ปัญหาและผลกระทบของครอบครัวไทยที่มีบุตรเป็นดาวน์ซินโดรม : กรณีศึกษาเฉพาะครอบครัวที่ด้อยโอกาส

Abstract

การวิจัยครั้งนี้มีวัตถุประสงค์เพื่อศึกษา (1) ปัญหาและผลกระทบของครอบครัวที่มีบุตรเป็นดาวน์ซินโดรม และ (2) วิธีการดูแลบุตรที่เป็นดาวน์ซินโดรม วิธีการวิจัยเป็นการวิจัยเชิงคุณภาพ กลุ่มตัวอย่าง แบ่งออกเป็น (1) ผู้ปกครองเด็กดาวน์ซินโดรม จำนวน 7 คน (2) บุคลากรภายในสำนักงานมูลนิธิช่วยคนปัญญาอ่อนแห่งประเทศไทย ในพระบรมราชินูปถัมภ์ จำนวน 15 คน รวมจำนวนทั้งสิ้น จำนวน 22 คน โดยใช้วิธีการสัมภาษณ์ การสังเกต และศึกษาจากเอกสารการวิเคราะห์ข้อมูลเป็นการวิเคราะห์เชิงพรรณนา ผลการวิจัยพบว่า (1) ปัญหาของครอบครัวที่มีบุตรเป็นดาวน์ซินโดรม แบ่งเป็น ปัญหาต่อตัว ผู้ปกครอง และปัญหาตัวเด็กที่มีอาการดาวน์ซินโดรม โดยผลกระทบต่อผู้ปกครองประกอบด้วย ภาระในการเลี้ยงดู ค่าใช้จ่ายเพิ่มสูงขึ้น ความสัมพันธ์ระหว่างคู่สมรส ความรู้ความเข้าใจในการเลี้ยงดูบุตรที่เป็นดาวน์ซินโดรม ปัญหาสุขภาพจิตและความเครียด และผลกระทบต่อตัวเด็กที่มีอาการดาวน์ซินโดรม ประกอบด้วย ด้านสุขภาพ ด้านการศึกษา ด้านการงาน ด้านการดูแลตัวเอง การยอมรับและการอยู่ร่วมกับ คนอื่นในสังคม (2) วิธีการดูแลบุตรที่เป็นดาวน์ซินโดรม จะต้องฝึกให้ช่วยเหลือตัวเอง และอยู่ร่วมกับคนอื่นในสังคมหรือดำรงชีวิตอยู่ในสังคมให้ได้เหมือนคนปกติมากที่สุด จึงต้องมีการฝึกทักษะสำคัญ ๆ คือ ทักษะด้านกล้ามเนื้อมัดใหญ่ ที่มีความจำเป็นต้องใช้ในการเคลื่อนไหวทุกทักษะกล้ามเนื้อมัดเล็ก เพื่อฝึกการทำงานประสานกันของมือและตา ทักษะด้านความรู้ความเข้าใจ ฝึกความพร้อมด้านวิชาการ ทักษะด้านภาษา ฝึกการติดต่อสื่อสาร ทักษะการช่วยเหลือตนเอง ในการทำกิจวัตรประจำวัน ทักษะด้านสังคม ฝึกให้เด็กรู้จักปฏิบัติตัวตนต่อคนใกล้ชิด

The objectives of this research were (1) to study problems and impacts the Thai families with children who have Down's syndrome; and (2) to study methods of caring for children with Down's syndrome. This was a qualitative research. The sample population consisted of (1) 7 parents of children with Down's syndrome; and (2) 15 personnel from the Foundation for the Welfare of the Mentally Retarded of Thailand Under the Royal Patronage, for a total of 22. Data were collected through interviews and observation and from documents. Data were analyzed through descriptive analysis. The results showed that (1) The families experienced problems for both the children with Down’s syndrome and their parents. The impacts on parents included the burden of care, extra expenses, the relationship between spouses, lack of knowledge and understanding about how to care for a special child, psychological problems and stress. The impacts on the children included health problems, education problems, occupation problems, problems with learning self care, acceptance by others in society and getting along with others in society. (2) The recommended method for taking care of a child with Down's syndrome is to teach them to be self sufficient and to teach them to learn how to live with other people in society in as normal a manner as possible. This entails skills training for skills such as gross motor coordination for the large muscle groups used in every movement, fine motor skills training to improve eye-to-hand coordination, cognition skills to prepare them for learning, language skills for effective communication, training on how to perform everyday tasks independently, and interpersonal skills to help them get along with other people.
