Abstract
การวิจัยนี้มีวัตถุประสงค์เพื่อ 1) หาต้นทุนต่อหน่วยบริการของศูนย์สุขภาพชุมชน 2) หาจุดคุ้มทุนของศูนย์สุขภาพชุมชน 3) เปรียบเทียบต้นทุนต่อหน่วยบริการของหน่วยบริการปฐมภูมิหลักและหน่วยบริการปฐมภูมิรอง 4) เปรียบเทียบจุดคุ้มทุนของศูนย์สุขภาพชุมชนในหน่วยบริการปฐมภูมิหลักและหน่วยบริการปฐมภูมิรอง 5) เปรียบเทียบต้นทุนต่อหน่วยบริการและจุดคุ้มทุนของหน่วยบริการปฐมภูมิรองในสังกัดของโรงพยาบาลศูนย์กับโรงพยาบาลชุมชน การศึกษานี้เป็นการวิจัยเชิงสำรวจโดยใช้วิธีเก็บข้อมูลย้อนหลังและเก็บข้อมูลบางส่วนศึกษาไปข้างหน้าช่วงเวลา 6 เดือนแรกของปีงบประมาณ 2545 ประชากรที่ศึกษา คือศูนย์สุขภาพชุมชนในหน่วยบริการปฐมภูมิหลักจำนวน 7 แห่งและหน่วยบริการปฐมภูมิรองจำนวน 13 แห่งจากการสุ่มตัวอย่างแบบชั้นภูมิจากหน่วยบริการปฐมภูมิหลักจำนวน 18 แห่ง และหน่วยบริการปฐมภูมิรอง จำนวน 30 แห่ง เครื่องมือที่ใช้ในการวิจัยคือ แบบบันทึกข้อมูล ศึกษาเฉพาะต้นทุนดำเนินการโดยสำรวจสัดส่วนการใช้เวลาของเจ้าหน้าที่และต้นทุนค่าวัสดุ
การคิดจุดคุ้มทุนใช้สูตรคำนวณโดยใช้ค่าเฉลี่ยต้นทุนต่อหน่วยบริการเป็นราคาที่ให้บริการโดยใช้สถิติ แมนวิทนีย์ ยูและวิลค์อกซอน ผลการศึกษาพบ 1) ศูนย์สุขภาพชุมชนในหน่วยบริการปฐมภูมิหลักจัดบริการสุขภาพได้ 6 กิจกรรม และหน่วยบริการปฐมภูมิรองจัดไต้ 9 กิจกรรมจากกิจกรรมหลัก 11 กิจกรรมค่าเฉลี่ยต้นทุนต่อหน่วยบริการ คือ 73.91 และ 83.52 บาทต่อครั้งตามลำดับ 2) จุดคุ้มทุนของหน่วยบริการปฐมภูมิหลักและหน่วยบริการปฐมภูมิรองจาก 6 กิจกรรมในข้อ 1 ค่าเฉลี่ย คือ 2,123.15 และ 1,457.21 ครั้งต่อปี ซึ่งหน่วยบริการปฐมภูมิหลักมีจำนวนการให้บริการตํ่ากว่าจุดคุ้มทุน แต่หน่วยบริการปฐมภูมิรองมีจำนวนการให้บริการสูงกว่าจุดคุ้มทุน 3) ต้นทุนต่อหน่วยบริการการตรวจรักษาและการส่งเสริมสุขภาพของหน่วยบริการปฐมภูมิหลักกับปฐมภูมิรองมีความแตกต่างกันระดับนัยสำคัญ 0.01 4) จุดคุ้มทุนการตรวจรักษาของหน่วยบริการปฐมภูมิหลักกับปฐมภูมิรองมีความแตกต่างกันระดับนัยสำคัญ 0.05 และจุดคุ้มทุนการตรวจรักษาของหน่วยปฐมภูมิหลักจะมีค่ามากกว่าหน่วยปฐมภูมิรอง 5) จุดคุ้มทุนบริการก่อนกลับบ้านของหน่วยบริการปฐมภูมิรองในสังกัดโรงพยาบาลศูนย์ลับโรงพยาบาลชุมชนมีความ

The study aimed to: 1) explore the unit cost of Primary Care Unit. 2) find out the basic breakeven of Primary Care Unit. 3) compare the unit cost of Primary Care Unit between the main and sub-contractors. 4) compare the basic breakeven of Primary Care Unit between main and sub-contractors. 5) compare the unit cost, basic breakeven of sub-contractors network between the Regional Medical Center and Community hospitals. The methodology included labor cost, material cost, variable cost, and the volume of services in Primary Care Unit’s document routine reports from retrospective and prospective data carried out between January and June 2002. The unit cost was computed by using formula: total cost divided by output (volume per visit) and the basic breakeven was computed by using formula: total fixed cost divided by average profit. The studied population consisted of main and sub-contractors of Primary Care Unit in Ubonratchatani Province. Stratified sampling was conducted to identify 7 of main and 13 sub-contractors. Data was analyzed Mann - Whitey บ test and Kruskal Wallis test. It was found that all main and sub-contractors of the Primary Care Unit in Ubonratchatani Province could not provide every health services to the providers. Results were showed that: 1.) The average of unit cost’s six health services was 73.91 and 83.52 Baht per year in main and sub - contractors, 2.) The average of six health services’ basic breakeven was 2,123.15 andl, 457.21 times per year in main and sub - contractors. Almost of the basic breakeven of the health services in main contractors was lower than the average, on the other hand the basic breakeven of the sub-contractors were height. 3.) Significantly the unit cost of some health services in main and sub-contractors differed (p<0.01) using the Mann-Whitey บ test. The unit cost of health services at main contractors was higher than sub-contractors.4) Both main and sub contractors, the basic breakeven of curative care (p<0.()5). 5) The basic breakeven of curative care and exit care in the sub-contractors network at Regional Medical Center was higher than community hospitals and were statically significant (p<0.05)
