Abstract
การวิจัยและพัฒนานี้มีวัตถุประสงค์เพื่อ 1) ศึกษาสถานการณ์การนิเทศทางคลินิกของหัวหน้าหอผู้ป่วย 2) พัฒนารูปแบบการนิเทศทางคลินิกของหัวหน้าหอผู้ป่วย และ 3) ประเมินความเหมาะสมของรูปแบบการนิเทศทางคลินิกของหัวหน้าหอผู้ป่วย โรงพยาบาลทั่วไปแห่งหนึ่ง เขตสุขภาพที่ 2 กลุ่มตัวอย่างแบ่งตามขั้นตอนการศึกษา ดังนี้ ขั้นตอนที่ 1 ศึกษาสถานการณ์การนิเทศทางคลินิกของหัวหน้หอผู้ป่วย กลุ่มตัวอย่างเป็นหัวหน้าหอผู้ป่วย จำนวน 8 คน ชั้นตอนที่ 2 พัฒนารูปแบบการนิเทศทางคลินิกของหัวหน้าหอผู้ป่วยตามแบบจำลองการนิเทศเจ็ดตา กลุ่มตัวอย่างเป็นหัวหน้าหอผู้ป่วยใน จำนวน 8 คน ขั้นตอนที่ 3 ประเมินความเหมาะสมของรูปแบบการนิเทศทางคลินิกที่พัฒนาขึ้นกลุ่มตัวอย่างเป็นพยาบาลวิชาชีพ จำนวน 100 คน เครื่องมือการวิจัย ประกอบด้วย 1) แนวสนทนากลุ่มเพื่อศึกษาสถานการณ์การนิเทศทางคลินิกของหัวหน้าหอผู้ป่วย 2) แนวสนทนากลุ่มเพื่อพัฒนารูปแบบการนิเทศทางคลินิกของหัวหน้าหอผู้ป่วย และ 3) แบบสอบถามความเหมาะสมของรูปแบบการนิเทศทางคลินิกที่พัฒนาขึ้น ซึ่งมีค่สัมประสิทธิ์แอลาครอนบาคเท่ากับ .98 วิเคราะห์ข้อมูลโดยการวิเคราะห์เนื้อหาและสถิติเชิงพรรณนา ผลการวิจัยพบว่า 1) สถานการณ์การนิเทศทางคลินิกของหัวหน้าหอผู้ป่วย พบปัญหาการ นิเทศ ได้แก่ การดูแลทางคลินิกควรได้รับการปรับปรุง อัตรากำลังไม่เพียงพอ ผู้นิเทศไม่เห็นความสำคัญ และขาดความมั่นใจในการนิเทศ มีความต้องการแนวทางการนิเทศ แผนและคู่มือการนิเทศ 2) รูปแบบการนิเทศทางคลินิกของหัวหน้าหอผู้ป่วย ประกอบด้วยการนิเทศทางคลินิก 6 ด้าน ได้แก่ ด้านผู้ป่วย ด้านกิจกรรมการพยาบาล ด้านสัมพันธภาพระหว่างผู้ป่วยและพยาบาลวิชาชีพ ด้านพยาบาลวิชาชีพ ด้านสัมพันธภาพระหว่างพยาบาลวิชาชีพและหัวหน้าหอผู้ป่วย และด้านหัวหน้าหอผู้ป่วย 3) รูปแบบการนิเทศ ทางคลินิกของหัวหน้าหอผู้ป่วยที่พัฒนาขึ้น มีความเหมาะสมในระดับมาก
The purposes of this research and development were: 1) to study the situation of clinical supervision of head nurses, 2) to develop a clinical supervision model of head nurses, and 3) to evaluate the appropriateness of the clinical supervision model of head nurses at one General Hospital in Regional Health 2. The sample was divided according to the study stages. The first step: to study the situation of the clinical supervision of head nurses, the sample consisted of 8 head nurses. The second step: to develop a clinical supervision model of head nurses based on the seven-eyed supervision model, the sample consisted of 8 head nurses. The third step: to evaluate the appropriateness of the developed clinical supervision model, the samples were 100 professional nurses. Research tools include: 1) a focus group guideline for studying the situation of clinical supervision of head nurses, 2) a focus group guideline for developing a clinical supervision model of head nurses, and 3) a questionnaire for evaluation of the appropriateness of the developed clinical supervision model. The Cronbach coefficient was .98. Data were analyzed by content analysis and descriptive statistics. The study results indicated as follows. 1) Analysis results of the situation of clinical supervision of head nurses indicated problems as follows: poor clinical care, shortage of manpower, unconcern and lack of confidence of clinical supervisors, need of supervision guideline, plan, and manual. 2) The clinical supervision model of head nurses includes six clinical supervisions: the patients, the nursing interventions, the relationship between the patients and the registered nurses, the registered nurse, the relationship between the registered nurses and the head nurses, and the head nurses. 3) The appropriateness of the clinical supervision model of head nurses at a General Hospital in Regional Health 2 was rated at the high level.
