Abstract
การศึกษานี้เป็นการวิจัยและพัฒนามีวัตถุประสงค์เพื่อ (1) ศึกษาสภาพการณ์การตัดสินใจเชิงจริยธรรมในการจัดการดูแลผู้ป่วย ระยะสุดท้ายของพยาบาลวิชาชิพ โรงพยาบาลพระมงกุฎเกล้า (2) พัฒนาโปรแกรมการตัดสินใจเชิงจริยธรรม และ (3) ประเมินความพึงพอใจโปรแกรมการตัดสินใจเชิงจริยธรรม ประชากรและผู้ให้ข้อมูลหลักได้รับการตัดเลือกแบบเฉพาะเจาะจง ประกอบด้วยพยาบาลวิชาชีพ 133 คน แพทย์ 5 คน และญาติผู้ป่วย 5 คน เครื่องมือที่ใช้ในการวิจัย ได้แก่ แบบสอบถาม ประเด็นสนทนา และ
แบบประเมินความพึงพอใจโปรแกรม การวิเคราะห์ข้อมูลจากแบบสอบถามและแบบประเมินความพึงพอใจใช้สถิติ พรรณนา ได้แก่ ค่าความถี่ ค่ารัอยละ ค่าเฉลี่ย ค่าเบี่ยงเบนมาตรฐาน และข้อมูลจากการสนทนาใช้การวิเคราะห์เนี้อหา ผลการศึกษาพบว่า (1) สภาพการณ์การตัดสินใจเชิงจริยธรรมในการจัดการดูแลผู้ป่วยระยะสุดท้ายครบ 4 ขั้นตอน ของกระบวนการพยาบาล แต่ละขั้นตอนมีค่าเฉลี่ยสูงสุด 4.05, 4.26, 4.25 4.41 ตามลำดับ และมีข้อที่ควรพัฒนาในแต่ละขั้นตอนซึ่งสอดคล้องกับข้อมูลไดัจากการสนทนาที่เกี่ยวข้อง ผู้วิจัยได้นำมาประกอบการพัฒนาโปรแกรม ได้แก่ การประเมินสภาพผู้ป่วยร่วมกับทีมสุขภาพ การวางแผนการพยาบาลครอบคลุมทั้งร่างกายจิตใจและจิตวิญญาณผู้ป่วย การนำมาตรา 12 ในพระราชบัญญิติสุขภาพแห่งชาติ พ.ศ. 2550 มา
ประกอบการตัดสินใจปฏิบัติการพยาบาล และการให้ญาติมีส่วนร่วมในการประเมินผลการดูแลผู้ป่วย (2) โปรแกรมการตัดสินใจเชิงจริยธรรม ประกอบด้วย 2 ส่วน คือ 1) แผนการสอนและกิจกรรมการสอน ได้แก่ การบรรยาย ชมภาพยนตร์ สวดมนต์ ทำสมาธิ และทำกิจกรรมกลุ่มย่อย และ 2) คู่มีอการสอนและสื่อการสอนอื่น ได้แก่ สถานการณ์จำลอง ซีดีเพลงบรรเลง และโปรแกรมนำเสนอด้วยคอมพิวเตอร์ (power point) (3) ผลการประเมินความพึงพอใจโปรแกรม พบว่า ผู้เข้าร่วมโปรแกรมมีความพึงพอใจอยู่ในระดับมากที่สุดและมาก (ร้อยละ 66.26 และ 33.34) ตามลำดับ และด้านประสิทธิผลการเข้าร่วมโปรแกรมมีค่าเฉลี่ยสูงสุดคือ 4.80 ผู้เข้าร่วมโครงการซึ่งเป็นพยาบาลวิชาชีพได้นำความรู้ไปใช้ในการปฏิบัติงามได้ ผลการวิจัยยังเป็นข้อมูลให้ผู้บริหารการพยาบาลและองค์กรสุขภาพเพื่อการตัดสินใจเชิงจริยธรรมในการจัดการดูแลผู้ป่วยระยะ
สุดท้ายได้อย่างมีประสิทธิภาพและประสิทธิผล
The purposes of the research and development were: 1) to analyze the situations of ethical decision making for taking care of patients with end of life by professional nurses in Phramongkutklao Hospital 2) to develop an ethical decision making program and 3) to evaluate satisfaction of the program for taking care of patients with end of life. Population and key informants which were selectec by using purposive sampling technique and comprised 133 professional nurses, 5 physicians, and 5 care givers. The research instruments comprised a questionnaire, semi-structure interview and the satisfaction evaluation form of the program. Data from the questionnaire and the satisfaction evaluation form were analyzed by frequency, percentage, mean, standard deviation; and the data from the interview was analyzed by content analysis. Research finding were as follows. (1) The situations of an ethical decision making for taking care of patients with end of life by Professional Nurses in Phramongkutklao Hospital included four steps of nursing process. The average value of each step were 4.05, 4.26,4.25 and 4.41 respectively. In each step which congruence with data from stakeholder’s interview should be implemented to the developed program, including evaluation with health care team, planning cover physical, mental and spiritual of patients, consideration section 12 in health national act 2550 into nursing practice and patients evaluation need care givers (2) The program consisted of 2 components : 1) lesson plan and planning activities, including lecturing, watching cinema, praying, meditation and small group activities. 2) manual for Palliative Care and other teaching media, such as simulation, CD song player, and power point presentation (3) satisfaction evaluation of the program is at the highest and high level (66.26% and 33.34%) respectively. The effectiveness of program’s participation is at the highest level 4.80. The program’s participants, Professional Nurses, have been implemented knowledge into their clinical setting. The research findings could be used as data to the Ethical decision making management for Taking Care of Patients with the End of Life by Nurses administrators and organizations effectively and efficiency.
