Abstract
การวิจัยครั้งนี้มีวัตถุประสงค์เพื่อ (1) ศึกษาระดับประสิทธิผลการให้บริการผู้ติดเชื้อเอชไอวีและผู้ป่วย
เอดส์ของโรงพยาบาลของรัฐในจังหวัดพะเยา (2) เปรียบเทียบระดับประสิทธิผลการให้บริการผู้ติดเชื้อเอชไอวีและ
ผู้ป่วยเอดส์ของโรงพยาบาลของรัฐแต่ละแห่งในจังหวัดพะเยา (3) เปรียบเทียบประสิทธิผลการให้บริการผู้ติดเชื้อ
เอชไอวีและผู้ป่วยเอดส์ของโรงพยาบาลของรัฐในจังหวัดพะเยาตามสิทธิประโยชน์การรับบริการ (4) ศึกษาปัจจัยที่มี
อิทธิพลต่อประสิทธิผลการให้บริการผู้ติดเชื้อเอชไอวีและผู้ป่วยเอดส์ของโรงพยาบาลของรัฐในจังหวัดพะเยา และ
(5) ศึกษาจุดแข็ง จุดอ่อน โอกาส อุปสรรค และเสนอแนะแนวทางในการปรับปรุงการให้บริการผู้ติดเชื้อเอชไอวีและ
ผู้ป่วยเอดส์ในโรงพยาบาลของรัฐในจังหวัดพะเยา
การวิจัยครั้งนี้เป็นการวิจัยเชิงสำรวจ ประชากรคือผู้ติดเชื้อเอชไอวีและผู้ป่วยเอดส์ที่เข้ารับบริการจาก
โรงพยาบาลของรัฐในจังหวัดพะเยา 7 แห่ง จำนวน 390 คน การกำหนดขนาดกลุ่มตัวอย่างใช้สูตรของทาโร่ ยามาเน่
ใช้เทคนิคการสุ่มเลือกกลุ่มตัวอย่างแบบโควต้า เครื่องมือที่ใช้เก็บข้อมูล ได้แก่ แบบสอบถามและการสนทนากลุ่ม
สถิติที่ใช้ในการวิเคราะห์ข้อมูลได้แก่ วิเคราะห์ความถี่ ร้อยละ ค่าเบี่ยงเบนมาตรฐาน การทดสอบค่าที การทดสอบค่าเอฟ
และการวิเคราะห์การถดถอยแบบพหุคูณ
ผลการวิจัยพบว่า (1) ภาพรวมประสิทธิผลการให้บริการผู้ติดเชื้อเอชไอวีและผู้ป่วยเอดส์ของ
โรงพยาบาลของรัฐในจังหวัดพะเยา อยู่ในระดับมากกว่าร้อยละ 80 (2) ประสิทธิผลการให้บริการผู้ติดเชื้อเอชไอวีและ
ผู้ป่วยเอดส์ของโรงพยาบาลของรัฐในจังหวัดพะเยา มีความแตกต่างกันในบางโรงพยาบาล ซึ่งแบ่งออกเป็นสองกลุ่ม
ได้แก่ (2.1) กลุ่มที่มีประสิทธิผลการให้บริการสูง ได้แก่ โรงพยาบาลแม่ใจ โรงพยาบาลดอกคำใต้ และโรงพยาบาลปง
ซึ่งมีค่าเฉลี่ยแตกต่างกันกับ (2.2) กลุ่มที่มีประสิทธิผลการให้บริการต่ำกว่ากลุ่มแรก ได้แก่ โรงพยาบาลพะเยา
โรงพยาบาลจุน โรงพยาบาลเชียงคำ และโรงพยาบาลเชียงม่วน แต่ค่าเฉลี่ยประสิทธิผลการให้บริการไม่มีความ
แตกต่างกันหากเปรียบเทียบระหว่างโรงพยาบาลภายในกลุ่ม (3) ผู้ติดเชื้อเอชไอวีและผู้ป่วยเอดส์ของโรงพยาบาลของ
รัฐในจังหวัดพะเยา ที่ใช้สิทธิประโยชน์ในการบริการที่ต่างกันประเมินประสิทธิผลการให้บริการของโรงพยาบาลไม่
แตกต่างกัน (4) ปัจจัยที่มีอิทธิพลต่อประสิทธิผลการให้บริการผู้ติดเชื้อเอชไอวีและผู้ป่วยเอดส์ของโรงพยาบาลของรัฐ
ในจังหวัดพะเยา ได้แก่ ปัจจัยกระบวนการบริหารแบบมุ่งผลสัมฤทธิ์ ปัจจัยมาตรฐานการบริการด้านเอดส์ และปัจจัย
สมรรถนะระบบบริการสุขภาพ (5) จุดแข็งการให้บริการพบว่าโรงพยาบาลของรัฐสามารถให้บริการอย่างเสมอภาค
และเท่าเทียมกัน มีการทำงานเป็นทีม ในขณะที่จุดอ่อนการให้บริการพบว่า บุคลากรและเครื่องมือการให้บริการมีไม่
เพียงพอ สถานที่ให้บริการคับแคบ ทำให้การให้บริการล่าช้า ประกอบกับผู้รับบริการด้านเอดส์ที่เพิ่มจำนวนมากขึ้น
ข้อเสนอแนะได้แก่ (1) พัฒนางานบริการผู้ติดเชื้อเอชไอวีและผู้ป่วยเอดส์ที่ทำอยู่ประจำให้เป็นผลงาน
ทางวิชาการสำหรับกลุ่มโรงพยาบาลที่มีประสิทธิผลการให้บริการที่สูง (2) ใช้การจัดการเชิงกลยุทธ์และการเทียบเคียง
ผลงานการให้บริการกับโรงพยาบาลที่มีบริการที่เป็นเลิศควบคู่กับการพัฒนางานอย่างต่อเนื่องและยั่งยืน และ (3) เพิ่ม
ทรัพยากรนำเข้าเพื่อให้บริการที่เพิ่มขึ้น เช่น แพทย์ พยาบาล อุปกรณ์ทางการแพทย์ อาคารสถานที่ พร้อมทั้งปรับปรุง
พฤติกรรมและขยายช่องทางการให้บริการ เพื่อการให้บริการด้านเอชไอวี/เอดส์ที่เป็นเลิศถ้วนทั่วทั้งจังหวัด
The purposes of this study are to (1) study the effectiveness of health services among people living with HIV/AIDS (PHA) in government hospitals of Phayao province (2) compare the effectiveness of health services among PHA in each hospitals and health service benefits (3) study influencing factors of effectiveness of health services among PHA, and (4) study the strengths, weaknesses, opportunities, threats and suggestions of health services among PHA in government hospitals, Phayao province. This study was a survey research. Populations consisted of 3,290 of HIV/AIDS Day Care Center’s service recipients, samples were 390 HIV infected and AIDS patients accessing health services in government hospitals during March-April 2010. Sample size was determined via Taro Yamane method, quota sampling technique was also applied. Two sets of questionnaires were developed to collect quantitative data together with a focused group discussion which, was employed to collect qualitative data. Statistical tools applied were frequency, percentage, standard deviation, t-test, F-test, and multiple regression analysis. The research results reveals that (1) the effectiveness of health services among PHA in the government hospitals in Phayao province was higher than 80% (2) the effectiveness of health services among PHA was significantly different in some hospitals which, are divided into two groups: (a) the group having high-effectiveness of services, including Mae Jai, Dok Kham Tai and Pong hospitals, and (b) the group having effectiveness of services lower than the first group including Phayao, Jun, Chiang Kham and Chiang Muan hospitals. However, the average of service’s effectiveness was not different when compared between hospitals within the group. The same result also appeared when compared to each health service benefits provided by each hospitals (3) Influencing factors of effectiveness of health services among PHA in Phayao’s government hospitals were Result- Based Management, HIVQUAL-T, and Health Service System Competencies respectively, and (4) the strengths of health services among PHA is equitable services provided by the hospital’s service teamwork, while the weaknesses are lacking of service resources such as physician, nurse, equipment and service area, these factors are significantly affect to service quality when the numbers of PHA is continuously increasing. The suggestions are to (1) develop service system among PHA group by using Routine to Academic (R2A) measure for the high-effectiveness hospitals. (2) use strategic management and benchmarking to develop service system for hospitals measured lower-effectiveness together with applying Research and Development (R&D) measure for sustainable improvements, and (3) input sufficient service resources, such as physicians, service rooms and buildings, medical equipments and service packages together with improving service behaviors and service channels of service providers in order to deliver HIV/AIDS universal excellent services across the province
