การศึกษาค้นคว้าอิสระเรื่อง คู่มือการปฏิบัติงานรังสี : การจัดท่าถ่ายภาพกระดูกรยางค์ส่วนบนด้วยท่าพิเศษมีวัตถุประสงค์เพื่อ 1) รวบรวมการจัดท่าถ่ายภาพกระดูกรยางค์ส่วนบนด้วยท่าพิเศษ 2)วิเคราะห์ข้อมูลการจัดทำถ่ายภาพกระดูกรยางค์ส่วนบนด้วยท่าพิเศษและ 3) จัดทำคู่มือการปฏิบัติงานของแผนกรังสีวินิจฉัย ในการจัดท่าถ่ายภาพรังสีกระดูกรยางค์ส่วนบนด้วยท่าพิเศษ วิธีการศึกษาโดยการเก็บรวบรวมรายชื่อภาพถ่ายทางรังสีท่าพิเศษของกระดูกรยางค์ ส่วนบน ย้อนหลัง 1 ปี ที่แผนกรังสีวินิจฉัย โรงพยาบาลสุราษฎร์ธานี จังหวัดสุราษฎร์ธานีแล้วทำการสืบค้น รวบรวมข้อมูลการจัดท่าถ่ายภาพกระดูกรยางค์ส่วนบนด้วยท่าพิเศษจากตำราอินเทอร์เน็ต และแหล่งข้อมูลอื่นๆที่เกี่ยวข้อง วิเคราะห์เชิงเนื้อหา และจัดทำคู่มือการปฏิบัติงานของแผนกรังสีวินิจฉัย ในการจัดท่าถ่ายภาพกระดูกรยางค์ส่วนบนด้วยท่าพิเศษประกอบด้วย ชื่อท่าพิเศษ ข้อบ่งชี้หลัก การจัดท่าจุดกึ้งกลางของแสงตก สิ่งที่แสดงในภาพถ่ายรังสี ภาพถ่ายรังสีที่ได้จากการจัดท่า ข้อควรระวัง และตารางค่าปริมาณรังสี ประเมินคุณภาพคู่มือโดยผู้เชี่ยวชาญได้แก่ แพทย์รังสี 1 คน ศัลยแพทย์ 1 คน และผู้ปฏิบัติงานด้านรังสีการแพทย์ 10 คน เลือกผู้เชียวชาญอย่างเจาะจง เครื่องมือที่ใช้ในการตรวจสอบ คือ แบบสอบถามประเมินคุณภาพคู่มือการปฏิบัติงาน และแบบสัมภาษณ์แบบเจาะลึก สร้างโดยผู้ศึกษา ดำเนินการเก็บรวบรวมข้อมูลโดยการสำรวจและวิเคราะห์ข้อมูลเชิงคุณภาพ ผลการศึกษาค้นคว้าอิสระ พบว่า 1) รายชื่อการจัดท่าถ่ายภาพกระดูกรยางค์ส่วนบนด้วยท่าพิเศษ สามารถรวบรวมได้ทั้งหมด 25 ท่า 2) การวิเคราะห์และบันทึกการจัดท่ากระดูกรยางค์ส่วนบนด้วยท่า
พิเศษประกอบด้วย การถ่ายภาพหัวไหล่ 5 ท่า,ไหปลาร้า 4 ท่า,ข้อศอก 4 ท่า ข้อมือ 9 ท่า และมือ 3 ท่า 3) จัดทำคู่มือการจัดท่าถ่ายภาพกระดูกรยางค์ส่วนบนด้วยท่าพิเศษ ผลการประเมินคุณภาพคู่มือโดยผู้เชี่ยวชาญ พบว่าคู่มือการจัดท่าถ่ายภาพกระดูกรยางค์ส่วนบนด้วยท่าพิเศษ จัดอยู่ในระดับดีมาก มีการเรียบเรียงเนื้อหาอย่างเป็นระบบ มีการนำเสนอที่ชัดเจนสมควรได้รับการสนับสนุนเพื่อจัดทำเป็นคู่มือการปฏิบัติงาน ผลการประเมินคุณภาพคู่มือโดยผู้ปฏิบัติงานด้านรังสี พบว่าเนื้อหาถูกต้องตามหลักวิชาชีพรังสีเข้าใจได้ง่าย สามารถนำไปใช้ปฏิบัติงานได้จริง สมควรได้รับการสนับสนุนเพื่อจัดทำเป็นคู่มือการปฏิบัติงานประจำแผนกรังสีวินิจฉัย
This study aimed : (1) to collect special radiographic positions of upper limb bones; (2) to analyze data of the special radiographic positions of upper limb bones; and (3) to prepare a Manual for Radiological Procedure: Special Radiographic Positions of Upper Limb Bones, at Suratthani Hospital's Diagnostic Radiology Department in Surat Thani province. The study was carried out by collecting the names of special radiographic positions of upper limbs over the past one year at Suratthani Hospital's Diagnostic Radiology Department. Moreover, similar data were searched from relevant textbooks, the Intemet, other related resources; and content analysis was undertaken. After that a Manual for Radiological Procedure: Special Radiographic Positions of Upper Limb Bones was prepared, containing the names of special radiographic positions, main indications, positioning, center ray, indications in the radiograph, radiographic image positions, cautions and a table of radiation quantities. The manual's quality was evaluated by 3 purposively selected experts (one radiologist, one surgeon, and one radiology practitioner) and 10 radiological technologists. The instruments for data collection included a questionnaire for assessing the manual's quality and an in-depth interview form designed by the researcher. Content analysis was performed for qualitative data The results showed that: (1) twenty special radiographic positions of upper limbs were identified and a list of such positions was prepared; (2) according to the radiographic position analysis, 5 postures were for shoulders, 4 for collarbones, 4 for elbows, 9 for wrists, and 3 for hands; and (3) as evaluated by the experts, the Manual for Diagnostic Procedure was rated very good as its content was systematically organized with a clear presentation. Additionally, as evaluated by radiological technologists, the manual's content was consistent with the radiological profession principles and easy to understand; and it could be used in practice. Thus, the manual should be adopted for use at the Hospital's Diagnostic Radiology Department.
