ความคิดเห็นของเจ้าหน้าที่สาธารณสุขเกี่ยวกับการพัฒนาบุคลากรผู้ให้บริการในศูนย์สุขภาพชุมชนของจังหวัดนครนายก

Abstract

การวิจัยแบบสำรวจเชิงพรรณนานี้ มุ่งหวังเพื่อหาแนวทางในการพัฒนาบุคลากรผู้ให้บริการในศูนย์สุขภาพชุมชน โดยดำเนินการกับเจ้าหน้าที่สาธารณสุขทุกคนจํานวน 210 คน ในศูนย์สุขภาพชุมชน เครื่องมือที่ใช้เป็นแบบสอบถามแบบอัตราส่วนประมาณค่าที่มีค่าสัมประสิทธิ์อัลฟ่าประมาณ 30 ขึ้นไปในทุกรายการ ซึ่งถูกส่งตามสายงานไปยังเจ้าหน้าที่สาธารณสุข ได้รับ แบบสอบถามคืน จํานวน 194 ชุด คิดเป็นอัตราการตอบกลับร้อยละ 93.4 สัดส่วนที่ไม่ตอบเป็นเพราะอยู่ในระหว่างการอบรมและย้ายไปทํางานที่อื่น ทำการวิเคราะห์ข้อมูลด้วยสถิติเชิงพรรณนาเป็นค่าร้อยละ และค่าเฉลี่ย ผลการวิจัยพบว่า 1) เจ้าหน้าที่สาธารณสุขทุกตำแหน่งเคยได้รับการอบรมเพิ่มพูนความรู้และทักษะที่จําเป็นสําหรับการปฏิบัติงานในศูนย์สุขภาพชุมชน ยกเว้นทันตาภิบาล ไม่เคยได้รับการอบรมเรื่องใดเลย 2 เจ้าหน้าที่ส่วนใหญ่มีความคิดเห็นคล้ายคลึงกันในระดับปานกลาง เกี่ยวกับความจําเป็นต้องจัดให้มีแพทย์ บริการตรวจรักษาโรคในศูนย์สุขภาพชุมชน และในระดับมากเกี่ยวกับการพัฒนาเจ้าหน้าที่สถานีอนามัยให้เป็นผู้ตรวจรักษาโรคที่พบบ่อยในชุมชนได้ 3) เจ้าหน้าที่ส่วนใหญ่มีความคิดเห็นในระดับปานกลางเกี่ยวกับความสามารถของคนในการดำเนินงานด้านบริการ บริหารจัดการและวิชาการ 4) เจ้าหน้าที่ส่วนใหญ่มีความคิดเห็นเกี่ยวกับ ความต้องการฝึกอบรม/เรียงตามลำดับจากระดับมากไปหาน้อยในเรื่อง การวางแผนแบบมุ่งผลสัมฤทธิ์ การดำเนินงานวิจัยสาธารณสุข การคิดต้นทุนต่อหน่วยบริการและการติดตามประเมินแผนงาน/โครงการ ข้อสรุปที่ได้จากการวิจัย คือ ความจำเป็นในการเพิ่มพูนความรู้และทักษะด้านการวางแผนและการบริหารแบบมุ่งเน้นผลสัมฤทธิ์ การวิเคราะห์ทางเศรษฐศาสตร์ ควบคู่ไปกับการกําหนดนโยบายเพื่อสนับสนุนการพัฒนามาตรฐานด้านบริการ บริหารจัดการ และวิชาการ ของศูนย์สุขภาพชุมชน

This descriptive survey design was aimed to determining a guideline to develop those who provide service at primary care unit (PCU) and conducted with a totality of 210 public health staffs (PHSs) of all PCUs. The instrument was a rating scale questionnaire with the Cronbach's alpha coefficient of 80 for all categories, which was sent to the subject along the command line. The return rate was 92.4%. Some of the lost out proportions were attending certain training courses and some transferred to other workplaces. Descriptive statistics was used to analyze data into percentage and average. Major findings were 1) the PHSs in all current positions received many training courses relating to primary care performance, expect for the dental health personnel; 2) most of the PHSS held similar opinions al moderate level about the need for assigning a doctor to provide medical care at PCU, and a high level about the development of health center staff to be a therapist of common diseases in community; 3) most of the PHSs held opinions at moderate level about their capabilities of primary care provision in three dimensions: service, management, and academic; 4) most of the PHSs held opinions the need for certain training ranking from high to low, namely, result-based planning, conducting public health research, estimating unit cost and monitoring and evaluating performance/project. In conclusion, there is a need for enhancing knowledge and skills in result-based planning and management, economic analysis, together with policy formulation in order to support the standard development of the three f ions of PCUs
