การพัฒนาระบบบันทึกทางการพยาบาลด้วยคอมพิวเตอร์สำหรับผู้ป่วยมะเร็ง ศูนย์มะเร็งลพบุรี

Abstract

การศึกษาครั้งนี้เป็นการวิจัยและพัฒนา มีวัตถุประสงค์ (1) เพื่อพัตเนาระบบบันทึกทาง การพยาบาล ด้วยคอมพิวเตอร์สำหรับผู้ป่วยมะเร็ง ศูนย์มะเร็ง ลพบุรี (2) เพื่อศึกษาผลของการใช้ระบบบันทึกทางการพยาบาลด้วยคอมพิวเตอร์สำหรับผู้ป่วยมะเร็ง ในด้านคุณภาพของการบันทึกทางการพยาบาล และประโยชน์ของระบบบันทึกทางการพยาบาลก่อนและหลังการพัฒนา กลุ่มตัวอย่างแบ่งเป็น 2 กลุ่มได้แก่ (1) พยาบาลวิชาชีพที่ปฎิบัติงานในหอผู้ป่วยมะเร็ง จำนวน 14 คน ได้จากการเลึอกตัวอย่างแบบเจาะจง และ (2) แบบบันทึกทางการพยาบาลของผู้ป่วย จำนวน 102 ชุด แบ่งเป็น แบบบันทึกด้วยระบบเดิม 51 ชุด ได้จากการสุ่มตัวอย่างแบบมีระบบ และแบบบันทึกด้วยระบบใหม่ 51 ชุด ได้จากการเลือกตัวอย่างแบบเจาะจง เครื่องมือทที่ใช้ในการวิจัย ได้แก่ (1) ระบบบันทึกทางการพยาบาลด้วยคอมพิวเตอร์สำหรับผู้ป่วยมะเร็ง (2) โครงการอบรมพยาบาลวิชาชีพและแบบทดสอบความรู้เรื่องการบันทึกทางการพยาบาลด้วยคอมพิวเตอร์ และ (3) เครื่องมือที่ใชัเก็บรวบรวมข้อมูลมี 2 ชุด ได้แก่ แบบตรวจสอบคุณภาพของการบันทึกทางการพยาบาล และแบบสอบถามประโยชน์ของระบบการบันทึกทางการพยาบาล เครื่องมือวิจัยได้ผ่านการหาความตรงตามเนื้อหาและความเที่ยง ได้ค่าความเที่ยง เท่ากับ 0.93 และ 0.91 ตามลำดับ วิเคราะห์ข้อมูลโดยหาค่าความถี่ ร้อยละ ค่าเฉลี่ย ส่วนเบี่ยงเบนมาตรฐาน Mann-Whitney U test และ สถิติทดสอบความแตกต่างของค่าเฉลี่ยในกลุ่มตัวอย่างสองกลุ่มที่มีความสัมพันธ์กับ (paired t-test) ผลการวิจัยสรุปได้ดังนี้ (1) ระบบบันทึกทางการพยาบาลด้วยคอมพิวเตอร์สำหรับผู้ป่วยมะเร็ง ศูนย์มะเร็ง ลพบุรี ประกอบด้วย วิธีการบันทึก แบบบันทึก และคู่มือการใช้ระบบบันทึกทางการพยาบาล ตามกระบวนการพยาบาลด้วยคอมพิวเตอร์ ซึ่งทำให้สามารถบันทึกได้ง่าย สะดวก ครอบคลุมความต้องการของผู้ป่ายแบบองค์รวมและต่อเนื่อง (2) ค่าเฉลี่ยของคะแนนคุณภาพของการบันทึกทางการ พยาบาลและประโยชน์ของระบบบันทึกโดยรวมหลังการพัฒนาสูงกว่าก่อนพัฒนาระบบบันทึกทางการพยาบาล ด้วยคอมพิวเตอร์อย่างมีนัยสำคัญทางสถิติที่ระดับ 0.01

The purposes of this research and development were: (1) to develop a computerized nursing documentation system for cancer patients at Lopburi Cancer Center and (2) to study the outcomes of using the computerized nursing documentation system in terms of quality of nursing documentation and to compare the advantages of the new documentation system with that of the old one. The sample comprised two groups: (1) fourteen professional nurses who worked in cancer wards were selected by purposive sampling and (2) 102 nursing records. These records were divided into 51 records using the previous charting system and selected by systematic sampling, the other 51 records using the new charting system, also selected by purposive sampling. Research tools consisted of (1) the computerized nursing documentation system, (2) a training project and a test on the computerized nursing documentation system, and (3) two types of data collection tools: (a) a quality of nursing record audit, and (b) questionnaires on the advantages of the nursing documentation system. Both tools were tested for validity and reliability. The reliabilities of the instruments were 0.93 and 0.91 respectively. Research data were analyzed by descriptive statistics (frequency, percentage, mean, and standard deviation), the Mann-Whitney บ test, and the paired t-test. The research findings were as follows. (1) The computerized nursing documentation system for cancer patients includes computerized documenting procedures, forms, and a manual which were developed aligned with a nursing process. The system contributed to easy and convenient charting, provided a holistic approach to meeting patients’ needs, and facilitated continuity of charting. (2) The mean scores for nursing documentation quality and for nurses’ opinion on the advantages of the documentation system after development of the new system were significantly higher than before at the level .01 (p < .01).
