Abstract
การวิจัยครั้งนี้เป็นการวิจัยและพัฒนา มีวัตถุประสงค์เพื่อพัฒนารูปแบบการบันทึกทางการพยาบาลโดยใช้กระบวนการพยาบาลงานผู้ป่วยในโรงพยาบาลราษีไศล จังหวัดศรีสะเกษ และศึกษาประสิทธิภาพของรูปแบบการบันทึกทางการพยาบาล
กลุ่มตัวอย่าง เลือกแบบเจาะจงคือ พยาบาลวิชาชีพงานผู้ป่วยใน จำนวน 12 คน และแบบบันทึกทางการพยาบาลงานผู้ป่วยในโรงพยาบาลราษีไศล ก่อนทดลองจำนวน 30 แฟ้ม หลังทดลองจำนวน 30 แฟ้ม เครื่องมือที่ใช้ในการวิจัยได้แก่ (1) แบบสำรวจสภาพการณ์ของการบันทึกทางการพยาบาล (2) แผนการสอนพยาบาลวิชาชีพ เรื่อง การบันทึกทางการพยาบาล (3) แบบทดสอบความรู้ของพยาบาลวิชาชีพ เรื่องการบันทึกทางการพยาบาลและการใช้กระบวนการพยาบาล (4) แบบสอบถามความคิดเห็นของพยาบาลวิชาชีพต่อประสิทธิภาพของรูปแบบการบันทึกทางการพยาบาล และ (5) แบบตรวจสอบคุณภาพของการบันทึกทางการพยาบาล เครื่องมือผ่านการตรวจสอบความตรงของเนื้อหาโดยผู้ทรงคุณวุฒิจำนวน 6 คน เครื่องมือชุดที่ 3 ได้ค่าดัชนีความสอดคล้องของแบบทดสอบ = 0.89 เครื่องมือชุดที่ 4 และ 5 ได้ค่าดัชนีความตรงเชิงเนื้อหา = 0.90 และ 0.79 ตามลำดับ และได้ค่าความเที่ยง = 0.98 และ 0.94 ตามลำดับ วิเคราะห์ข้อมูลคุณภาพของการบันทึกทางการพยาบาลโดยใช้สถิติทดสอบที และวิเคราะห์ข้อมูลความคิดเห็นของพยาบาลวิชาชีพต่อประสิทธิภาพของรูปแบบการบันทึกทางการพยาบาลโดยหาค่าเฉลี่ยอันดับที่ค่ามัธยฐาน ทดสอบความแตกต่างด้วยสถิติวิลคอกซัน ผลการวิจัยสรุป ดังนี้ (1) ได้รูปแบบการบันทึกทางการพยาบาลเป็นการเขียนบรรยายในแบบบันทึกตามกระบวนการพยาบาล โดยใช้รูปแบบโฟคัส ประกอบด้วย ปัญหา / ข้อวินิจฉัยทางการพยาบาลบันทึกความก้าวหน้าทางการพยาบาลในรูปแบบการประเมินการปฏิบัติการพยาบาล และการประเมินผล (2) ผลการทดสอบความรู้ของพยาบาลวิชาชีพมีผู้ได้คะแนนหลังทดลองสูงกว่าก่อนทดลอง ร้อยละ 83.33 (3) ค่ามัธยฐานของความคิดเห็นของพยาบาลวิชาชีพเกี่ยวกับประสิทธิภาพของรูปแบบการบันทึกทางการพยาบาลหลังทดลองสูงมีค่าเฉลี่ยมัธยฐานกว่าก่อนทดลองอย่างมีนัยสำคัญทางสถิติที่ระดับ .05 และ (4) คุณภาพของการบันทึกทางการพยาบาลโดยรวมทุกด้านค่าเฉลี่ยหลังทดลองสูงกว่าก่อนทดลองอย่างมีนัยสำคัญทางสถิติที่ระดับ .05
This is a research and development study. The objectives were to develop a nurses’ notes model by using the nursing process for the In-patient Department at Rasisalai Hospital, Sisaket Province and to study the effects of that nurses’ notes model. The sample was selected by the purposive sampling technique and included 12 nurses as well as nurses’ notes (30 files before and 30 files after experiment) at Rasisalai Hospital, Sisaket Province. Research tools comprised: (1) a survey of a nurses’ notes situation, (2) lesson plans focused on how to do nurses’ notes and the nursing process for professional nurses, (3) a knowledge test of how to do nurses’ notes and the nursing process [This test was verified by 6 experts, and IOC was 0.89], (4) questionnaires which asked professional nurses their opinion of the nurses’ notes model, (5) a form for checking quality of nurses’ notes These questionnaires (item 4) and the checking quality form (item 5) were verified by experts, CVI were .0.90 and 0.79, and Cronbach’s alpha were 0.98 and 0.94]. Data were analyzed by statistics: t-test for quality of nurses’ notes, mean and median for opinion of professional nurses on the nurses’ notes model, and Wilcoxon for testing difference. The results were as follows. ( 1) The nurses’ notes model was in a form of descriptive writing according to the nursing process: focus - charting format. This included problems/ nursing diagnosis, progress note of nursing by evaluating, nursing intervention, and evaluation. (2) The scores of post-test was higher than that of pretest (83.30%). ( 3) The median value of opinion of professional nurses on the effectiveness of the nurses’ notes model after experimenting was significantly higher than before experimenting (p < .05). (4) The mean score of quality of nurses’ notes was significantly higher than before experimenting (p < .05).
