Abstract
การวิจัยนี้มีวัตถุประสงค์เพื่อ 1) ศึกษาสถานการณ์ด้านสื่อสารสุขภาพในพื้นที่ศึกษา 2) ศึกษาวิธีการสื่อสารสุขภาพในชุมชนที่ประสบความสำเร็จด้านการสื่อสารสุขภาพแบบมีส่วนร่วมโรคไม่ติดต่อเรื้อรัง และ 3) นำเสนอแนวทางและรูปแบบการจัดการสื่อสารสุขภาพแบบมีส่วนร่วมโรคไม่ติดต่อเรื้อรัง การวิจัยนี้เป็นการวิจัยเชิงคุณภาพโดยการเก็บข้อมูลจากการสัมภาษณ์เชิงลึก จากผู้นำชุมชน เจ้าหน้าที่สาธารณสุข และอาสาสมัครสาธารณสุขตำบลแคราย อำเภอกระทุ่มแบน จังหวัดสมุทรสาคร จำนวน 8 คน และผู้นำชุมชน เจ้าหน้าที่สาธารณสุข และอาสาสมัครสาธารณสุขตำบลแสนชาติ อำเภอจังหาร จังหวัดร้อยเอ็ด จำนวน 10 คน ร่วมกับการใช้การสนทนากลุ่ม จากผู้นำชุมชน เจ้าหน้าที่สาธารณสุข และอาสาสมัครสาธารณสุข ตำบลแคราย อำเภอกระทุ่มแบน จังหวัดสมุทรสาคร จำนวน 11 คน และผู้นำชุมชน เจ้าหน้าที่สาธารณสุข และ อาสาสมัครสาธารณสุขตำบลแสนชาติ อำเภอจังหาร จังหวัดร้อยเอ็ด จำนวน 10 คน วิเคราะห์ข้อมูลโดยการสร้างข้อสรุป ผลการวิจัย พบว่า 1) สถานการณ์การด้านสื่อสารสุขภาพในพื้นที่ศึกษา ศึกษาในพื้นที่ชุมชนตำบล แคราย อำเภอกระทุ่มแบน จังหวัดสมุทรสาคร ประกอบไปด้วย 3 ส่วน คือ นโยบายสาธารณสุข แผนสุขภาพ ชุมชน และการดำเนินงานสุขภาพชุมชน ด้านนโยบายสาธารณสุขจะถูกกำหนดมาจากกระทรวงสาธารณสุข และ องค์กรบริหารจัดการด้านสุขภาพระดับพื้นที่เป็นผู้จัดทำแผนสุขภาพชุมชนเอง เพราะเป็นชุมชนเมืองคนในชุมชน ต้องประกอบอาชีพ ไม่มีเวลาร่วมประชาคมเพื่อทำแผนสุขภาพ ทำให้คนในชุมชนมองไม่เห็นเป้าหมายร่วมกันในการขับเคลื่อนในการการดำเนินงานสุขภาพชุมชน 2) วิธีการสื่อสารสุขภาพในชุมชนที่ประสบความสำเร็จด้าน การสื่อสารสุขภาพแบบมีส่วนร่วมโรคไม่ติดต่อเรื้อรัง ศึกษาในพื้นที่ชุมชนตำบลแสนชาติ อำเภอจังหาร จังหวัด ร้อยเอ็ด ประกอบไปด้วย 3 ส่วน คือ นโยบายสาธารณสุข แผนสุขภาพชุมชน และการดำเนินงานสุขภาพชุมชน โดยนโยบายสาธารณสุขถูกกำหนดจากกระทรวงสาธารณสุขลงมา ให้องค์การบริหารส่วนตำบล โรงพยาบาล ส่งเสริมสุขภาพตำบลและคนในชุมชนเป็นผู้จัดทำแผนสุขภาพชุมชนโดยใช้วิธีจัดเวทีประชาคม เมื่อได้แผน สุขภาพชุมชนผ่านการประชาคมจากชุมชนแล้ว โรงพยาบาลส่งเสริมสุขภาพตำบล อาสาสมัครสาธารณสุขตำบล และคนในชุมชนช่วยกันดำเนินงานสุขภาพชุมชน โดยการจัดโครงการส่งเสริมสุขภาพในชุมชน 3) แนวทางและรูปแบบการจัดการสื่อสารสุขภาพแบบมีส่วนร่วมโรคไม่ติดต่อเรื้อรัง การใช้นโยบายสาธารณสุข และแผนสุขภาพ ชุมชนเป็นแนวทางในการดำเนินงาน โดยมีรูปแบบคือการจัดโครงการกิจกรรมส่งเสริมสุขภาพในชุมชน โดยมีปัจจัยประสบความสำเร็จประกอบไปด้วย ธรรมนูญสุขภาพ งบประมาณสนับสนุน เครือข่ายชุมชน แผนระยะยาว 5 ปี (ตำบลจัดการสุขภาพดีวิถีชีวิตไทย) การจัดการความรู้ และการประเมินโครงการกิจกรรมส่งเสริมสุขภาพใน ชุมชนโดยคณะกรรมการภายในและภายนอก
The objectives of this research were to 1) study the situation of health communication in the study areas; 2)study the health communication methods used in a community that was deemed successful in implementing participatory health communication about non-communicable diseases (NCD); and 3) form recommendations on approaches and patterns for managing participatory health communication about NCD. This was a qualitative research of two sample communities. A focus group discussion was held with 11 people from Kaerai Sub-district, Kratumbaen District, Samut Sakorn Province, comprising community leaders, public health personnel, and sub-district health volunteers, and in-depth interviews were held with 8 of them. A focus group discussion was held with 10 people from Saenchat Sub-district, Jungharn District, Roi Et Province, comprising community leaders, public health personnel, and sub-district health volunteers, and in-depth interviews were held with all 10 of them. The data were analyzed by drawing conclusions. The results showed that 1) There were 3 parts to the health communication situation in the study areas: the national public health policy, the community health plan and implementation of the plan. The public health policy was set by the Ministry of Public Health and the community health plan was set by the local public health organization. Because it (Kaerai Sub-district) was an urban community, the people in the community were working and didn’t have time to attend a public hearing to draw up a community health plan. Then they did not share common goals for the implementation of the plan. 2) In a community that was deemed successful in implementing participatory health communication about NCD (Saenchat Sub-district), there were 3 methods: the national public health policy, the community health plan and implementation of the plan. The Ministry of Public Health handed down the policy and instructed the Sub-district Administrative Organization, the sub-district health centers and people in the community to organize a public hearing to draw up a community health plan. After they formed the plan together, the sub-district health center, sub-district public health volunteers and people in the community helped each other implement the plan by organizing health promotion projects. 3) The recommended approach and pattern for managing participatory health communication about NCD diseases is to use the national public health policy and a community health plan as guidelines, and to use the method of organizing health promotion projects in the community. There are 5 factors of success: a “health constitution,” financial support, a community network, a 5-year plan (such as “good health in Thai way of life by sub-district management”), knowledge management, and evaluation of the community health promotion projects by internal and external committees.
