Abstract
การวิจัยครั้งนี้มีวัตถุประสงค์เพื่อ (1) ศึกษาความรู้ ความคิดเห็น และกระบวนการบริหารเงินงบประมาณสาธารณสุขมูลฐานของอาสาสมัครสาธารณสุขประจำหมู่บ้าน ในอำเภอดำเนินสะดวก จังหวัดราชบุรี (2) ศึกษาปัจจัยที่มีความสัมพันธ์กับความรู้และความคิดเห็น (3) ศึกษาความสัมพันธ์ระหว่างความรู้ ความคิดเห็น กับกระบวนการบริหารเงินงบประมาณสาธารณสุขมูลฐาน (4) ศึกษาปัญหา อุปสรรค และข้อเสนอแนะ กลุ่มตัวอย่าง คือ อาสาสมัครสาธารณสุขประจำหมู่บ้านในเขตชนบท อำเภอดำเนินสะดวก จังหวดราชบุรี จำนวน 192 คน ที่ได้จากการคัดเลือกเจาะจงผู้ที่เป็นกรรมการเบิก-จ่ายเงินและกลุ่มผู้ที่ไม่ได้
เป็นกรรมการเบิก-จ่ายเงิน เครื่องมือที่ใช้ในการเก็บข้อมูลเป็นแบบสอบถาม และแนวการสนทนากลุ่ม สถิติที่ใช้ในการวิเคราะห์ข้อมูลได้แก่ ร้อยละ ค่าเฉลี่ย ค่าส่วนเบี่ยงเบนมาตรฐาน ค่าสัมประสิทธิ์สหสัมพันธ์แบบเพียร์สัน และการทดสอบไค-สแควร์
ผลการวิจัยพบว่า (1) อาสาสมัครสาธารณสุขในเขตอำเภอดำเนินสะดวกจังหวดราชบุรี มีความรู้ และความคิดเห็นเกี่ยวกับการบริหารงบประมาณสาธารณสุขมูลฐานอยู่ในระดับปานกลาง ส่วนใหญ่มีการปฏิบัติตามกระบวนการบริหารงบประมาณสาธารณสุขมูลฐาน (2) ปัจจัยที่มีความสัมพันธ์กับความรู้เกี่ยวกับการบริหารงบประมาณสาธารณสุขมูลฐาน ได้แก่ ระดับการศึกษา และตำแหน่งทางสังคม (p-value<0.01 และ 0.05 ตามลำดับ) ส่วนอายุมีความสัมพันธ์เชิงลบกับความรู้เกี่ยวกับการบริหารงบประมาณสาธารณสุขมูลฐาน (r = -0.182, p-value<0.05) ปัจจัยส่วนบุคคลและปัจจัยด้านการปฏิบัติงานทุกตัวแปรไม่มีความสัมพันธ์กับความคิดเห็นเกี่ยวกับการบริหารงบประมาณสาธารณสุขมูลฐาน (3) ความรู้ และความคิดเห็นมีความสัมพันธ์กับกระบวนการบริหารงบประมาณลาธารณสุขมูลฐานในภาพรวม (r = 0.266 และ 0.290 ตามลำดับ p-value<0.001) (4) ส่วนปัญหา/อุปสรรค พบว่าขาดความร่วมมือจากชุมชนในการจัดทำแผน ขาดความรู้เกี่ยวกับการบริหารงบประมาณสาธารณสุขมูลฐาน งบประมาณได้รับล่าช้า นำงบประมาณไปใช้ใม่ตรงตามแผนขาดการเก็บหลักฐานใบสำคัญการเงิน และ
ไม่มีการตรวจสอบการใช้จ่ายเงินจากคณะกรรมการ
The purposes of this research were (1) to identify knowledge, opinions and management process of Primary Health Care (PHC) budget among the Village Health Volunteers (VHVs) in Damnoensaduak District, Ratchaburi Province; (2) to examine factors related to knowledge and opinions; (3) to explore the relationships between knowledge, opinions and management process of PHC budget; and (4) to determine problems, obstacles and suggestions. The respondents were 192 VHVs in ral area of Damnoensaduak District, Ratchaburi Province comprised financial committees by purposive sampling, and non-financial committees by simple random sampling. The instruments used were questionnaires and focus group interviews. Data were analyzed by using percentage, mean, standard deviation, Pearson’s Product Moment Correlation Coefficient and Chi-square test. Research findings showed that, (1) knowledge and opinions about PHC budget management process among VHVs in Damnoensaduak District, Ratchaburi Province were at a moderate level, most of the samples carried on activities according to the PHC budget management process; (2) education and social position were found to be positive related to knowledge about PHC budget management process (p-value<0.01,0.05), age was found to be negative related (r=-0.182, p-value<0.05), personal and performance factors were not related to the opinions about PHC budget management process; (3) overall knowledge and opinions were significantly and positively related to PHC budget management process at the 0.001 level (r=0.266, 0.290); (4) the relevant problems and obstacles were the lack of community’s cooperation on PHC planning and knowledge of the PHC budget management, the delay of the PHC budget allocation, non-expenditure of the budget as planned, there was no collation of the payment slips and no monitoring of the expenses from the committees
