการวิเคราะห์เนื้อหา การแจกแจงความถี่ ร้อยละ ทดสอบความต่างด้วยสถิติ t (paired I test) และ Wilcoxon matched - pair signed ranks test ผลการวิจัยพบว่า (1) รูปแบบการนิเทศทางการพยาบาลประกอบด้วย 6 องค์ประกอบ คือ 1) การเตรียมผู้นิเทศ 2) การกำหนดข้อตกลง 3) การรับฟังปัญหา 4) การตรวจสอบความเข้าใจและวางแผน 5) การนำไปปฏิบัติ 6) การทบทวนประเมินผลและสะท้อนกลับข้อมูล (2) ความรู้เรื่องการนิเทศทางการพยาบาลของผู้นิเทศหลังการเสริมความรู้สูงกว่าก่อนเสริมความรู้ (3) ความคิดเห็นของผู้รับการนิเทศเกี่ยวกับประสิทธิภาพของรูปแบบการนิเทศทางการพยาบาลก่อน และหลังการใช้รูปแบบไม่แตกต่างกัน และ (4) ความคิดเห็นของผู้นิเทศเกี่ยวกับประสิทธิภาพของรูปแบบการนิเทศทางการพยาบาลหลังใช้รูปแบบมี ค่าเฉลี่ยอันดับที่มาตรฐานสูงกว่าก่อนใช้รูปแบบอย่างมีนัยสำคัญทางสถิติที่ระดับ .05

The purposes of this research and development were : (1) to develop a nursing supervision model, and (2) to study the efficiency of the nursing supervision model at Chiangkham Hospital, Phayao Province. The sample group included eight head nurses and subhead nurses and 34 professional nurses who worked at four in-patient departments: obstetric, surgery, medicine, and pediatric wards. Four research tools were used: a survey of the nursing supervision situational, : test of nursing supervision knowledge, and two questionnaires of the efficiency of the nursing supervision model. One is for supervisors and the other is for nurses. The content validity of these tools was verified by 5 experts. The Cronbach alpha reliability coefficients of the second, the third, and the fourth sets were 0.75,0.98, and 0.97 respectively. Data were analyzed by content analysis. Statistics used for data analysis were frequency, percentage, paired I test and Wilcoxon matched - pair signed ranks test. The research findings were as follows (1) The nursing supervision model consisted of 6 components 1) prepare supervisors, 2) contract, 3) listen, 4) explore, 5) action, and 6) review after action, evaluate, and reflect. (2) Post-test scores of nursing supervision knowledge were higher than pre-test scores. (3) There was no difference in the efficiency of implementing the supervision model by nurses before and after the experiment. Finally, (4) Supervisors rated the efficiency of the supervision model after implementation significantly higher than before, (p< .05).
