คู่มือการตรวจลำไส้ใหญ่ด้วยเอกซเรย์คอมพิวเตอร์ โรงพยาบาลราชบุรี

Abstract

โรคมะเร็งลำไส้ใหญ่เป็นมะเร็งที่พบบ่อยและมีแนวโน้มสูงขึ้นในประเทศไทยการตรวจคัดกรองที่ดีและถูกต้องจะช่วยให้พบรอยโรคได้ตั้งแต่ระยะก่อนเป็นมะเร็งหรือมะเร็งระยะแรก ทำให้สามารถป้องกันหรือให้การรักษาโรคได้อย่างทันท่วงที การศึกษาค้นคว้าอิสระนี้ จึงมีวัตถุประสงค์เพื่อจัดทำคู่มือการตรวจลำไส้ใหญ่ด้วยเครื่องเอกซเรย์คอมพิวเตอร์ โรงพยาบาลราชบุรี การจัดทำคู่มือได้จากการทบทวนเอกสารและงานวิจัยที่เกี่ยวข้อง เพื่อจัดทำร่างคู่มือการตรวจลำไส้ใหญ่ด้วยเครื่องเอกซเรย์คอมพิวเตอร์ ตรวจสอบความตรงด้านเนื้อหาโดยผู้เชี่ยวชาญ 3 คน ได้ค่าดัชนีความสอดคล้องอยู่ในเกณฑ์ที่ใช้ได้ทั้งหมด และภายหลังการปรับแก้ไข ได้นำคู่มือไปทดลองใช้กับผู้ปฏิบัติงานที่เกี่ยวข้องของโรงพยาบาลราชบุรีจำนวน 7 คน แล้วให้ประเมินความพึงพอใจต่อการใช้คู่มือดังกล่าว ผลการศึกษาพบว่า คู่มือการตรวจลำไส้ใหญ่ด้วยเครื่องเอกซเรย์คอมพิวเตอร์ โรงพยาบาลราชบุรีที่จัดทำขึ้น มีส่วนบทนำและส่วนเนื้อหา โดยส่วนเนื้อหาประกอบด้วย 9 ตอน ได้แก่ 1) กายวิภาคและสรีรวิทยาของลำไส้ใหญ่ 2) ความรู้เกี่ยวกับโรคมะเร็งลำไส้ใหญ่ 3) ความรู้เกี่ยวกับเครื่องเอกซเรย์คอมพิวเตอร์ 4) การเตรียมตัวผู้ป่วยก่อนเข้ารับการตรวจลำไส้ใหญ่ด้วยเอกซเรย์คอมพิวเตอร์ 5) ขั้นตอนการปฏิบัติของนักรังสีการแพทย์ในขณะตรวจ 6) ขั้นตอนการสร้างภาพ 3 มิติ 7) ภาพทางรังสีวิทยาของลำไส้ใหญ่ 8) บทบาทหน้าที่ของนักรังสีการแพทย์ และ 9) แนวทางป้องกันการเกิดมะเร็งลำไส้ใหญ่ สาหรับผลการทดลองใช้โดยผู้ปฏิบัติงานจำนวน 7 คน พบว่า ความพึงพอใจโดยรวมอยู่ในระดับมากที่สุด ซึ่งด้านเนื้อหาในเรื่องการประยุกต์ใช้ในการปฏิบัติงานได้จริงมีคะแนนสูงที่สุด

Colorectal cancer is a common cancer in Thailand with an increasing trend. Good and accurate screening will help to detect lesions in the pre pre-cancerous or early cancer stages, making it possible to prevent or provide timely treatment of the disease. This independent study aimed to create a manual of colonography with computed tomography in Ratchaburi Hospital. The manual preparation involved the reviews of relevant documents and previous studies; and then a manual for colonography with computed tomography was drafted. The content validity of the draft manual was checked by three experts and its index of objective congruence was consistent with the criteria. After revision, the manual was pre pre-tested with seven radiographers involved in computed tomography in the hospital; and then their satisfaction was assessed. The results have shown that the manual of colonography with computed tomography in Ratchaburi Hospital has two sections: introduction and content. The content section consists of nine parts: 1) Anatomy and physiology of the colon, 2) Knowledge of colon cancer, 3) Knowledge of computed tomography, 4) Preparation of the patient for a colonography, 5) Colonography procedures for a medical radiographer, 6) The process of creating 3D images, 7) Radiological image of the colon, 8) Roles and duties of a radiographer, and 9) Guidelines for the prevention of colon cancer. The manual pre pre-test among seven participants indicated their overall satisfaction with the manual at the highest level in all aspects, of which, the practical application had the highest score
