การจัดอัตรากำลังพยาบาลในกลุ่มมารดาหลังคลอดและทารกแรกเกิดโรงพยาบาลท่าม่วง จังหวัดกาญจนบุรี

Abstract

การวิจัยครั้งนี้เป็นการวิจัยเชิงพรรณนา (Descriptive research) มีวัตถุประสงค์เพื่อศึกษา (1) ปริมาณเวลาการพยาบาล ตามมาตรฐานที่พยาบาลใช้ไนการปฏิบัติกิจกรรมการพยาบาลในการดูแลมารดาหลังคลอดและทารกแรกเกิด จำแนกประเภทตามเกณฑ์การจำแนกผู้ป่วย 10 ประเภท (2) วิเคราะห์อัตรากำลังพยาบาลที่เหมาะสมในการดูแลมารดาหลังคลอดและทารกแรกเกิดของ โรงพยาบาลท่าม่วง จังหวัดกาญจนบุรี กลุ่มตัวอย่าง ได้แก่ กิจกรรมการพยาบาลที่ปฏิบัติต่อมารดาหลังคลอดและทารกแรกเกิดทั้งหมด 4,180 กิจกรรม และมารดาหลังคลอด ทารกแรกเกิด จำแนกตามประเภท จำนวน 169 ราย โดยผู้วิจัยและผู้ช่วยวิจัย สังเกตการปฏิบัติกิจกรรมการพยาบาลของพยาบาลที่เป็นแบบในการจับเวลา เครื่องมีอประกอบด้วย 2 ส่วน คือ (1) เครื่องมือบันทึกข้อมูล ได้แก่แบบบันทึกข้อมูลการบริการของหอผู้ป่วย สูตินรีเวชกรรม แบบบันทึกเวลาและความถี่ ในการปฏิบัติกิจกรรมการพยาบาล (2) คู่มือคำอธิบายกิจกรรมการพยาบาล ซึ่งผ่านการตรวจสอบความตรงตามเนี้อหา โดยผู้เชี่ยวชาญ 6 ท่าน และมีค่าดัชนีความสอดคล้องของการสังเกต เท่ากับ 0.93 และ 0.92 ตามลำดับ คำนวณอัตรากำลังพยาบาลที่ต้องการโดยใช้สูตรการคำนวณของ Brown พร้อมทั้งวิเคราะห์อัตรากำลังพยาบาลที่ต้องการในแต่ละวันและการกระจายพยาบาลในแต่ละเวร ผลการวิจัย พบว่า (1) มารดาหลังคลอดประเภทอาการหนักปานกลางและต้องการการดูแลปานกลาง (2c) ประเภทอาการหนักปานกลางและต้องการการดูแลมาก (2b) และอาการหนักและต้องการการดูแลมาก (3b) ใช้เวลาการพยาบาลตามมาตรฐาน คือ 3.82, 4.63 และ 5.35 ชั่วโมง/คน/วัน ส่วนทารกแรกเกิดประเภทอาการหนักปานกลางและต้องการการดูแลมาก (2b) ประเภทอาการหนักปานกลางและต้องการการดูแลมากตลอดเวลา (2a) และประเภทอาการหนักและต้องการการดูแลมากตลอดเวลา (3a) ใช้เวลาการพยาบาลตามมาตรฐานเท่ากับ 4.18, 6.32, 8.25 ชั่วโมง/คน/วัน ตามลำดับ (2) อัตรากำลังพยาบาลที่ต้องการในการดูแลมารดาหลังคลอดและทารกแรกเกิด เท่ากับ 15 คน/วัน และบุคลากรพยาบาลที่ขึ้นปฏิบัติงานในเวร เช่า : บ่าย : ดึก เท่ากับ 6 : 5 : 4 คน

The purposes of this descriptive research were: (1) to study standard times of nursing activities in postpartum care for the mother and newborn group according to 10 categories of patient classification, and (2) to analyze nurse staffing in postpartum care for the mother and newborn group at Thamuang hospital in Kanchanaburi province. The items included 4,180 activities of nursing care provided for 169 postpartum mothers and newboms. The researcher and the research assistant observed nurses who provided nursing care and counted times of each activity. Two research instruments were used. (l)Two record forms included nursing services of postpartum and newborn unit, and the time and frequency of nursing practice. (2) The nursing manual described nursing activities which provided for postpartum and newborn. The content validity of these tools were verified by six experts and their inter-rater reliabilities of observation score were 0.93 and 0.92 respectively. Nurse staffing were assessed by Brown’s formula, and nurse staffing of daily and shift was analyzed. The results showed as follows. (1) Time spent on nursing care according to standard of postpartum patients who were in a moderate severe condition and needed moderate care (2c), postpartum patients who were in a moderate severe condition needed more care (2b), and postpartum patients who were in a severe condition and needed more care (3b) was 3.82, 4.63, and 5.35 hours/ person/ day respectively. Time spent on nursing care according to standard of newborn babies who were in a moderate severe condition needed moderate care (2b), newborn babies who were in a moderate severe condition and needed more care at all times (2a), newborn babies who were in a severe condition and needed more care at all times (3a) was 4.18, 6.32, and 8.25 hour / person / day, respectively. (2) Nursing staffing caring for mothers and newboms was 15 persons/ day. The ratio of nursing personnel who worked in the morning shift: the evening shift: and the night shift was 6: 5: 4.
