Abstract
การวิจัยนี้มีวัตถุประสงค์เพื่อ (1) ศึกษาความคิดเห็นของพยาบาลต่อการดำเนินงานประกันคุณภาพการพยาบาล (2) เปรียบเทียบความคิดเห็นของพยาบาลต่อการดำเนินงานประกันคุณภาพการพยาบาลในโรงพยาบาลชุมชนที่มีขนาดต่างกัน (3) ศึกษาปัญหา อุปสรรค และข้อเสนอแนะในการดำเนินงานประกันคุณภาพการพยาบาล กลุ่มตัวอย่างที่ศึกษาในครั้งนี้ คือ พยาบาลหัวหน้างานและพยาบาลผู้ปฏิบัติงานสังกัดกลุ่มงานการพยาบาลโรงพยาบาลชุมชน จังหวัดศรีสะเกษ ซึ่งได้มาจากการสุ่มตัวอย่างแบบจัดชั้นจำนวน 250 คน ใช้แบบสอบถามในการเก็บรวบรวมข้อมูล และวิเคราะห์ข้อมูลโดยการหาค่าร้อยละ
ค่าเฉลี่ย ส่วนเบี่ยงเบนมาตรฐาน และหาค่าความแปรปรวนทางเดียว ผลการวิจัย พบว่า (1) ความคิดเห็นของพยาบาลโรงพยาบาลชุมชนจังหวัดศรีสะเกษต่อภาพรวมการดำเนินงานประกันคุณภาพการพยาบาลอยู่ในระดับดี เมื่อจำแนกเป็นรายด้านพบว่า ความคิดเห็นด้านการเตรียมการและการจัดการดำเนินงานประกันคุณภาพการพยาบาลอยู่ในระดับดี ด้านการดำเนินงานตามขั้นตอนอยู่ในระดับปานกลาง และด้านการประเมินผลการดำเนินงานประกันคุณภาพการพยาบาลอยู่ในระดับปานกลาง (2) พยาบาลที่สังกัดโรงพยาบาลที่มีขนาดต่างกันมีความคิดเห็นต่อการดำเนินงานประกันคุณภาพการพยาบาลแตกต่างกันอย่างมีนัยสำคัญทางสถิติที่ระดับ 0.01 โดยโรงพยาบาลขนาด 90 เตียงขึ้นไป และโรงพยาบาลขนาด 60 เตียง มีการดำเนินงาน
ดีกว่าโรงพยาบาลขนาด 30 เตียง (3) ปัญหาและอุปสรรคในการดำเนินงานประกันคุณภาพการพยาบาล ได้แก่ ความไม่ต่อเนื่องในการดำเนินงาน ขาดการนิเทศ ติดตาม ประเมินผล ขาดการเตรียมบุคลากร บุคลากรขาดความรู้ความเข้าใจ ไม่ให้ความสำคัญ ขาดแคลนบุคลากร ภาระงานมากเกินไป และลักษณะงานเวร เข้า บ่าย ดึก ส่วนข้อเสนอแนะในการดำเนินงานประกันคุณภาพการพยาบาลนั้น ควรมีการอบรมเพิ่มความรู้ความเข้าใจเกี่ยวกับการประกันคุณภาพการพยาบาลมีความต่อเนื่องในการดำเนินงาน มีโรงพยาบาลนำร่องพัฒนาเป็นเครือข่าย มีการแลกเปลี่ยนความคิดเห็นระหว่างโรงพยาบาล ผู้บริหารเห็นความสำคัญ และปฏิปัติอย่างจริงจัง
The objectives of this research were to (1) study the nurse’s opinion about implementation of nursing quality assurance; (2) compare the differences of hospitals; and (3) study the problems, obstacles and suggestions on the implementation of nursing quality assurance. A total of 250 nursing supervisors and nursing operator who involved in the nursing department were selected by using stratified random sampling. Data collected by questionnaires were analyzed by using percentage, mean, standard deviation and one-way ANOVA. The research findings were (1) the implementation of nursing quality assurance in general in the community hospitals of sisaket province was in good level. When classified into each item, the opinions about the popuration and management of the implementation was in good level, the implementation complied with the process was in moderate level and the evaluation of the implementation of nursing quality assurance was in moderate level.(2) The different size of the hospitals differently in the implementation of nursing quality assurance with the statistical significant difference at level of 0.01.The implementation of the 90-bed and 60-bed community hospital were better than the 30-bed community hospital. (3) The problems and obstacles of the implementation of nursing quality assurance consisted lack of continuous operation, supervision, monitoring, evaluation and staff preparation; staff of the quality assurance knowledge did not realize its importance, staff inadequacy, over workload and type of shiftwork. These result suggested that additional training about the nursing quality assurance should be provided to continuous implementation should be performed. The pilot hospital should be developed as in the hospital network for exchange the ideas to each other. In addition, the administrators should realize on the importance of the nursing quality assurance and the implementation should be seriously perform in practice
