Abstract
การศึกษานี้มีวัตถุประสงค์เพื่อ 1) เปรียบเทียบผลการดำเนินงานตามตัวชี้วัดการใช้ยาอย่างสมเหตุผล และ 2) ศึกษาผลการประเมินตามเกณฑ์โรงพยาบาลส่งเสริมการใช้ยาอย่างสมเหตุผลของกลุ่มโรงพยาบาลศูนย์/โรงพยาบาลทั่วไป กับกลุ่มโรงพยาบาลชุมชนในสังกัดสำนักงานปลัดกระทรวงสาธารณสุข การศึกษาเชิงพรรณนาแบบภาคตัดขวาง ประชากรที่ศึกษาใช้ข้อมูลทุติยภูมิผลการดำเนินงานของโรงพยาบาลศูนย์/โรงพยาบาลทั่วไป จำนวน 110 แห่ง และโรงพยาบาลชุมชน จำนวน 705 แห่ง ของกระทรวงสาธารณสุข ปีงบประมาณ 2563 (1 ตุลาคม 2562 – 30 กันยายน 2563) วิเคราะห์ข้อมูลโดยใช้สถิติ ร้อยละ การทดสอบที และไคสแควร์ ผลการศึกษาพบว่า 1) ผลการดำเนินงานตามตัวชี้วัดการใช้ยาอย่างสมเหตุผล พบความแตกต่างอย่างมีนัยสำคัญทางสถิติที่ระดับ 0.05 ของโรงพยาบาลทั้งสองกลุ่มจำนวน 17 ตัวชี้วัด จากจำนวนทั้งหมด 20 ตัวชี้วัด โดยกลุ่มโรงพยาบาลชุมชนมีผลการดำเนินงานที่ดีกว่ากลุ่มโรงพยาบาลศูนย์/โรงพยาบาลทั่วไป จำนวน 14 ตัวชี้วัด ส่วนกลุ่มโรงพยาบาลศูนย์/โรงพยาบาลทั่วไปมีผลการดำเนินงานที่ดีกว่ากลุ่มโรงพยาบาลชุมชน จำนวน 3 ตัวชี้วัด และ 2) ผลการประเมินตามเกณฑ์โรงพยาบาลส่งเสริมการใช้ยาอย่างสมเหตุผล พบความแตกต่างกันอย่างมีนัยสำคัญทางสถิติที่ระดับ 0.05 โดยกลุ่มโรงพยาบาลชุมชนมีผลการประเมินตามเกณฑ์ที่ดีกว่ากลุ่มโรงพยาบาลศูนย์/โรงพยาบาลทั่วไป ทำให้เห็นถึงความแตกต่างในการใช้ยาและปัญหาการใช้ยาที่พบบ่อยในโรงพยาบาล กระทรวงสาธารณสุขควรทบทวนเป้าหมายที่ดำเนินการอยู่ และกำหนดนโยบายให้เหมาะสมกับบริบทของโรงพยาบาลในแต่ละระดับ
This study aimed to (1) compare performance results according to the indicators of rational drug use (RDU) and (2) explore the evaluation results according to the criteria for promoting RDU at regional/general hospitals with those at community hospitals under the Office of the Permanent Secretary, Ministry of Public Health.This is a cross-sectional descriptive study using secondary data from performance outcome of 110 regional/general hospitals and 705 community hospitals under the Ministry of Public Health in the fiscal year 2020 (1 October 2019 – 30 September 2020). Data were analyzed using percentages, t-test and Chi-square test.The results showed that: (1) performance results according to the RDU indicators were found significantly different at the 0.05 level in 17 indicators out of a total of 20 indicators; the community hospitals had better performance than the regional/general hospitals on 14 indicators; as for the regional/general hospitals, performance indicators were better than the community hospitals on three indicators; and (2) evaluation results according to the criteria for promoting RDU hospitals showed significant difference at the 0.05 level; the community hospitals had better results regarding evaluation criteria than the regional/general hospitals. This highlights the differences in drug utilization and common drug-related problems in hospitals. Therefore, the Ministry of Public Health should review the current objectives and establish policies tailored to the context of hospitals at each level.
