Abstract
การวิจัยเชิงพรรณนานี้ มีวัตถุประสงค์เพื่อ วิเคราะห์ค่าเฉลี่ยต้นทุนกิจกรรมบริการของงานผู้ป่วยนอกจักษุโรงพยาบาลเมตตาประชารักษ์ (วัดโร่ขิง) โดยใช้ระบบการคิดต้นทุนกิจกรรม โดยจำแนกเป็นกิจกรรมบริการทางตรงและกิจกรรมบริการทางล้อม เก็บข้อมูลในระหว่างเดือนกรกฎาคม - กันยายน 2549 ประชากร คือ บุคลากรที่ปฏิบัติงานและผู้รับบริการในหน่วยงานผู้ป่วยนอกจักษุ เครื่องมือที่ใช้ในการวิจัยแบ่งเป็น 2 ชุด ชุดที่ 1 เป็นพจนานุกรมกิจกรรมบริการของงานผู้ป่วยนอกจักษุ ชุดที่ 2 เป็นแบบบันทึกข้อมูลจำนวน 6 แบบฟอร์ม ได้แก่ 1) แบบบันทึกปริมาณกิจกรรม 2) แบบบันทึกเวลาที่ใช้ในการปฏิบัติกิจกรรม 3) แบบบันทึกต้นทุนค่าวัสดุ 4) แบบบันทึกต้นทุนค่าเสื่อมราคาครุภัณฑ์ 5) แบบบันทึกต้นทุนค่าสาธารณูปโภค 6) แบบบันทึกต้นทุนค่าแรง วิเคราะห์ข้อมูลโดยหาค่าเฉลี่ยและร้อยละ ผลการศึกษาพบว่างานผู้ป่วยนอกจักษุมีกิจกรรมบริการทั้งหมด 68 กิจกรรม แบ่งเป็นกิจกรรมบริการทางตรง 52 กิจกรรม และกิจกรรมบริการทางอ้อม 16 กิจกรรม ต้นทุนรวมของกิจกรรมบริการ งานผู้ป่วยนอกจักษุ โรงพยาบาลเมตตาประชารักษ์เท่ากับ 4,141,102.71 บาท โดยที่กิจกรรม
ทางตรงเกี่ยวกับการฉีดสีถ่ายภาพจอประสาทตามีต้นทุนต่อหน่วยกิจกรรมมากที่สุด เท่ากับ 898.47 บาทต่อครั้ง และกิจกรรมการระบุตัวผู้ป่วยเก่ามีค่าเฉลี่ยต้นทุนต่อหน่วยกิจกรรมน้อยที่สุด เท่ากับ 4.20 บาทต่อครั้ง ส่วนกิจกรรมบริการทางล้อมเกี่ยวกับการจัดตารางเวรมีค่าเฉลี่ยต้นทุนต่อหน่วยกิจกรรมมากที่สุด คือ เท่ากับ 362.81 บาทต่อครั้ง กิจกรรมการประสานก่อนส่งผู้ป่วยไปตึกผู้ป่วยใน (admit) มีค่าเฉลี่ยต้นทุนต่อหน่วยกิจกรรมน้อยทึ่สุดเท่ากับ 12.09 บาทต่อครั้ง ข้อเสนอแนะจากการวิจัยนี้คือการจัดบริการผู้ป่วย ทุกประเภทจะต้องให้บริการทั้งกิจกรรมทางตรงและกิจกรรมทางอ้อมควบคู่กันไป ในขณะที่อัตราการเรียกเก็บค่าบริการตามที่รัฐกำหนดให้เรียก
เก็บได้เฉพาะกิจกรรมทางตรงเพียงบางรายการเท่านี้น ซึ่งเงินที่ได้รับไม่เพียงพอกับค่าใช้จ่ายที่เกิดขึ้นตามความเป็นจริง จึงส่งผลให้งบประมาณที่ได้รับไม่เพียงพอต่อการจัดบริการที่เกิดขึ้นจริงในผู้ป่วยแต่ละราย
This present study was a descriptive research to analyze the average cost of service activities in Ophthalmology Out-patient Unit, Mettapracharuk Hospital using the Activities-Based Costing (ABC) which was classified into direct and indirect services. Data was collected during July and September 2006. The samples were the staff and patients in Ophthalmology Out-patient Unit. Two sets of research instruments were used; the first one was an activity service dictionary of Ophthalmology Out-patient Unit, while the second was 6 data forms including 1) activity amount form, 2) time table of activity form, 3) material cost form, 4) depreciation cost form, 5) public utilities cost form, and 6) labor cost form. Mean and percentage were used to analyze the data. The study results revealed total 68 activities among which 52 activities were direct and 16 were indirect. The result showed that the total of the cost of service activities in Ophthalmology Out-patient Unit was 4,141,102.71 baht. The highest cost was the Findus Fluoresce in Angiography (FFA), 898.47 baht per time. The lowest cost was the identification of the old patients, 4.20 baht per time. The highest cost of the indirect activities, 362.81 baht per time was the staffs time table scheduling. The average of lowest cost, 12.09 baht per time was the contact activities between out- patient unit and inner-patient unit before admitted. It is recommend that the patient services are to include both direct and indirect activities but the service charges according to the government fees schedules can only cover certain direct activities of the services. The fees are not sufficient to cover true costs. This has caused the acquired budget insufficient for each actual patient care
