Abstract
การวิจัยประเภทสำรวจเชิงพรรณนาครั้งนื้มีวัตถุประสงค์เพื่อศึกษา (1) ปัจจัยด้านสังคมประชากรการใช้บริการสุขภาพ และการสนับสนุนผู้ป่วยนอกที่มารับบริการตามนโยบายหลักประกันสุขภาพถ้วนหน้า (2) ระดับความพึงพอใจของผู้ป่วย (3) ความสัมพันธ์ระหว่างปัจจัยด้านสังคมประชากร การใช้บริการสุขภาพ และการสนับสนุน กับความพึงพอใจของผู้ป่วย (4) ปัญหา อุปสรรค และข้อเสนอแนะ ของผู้ป่วยนอกที่โรงพยาบาลศรีสะเกษ การเก็บรวบรวมข้อมูลโดยวิธีสัมภาษณ์กลุ่มตัวอย่าง คือ ผู้ป่วยนอกที่มีสิทธิตามนโยบายหลักประกันสุขภาพถ้วนหน้าที่มารับบริการในโรงพยาบาลศรีสะเกษจำนวน 420 คน ที่ได้มาจากการสุ่มอย่างง่าย และเก็บข้อมูลเชิงคุณภาพจากการสัมภาษณ์เจาะลึกจำนวน 30 คน ในระหว่างวันที่ 25 กุมภาพันธ์ - 30 เมษายน 2545 เครื่องมือที่ใช้คือ แบบสอบถามที่ผู้วิจัยสร้างขึ้นเองซึ่งมีคำความเชื่อมั่นเท่ากับ 0.9540 และแบบสัมภาษณ์เจาะ
ลึก การวิเคราะห์ข้อมูลใช้ร้อยละ ค่าเฉลี่ย ส่วนเบี่ยงเบนมาตรฐาน พิสัย การวิเคราะห์เนื้อหา การทดสอบค่าที การวิเคราะห์ความแปรปรวนทางเดียว สัมประสิทธิ์สหสัมพันธ์แบบเพียร์สัน สัมประสิทธิ์สหสัมพันธ์แบบสเปียร์แมนแรงค์ และวิเคราะห์การถดถอยพหุคูณ ผลการวิจัยพบว่า (1) ผู้ตอบแบบสอบถามส่วนใหญ่เป็นเพศหญิงอายุระหว่าง 25 - 44 ปี จบการศึกษาระดับประถมศึกษา อาชีพเกษตรกร รายได้เฉลี่ยของครอบครัวต่อเดือนตํ่ากว่า 3,000 บาท ระยะเวลาเดินทางจากบ้านถึงโรงพยาบาลประมาณครึ่งชั่วโมง มารับบริการส่วนใหญ่เป็นช่วงเช้าระยะเวลาโดยรวมตั้งแต่ผู้ป่วยมาถึงโรงพยาบาลจนได้รับบริการแล้วเสร็จประมาณ 2 ชั่วโมง แผนกที่มารับบริการส่วนใหญ่คือแผนกอายุรกรรม มารับบริการครั้งนี้เป็นครั้งแรกภายหลังเริ่มนโยบาย มีระดับความรู้ปานกลางเกี่ยวกับนโยบายหลักประกันสุขภาพถ้วนหน้า (2) ระดับความพึงพอใจโดยรวมทุกด้านอยู่ในระดับดี (3) ปัจจัยที่มีความสัมพันธ์กับความพึงพอใจของผู้ป่วย ได้แก่ ระดับการศึกษาช่วงเวลาในการรับบริการ และระดับความรู้เกี่ยวกับนโยบายหลักประกันสุขภาพถ้วนหน้า มีความสัมพันธ์อย่างมีนัยสำคัญทางสถิติที่ระดับ .001,-05 และ .001 ตามลำดับ (4) ปัญหาอุปสรรคและข้อเสนอแนะของผู้ป่วยนอกที่มารับบริการส่วนใหญ่เป็นด้านความสะดวกในการรับบริการ
The objectives of this descriptive survey research were to study (1) the sociocultural factors, the health service utilization factors and the other supporting factors of out-patients under the Universal Coverage Policy; (2) the out-patients’ satisfaction; (3) the relationship between those 3 group factors and the patients’ satisfaction; and (4) the problems, obstacles and suggestions of the out-patients at Sisaket Hospital Data were collected by interviewing the 420 samples randomly selected from the out-patients who have the rights under the Universal Coverage Policy in Sisaket Hospital Qualitative data were collected by in-depth interview of the other 30 out-patients samples during February 25,2002 and April 30,2002. The research instruments used were interviewing questionnaire with reliability level at 0.9540 and in-depth interviewing questionnaire. The percentage, mean, standard deviation, range, content analysis, t-test, One Way - ANOVA, Pearson Correlation Coefficient, Spearman Rank Correlation Coefficient and Multiple Regression were used in data analysis. The findings were as follows; (1) most of samples were female aged between 25-44 years, had education at prathomsuksa level, were farmers, had average household incomes below 3,000 Baht per month , took half an hour to the Hospital, mostly attended in the morning, spent 2 hours after they arrived the Hospital until they had already received all services they need, mostly attended in the Medicine Department, firstly came to the Hospital after the Universal Coverage Policy implementation and had knowledge about the Policy m middle level; (2) the overall patients’ satisfaction were in good level; (3) the factors related to the patients’ satisfaction were education level, attended time and their knowledge about the Policy with statistical significant at the .001, .05 and .001, respectively; and (4) most of the problems, obstacles and suggestions of the out-patients were the convenience in receiving health services
