การศึกษาประสิทธิภาพการจัดการของโรงพยาบาลทหารบกโดยการวิเคราะห์เส้นห่อหุ้มข้อมูล

Abstract

การศึกษาครั้งนี้มีวัตถุประสงค์เพื่อ 1) ศึกษาประสิทธิภาพการจัดการของโรงพยาบาลทหารบก และ 2) ศึกษาแนวทางการเพิ่มประสิทธิภาพการจัดการของโรงพยาบาลทหารบก ประชากรที่ใช้ในการศึกษา คือ โรงพยาบาลทหารบกทั้ง 37 แห่ง ใช้ข้อมูลทุติยภูมิในการศึกษา โดยรวบรวมจากโรงพยาบาลทั้ง 37 แห่ง และกรมแพทย์ทหารบก ปีงบประมาณ 2561 ใช้เทคนิคการวิเคราะห์เส้นห่อหุ้มในการวัดประสิทธิภาพการจัดการของโรงพยาบาลทหารบก การวัดประสิทธิภาพการจัดการทรัพยากรสุขภาพใช้วิธีสองลำดับขั้น และการวัดประสิทธิภาพการจัดการด้านต้นทุนใช้แบบจําลองต้นทุนที่หาค่าต่ำที่สุด โดยวิเคราะห์แยกตามกลุ่มขนาดของโรงพยาบาลประกอบด้วยโรงพยาบาลระดับภาค ขนาดใหญ่ ขนาดกลางและขนาดเล็กผลการศึกษาพบว่า 1) ประสิทธิภาพการจัดการด้านทรัพยากรสุขภาพ มีค่าเฉลี่ยประสิทธิภาพเท่ากับ 0.889 จํานวนโรงพยาบาลที่มีประสิทธิภาพจํานวน 16 แห่ง ร้อยละ 43.2 และโรงพยาบาลที่ไม่มีประสิทธิภาพจํานวน 21 แห่ง ร้อยละ 56.8 ประสิทธิภาพการจัดการด้านต้นทุน มีค่าเฉลี่ยประสิทธิภาพเท่ากับ 0.989 โรงพยาบาลที่มีประสิทธิภาพจํานวน 24 แห่ง ร้อยละ 64.9 และโรงพยาบาลที่ไม่มีประสิทธิภาพจํานวน 13 แห่ง ร้อยละ 35.1 และ 2) แนวทางการเพิ่มประสิทธิภาพด้านทรัพยากรสุขภาพของโรงพยาบาลที่ไม่มีประสิทธิภาพ พบว่า สามารถลดปัจจัยนําเข้าลงเฉลี่ยร้อยละ 11.1 โดยการดำเนินงานอยู่ในช่วงผลได้ต่อขนาดเพิ่มขึ้น ซึ่งปัจจัยนำเข้าสำคัญที่สามารถจัดสรรได้ คือ จํานวนพยาบาลและจํานวนแพทย์ โดยจัดสรรให้เหมาะสมกับอัตราการให้บริการในกลุ่มโรงพยาบาลทหารบก สําหรับแนวทางการเพิ่มประสิทธิภาพด้านการจัดการด้านต้นทุนของโรงพยาบาล พบว่า การดำเนินงานอยู่ในช่วงผลได้ต่อขนาดลดลง การลดปัจจัยนําเข้าไม่สามารถเพิ่มประสิทธิภาพได้ ดังนั้นจึงควรมีการปรับเปลี่ยนนโยบายการบริหารจัดการด้านต้นทุนของโรงพยาบาลทหารบก

The objectives of this study were to 1) study the management efficiency of the Royal Thai Army Hospital and 2) study the guidelines for increasing the management efficiency of the Royal Thai Army Hospital. The population used in the study was 37 Royal Thai Army hospitals. The secondary data used in this study were collected from 37 hospitals and the Royal Thai Army Medical Department in the fiscal year 2018. Data Envelopment Analysis was used to measure the management efficiency of the Royal Thai Army Hospital. The twostage process was applied for health resource managing efficiency measurement and cost minimization model was employed to measure the efficiency of cost management. The hospitals were categorized by size which included the large, medium, and small size regional hospitals. The results of the study showed that 1) in the case of efficiency of health resource management, The mean efficiency was 0.889, there were 16 efficient hospitals, 43.2%, and 21 inefficient ones, 56.8%. In the case of cost management efficiency, The mean efficiency was 0.989, there were 24 efficient hospitals, 64.9%, and 13 inefficient ones, 35.1% and 2) for the approach to increase the efficiency of health resources of inefficient hospitals, it shows that it was able to reduce 11.1 percent of inputs by operating in the range of increasing return to scale. The crucial inputs that could be allocated were the number of nurses and doctors by allocating them to fit with the rate of service provided by the Army Hospital Group. For the guidelines to improve the cost management efficiency of the hospitals, it found that the operations in the range of decreasing return to scale and the reduction of inputs cannot increase the efficiency. Therefore the cost management policy of the Royal Thai Army Hospital should be adjusted
