คุณภาพชีวิตด้านสุขภาพช่องปากของผู้สูงอายุที่มารับบริการใส่ฟันปลอมถอดได้ฐานอะคริลิก โรงพยาบาลเจ้าพระยาอภัยภูเบศร

Abstract

การศึกษาครั้งนี้มีวัตถุประสงค์เพื่อศึกษา 1) ปัจจัยส่วนบุคคลและคุณภาพชีวิตในมิติสุขภาพช่องปากของผู้สูงอายุ และ 2) เปรียบเทียบคุณภาพชีวิตในมิติสุขภาพช่องปากของผู้สูงอายุหลังการใส่ฟันปลอมถอดได้ฐานอะคริลิก โดยใช้รูปแบบการวิจัยเชิงสำรวจแบบภาคตัดขวาง ประชากรในการศึกษาครั้งนี้คือผู้สูงอายุที่มารับบริการใส่ฟันปลอมถอดได้ฐานอะคริลิก โรงพยาบาลเจ้าพระยาอภัยภูเบศร จำนวน 203 คน สุ่มตัวอย่างโดยการเลือกตัวอย่างแบบมีระบบ จำนวน 135 คนเก็บข้อมูลโดยการสัมภาษณ์ตามแบบสอบถาม แบ่งเป็น 2 ตอน คือ ตอนที่ 1 ปัจจัยส่วนบุคคล ได้แก่ เพศ อายุ การศึกษา รายได้ต่อเดือน ที่อยู่อาศัย การกลับมาตรวจเช็ค และสิทธิการรักษาพยาบาล ตอนที่ 2 คุณภาพชีวิตในมิติสุขภาพช่องปาก ตรวจสอบความตรงของเครื่องมือด้วยดัชนีความสอดคล้อง วิเคราะห์ข้อมูลโดยใช้สถิติ ร้อยละ ค่าเฉลี่ย แมนน์-วิทนีย์ ยู และ ครัสคัลวอลลิส ผลการศึกษาพบว่า 1) ผู้สูงอายุส่วนใหญ่เป็นเพศหญิงมีอายุอยู่ในช่วง 60-69 ปี จบการศึกษาระดับมัธยมศึกษา ไม่มีรายได้ อาศัยอยู่ในเขตเทศบาล เป็นสิทธิหลักประกันสุขภาพถ้วนหน้าและไม่เคยกลับมาตรวจเช็คฟันปลอม ระดับคุณภาพชีวิตในมิติสุขภาพช่องปากอยู่ในระดับดี โดยกิจกรรมที่เป็นปัญหาหลัก 3 อันดับแรก คือ การรับประทานอาหาร รองลงมาคือด้านการพูด และการทำความสะอาดช่องปากหรือฟันปลอม และ 2) ผู้สูงอายุที่มีการกลับมาตรวจเช็คต่างกัน มีคุณภาพชีวิตในมิติสุขภาพช่องปากแตกต่างกัน อย่างมีนัยสำคัญทางสถิติ ที่ระดับ 0.05

The objectives of this study were to: (1) identify personal factors and determine oral health-related quality of life; and (2) compare the oral health-related quality of life of elderly persons who wore removable acrylic dentures at Chaophraya Abhaibhubejhr Hospital. This cross-sectional study was carried out among a sample of 135 elderly persons selected using the systemic sampling method from 203 elders who wore removable acrylic dentures. Data were collected by interviewing the respondents using a structured questionnaire containing 2 parts: part 1, for personal factors including sex, age, educational background, income, residence, denture recheck, and health-care eligibility; and part 2, for oral health-related quality of life. The questionnaire content validity was checked with the item-objective congruence index. Data were analyzed to determine percentage and mean, and to perform Mann-Whitney U test and Kruskall-Wallis test. The results showed that: (1) the majority of respondents were female, aged 60–69 years, and eligible for universal health care; they completed secondary education, had no income, lived in municipal areas, and never came back to get their dentures rechecked; their oral health-related quality of life was at the good level; the three most common difficulties were eating, speaking and cleaning the mouth or dentures; and (2) the elderly who had dentures rechecked tended to have significant difference in their oral health-related quality of life (p value<0.05)
