การศึกษาความพร้อมขององค์กรปกครองส่วนท้องถิ่น ในการจัดบริการคลินิกชุมชนอบอุ่นในระบบหลักประกันสุขภาพถ้วนหน้า จังหวัดนนทบุรี

Abstract

การวิจัยครั้งนี้มีวัตถุประสงค์เพื่อ (1) ศึกษาความพร้อมขององค์กรปกครองส่วนท้องถิ่นของจังหวัดนนทบุรีในการจัดบริการคลินิกชุนชนอนอุ่น (2) ศึกษาปัญหาอุปสรรคเกี่ยวกับการเตรียมความพร้อมขององค์กรปกครองส่วนท้องถิ่นในการจัดบริการคลินิกชุนชนอบอุ่นและ (3) เสนอแนะแนวทางในการเตรียมความพร้อมขององค์กรปกครองส่วนท้องถิ่นในการจัดบริการ คลินิกชุมชนอบอุ่น ประชากรที่ใช้ในการศึกษา ประกอบด้วย นายกเทศมนตรีเทศบาลนคร นายกเทศมนตรีเทศบาลเมือง นายกเทศมนตรีเทศบาลตำบล และนายกองค์การบริหารส่วนตำบล จำนวน 31 คน เครื่องมือที่ใช้ในการเก็บข้อมูลเป็นแบนสอบถามและแบบสัมภาษณ์ การวิเคราะห์ข้อมูลใช้การแจกแจงความถี่ ค่าร้อยละ ค่าเฉลี่ย ค่าเบี่ยงเบนมาตรฐาน ค่าทีและการวิเคราะห์ความแปรปรวน ผลการวิจัยพบว่า (1) ระดับความพร้อมขององค์กรปกครองส่วนท้องถิ่นในจังหวัดนนทบุรีในการจัดบริการคลินิกชุมชนอบอุ่นตามเกณฑ์มาตรฐานของสำนักงานหลักประกันสุขภาพแห่งชาติ มีความพร้อมอยู่ในระดับต้องปรับปรุง ทั้งทางด้านบุคลากร ด้านวัสดุอุปกรณ์ อาคารสถานที่ ด้านการจัดการ และด้านการส่งต่อและเชื่อมต่อกับบริการอื่น (2) องค์กรปกครองส่วนท้องถิ่นที่มีขนาดและประเภทที่แตกต่างกัน มีความพร้อมในการจัดบริการคลินิกชุมชนอบอุ่นไม่ แตกต่างกัน อย่างมีนัยสำคัญทางสถิติที่ระดับ 0.05 จากการวิจัยมีข้อเสนอแนะ คือ สำนักงานหลักประกันสุขภาพแห่งชาติ ซึ่งต้องการให้ องค์กรปกครองส่วนท้องถิ่นดำเนินการจัดตั้งคลินิกชุมชนอนอุ่น เพื่อให้เกิดการบริการทางด้าน สาธารณสุขระดับปฐมภูมิ ควรทำการประชาสัมพันธ์และจัดประชุมสัมมนาแลกเปลี่ยนความ คิคเห็นรวมกันระหว่างภาคเอกชน องค์กรปกครองส่วนท้องถิ่นและภาคประชาชน เพื่อให้เกิดความ เข้าใจความร่วมมือ และการทำแผนร่วมกันให้เกิดการพัฒนาการบริการสาธารณสุขระดับปฐมภูมิ อย่างมีประสิทธิภาพ.

The objectives of this research were (1) to study the readiness of local government organizations in Nonthaburi Province on Provision of Primary Care Unit service (2) to study the problems and the obstacles of local government organizations on Primary Care Unit servicing and (3) to recommend the proper guidelines to prepare the readiness of local government organizations on Provision of Primary Care Unit service. The population for this study consisted of City Mayors, Town Mayors, Subdistrict Mayors and the Mayors of Subdistrict Administration Organization in the amount of 31 persons. The tools for gathering data were questionnaires and interviews. Data was analyzed by the distribution of the frequency, percentage, average, standard deviation, t-test and One-way ANOVA. The research found that (1) the level of the readiness of local government organizations in Nonthaburi Province on Primary Care Unit servicing according to the standard criterias of National Health Security Office was at the level improvement for personnel, supplies and buildings, management, and transferring and connecting to other services. (2) the local government organizations with different of size and type had similar readiness of Provision of Primary Care Unit service at the significance of .05. The recommendations from the research were that National Health Security Office demanding the local government organizations to establish Primary Care Unit for basic health services should promote public relation on the matter and hold seminar for exchanging views and ideas among private sector, local government organizations and people sector for better understanding, cooperating, planning to develop the basic health services efficiently.
