Abstract
การวิจัยและพัฒนาครั้งนี้มีวัตถุประสงค์เพื่อ 1) ศึกษาสภาพการณ์การดูแลผู้ป่วยที่มีภาวะติดเชื้อในกระแสเลือดหอผู้ป่วยใน โรงพยาบาลปทุมราชวงศา 2) พัฒนารูปแบบการดูแลผู้ป่วยที่มีภาวะติดเชื้อในกระแสเลือดหอผู้ป่วยใน และ 3) ศึกษาผลของรูปแบบการดูแลผู้ป่วยที่มีภาวะติดเชื้อในกระแสเลือดหอผู้ป่วยใน กลุ่มตัวอย่างแบ่งตามระยะการพัฒนารูปแบบ ได้แก่ ระยะที่ 1 การศึกษาสภาพการณ์การดูแลผู้ป่วยที่มีภาวะติดเชื้อในกระแสเลือดหอผู้ป่วยใน และระยะที่ 2 การพัฒนารูปแบบการดูแลผู้ป่วยที่มีภาวะติดเชื้อในกระแสเลือดหอผู้ป่วยใน กลุ่มตัวอย่างเป็นทีมสหสาขาวิชาชีพจำนวน 11 คน ประกอบด้วย แพทย์ เภสัชกร นักเทคนิคการแพทย์ นักโภชนาการ พยาบาลป้องกันและควบคุมการติดเชื้อในโรงพยาบาล พยาบาลวิชาชีพที่แผนกผู้ป่วยนอก พยาบาลวิชาชีพที่แผนกอุบัติเหตุ-ฉุกเฉิน และพยาบาลวิชาชีพที่หอผู้ป่วยใน และระยะที่ 3 การศึกษาผลของรูปแบบการดูแลผู้ป่วยที่มีภาวะติดเชื้อในกระแสเลือดหอผู้ป่วยใน กลุ่มตัวอย่างประกอบด้วย พยาบาลวิชาชีพที่หอผู้ป่วยใน จำนวน 11 คน กลุ่มตัวอย่างถูกเลือกแบบเจาะจง เครื่องมือการวิจัยมี 4 ส่วน ได้แก่ 1) แนวคำถามเชิงกึ่งโครงสร้างที่ใช้ในการสนทนากลุ่ม 2) รูปแบบการดูแลผู้ป่วยที่มีภาวะติดเชื้อในกระแสเลือด หอผู้ป่วยใน โรงพยาบาลปทุมราชวงศา 3) แบบประเมินการปฏิบัติการพยาบาลตามรูปแบบ และ 4) แบบประเมินความพึงพอใจของพยาบาลต่อการใช้รูปแบบ มีค่าดัชนีความตรงเชิงเนื้อหาเท่ากับ 0.82, 0.86, 0.87, และ 0.89 ตามลำดับ และค่าสัมประสิทธิ์อัลฟาของครอนบาคของส่วนที่ 3 และ 4 เท่ากับ 0.88 และ 0.91 ตามลำดับ วิเคราะห์ข้อมูลเชิงคุณภาพด้วยการวิเคราะห์เชิงเนื้อหา วิเคราะห์ข้อมูลเชิงปริมาณโดยใช้ค่าเฉลี่ย ร้อยละ ส่วนเบี่ยงเบนมาตรฐาน และสถิติทดสอบที ผลการวิจัยมีดังนี้ 1) สภาพการณ์การดูแลผู้ป่วยที่มีภาวะติดเชื้อในกระแสเลือดหอผู้ป่วยใน รพ. ปทุมราชวงศา มีแนวปฏิบัติการพยาบาลไม่ชัดเจน การสื่อสารและสร้างความเข้าใจแนวปฏิบัติการพยาบาลไม่ทั่วถึง การกำกับติดตามประเมินอาการผู้ป่วยขาดความต่อเนื่อง 2) รูปแบบการดูแลผู้ป่วยที่มีภาวะติดเชื้อในกระแสเลือดมี 3 องค์ประกอบ ได้แก่ (1) การพัฒนาแนวปฏิบัติการพยาบาลผู้ป่วยที่มีภาวะติดเชื้อในกระแสเลือดหอผู้ป่วยใน (2) การสื่อสารและสร้างความเข้าใจแนวปฏิบัติการพยาบาลผู้ป่วยที่มีภาวะติดเชื้อในกระแสเลือด และ (3) การกำกับติดตามประเมินอาการผู้ป่วยที่มีภาวะติดเชื้อในกระแสเลือด และ 3) ผลของรูปแบบการดูแลผู้ป่วยที่มีภาวะติดเชื้อในกระแสเลือดหอผู้ป่วยใน พบว่าพยาบาลมีการปฏิบัติการพยาบาลตามรูปแบบในระดับมากที่สุด และพยาบาลมีความพึงพอใจต่อการใช้รูปแบบในระดับมากที่สุด
This research and development study aimed 1) to explore the situation of sepsis patients caring at inpatient unit, Pathumratchawongsa Hospital, 2) to develop Sepsis Patients Caring Model inpatient unit, and 3) to study the effects of Sepsis Patients Caring Model at inpatient unit. The sample was divided according to the development phase, which are Phase 1 explore the situation of sepsis patients caring at inpatient unit and Phase 2 develop Sepsis Patients Caring Model at inpatient unit using the multidisciplinary team 11 people comprised of doctor, pharmacist, medical technicians, nutritionist, infection control nurses, registered nurses at the outpatient unit, registered nurses at the emergency room and registered nurses at the inpatient unit. Phase 3 study the effects of Sepsis Patients Caring Model at inpatient unit using the registered nurses who working at the inpatient unit 11 people. The samples were purposive sampling. The research tools were 4 parts 1) The semi structure interview for group discussion 2) The Sepsis Patients Caring Model at Inpatient Unit, Pathumratchawongsa Hospital. 3) The evaluation form for nursing practice guidelines and 4) The evaluation form of registered nurses’s satisfaction on using the model. Content validity index values 0.82, 0.86, 0.87, and 0.89 respectively and the Cronbach’s alpha coefficient of 3 and 4 part were 0.88 and 0.91 respectively. The qualitative data was analyzed by content analysis and the quantitative data analyzed by average, percentage, standard deviation and t-test The results of this study were as follows. 1) The problems and obstacles of former sepsis patients caring at inpatient unit, Pathumratchawongsa Hospital were nursing practice guidelines were not clear, inadequate communication and understand of nursing practices guidelines, lack of continuity on monitor and evaluation of Sepsis patients 2) The Sepsis Patients Caring Model at Inpatient Unit consists of 3 components (1) Nursing Practice Guidelines for Sepsis Patients Caring (2)Communication and understanding of nursing practice guideline for Sepsis Patients Caring and (3) Monitoring and evaluation of sepsis Patients and 3) The registered nurses rated their nursing practice guidelines at the highest level. They rated their satisfaction on using this model at the highest level also.
