ปัจจัยความสำเร็จในการให้บริการวัคซีนป้องกันโรคโควิด-19 ในผู้ป่วยคลินิกโรคเบาหวาน ศูนย์บริการสาธารณสุข 49 วัดชัยพฤกษมาลา กรุงเทพมหานคร

Abstract

การศึกษานี้มีวัตถุประสงค์เพื่อ (1) ปัจจัยที่มีความสัมพันธ์กับการรับบริการวัคซีนป้องกันโรคโควิด-19 ในผู้ป่วยคลินิกโรคเบาหวาน ศูนย์บริการสาธารณสุข 49 วัดชัยพฤกษมาลา กรุงเทพมหานคร (2) ปัจจัยที่มีความสัมพันธ์กับการติดเชื้อโควิด-19 และ (3) ปัจจัยความสำเร็จในการให้บริการวัคซีนป้องกันโรคโควิด-19 ใน ด้านองค์กร ด้านความรู้และ ทัศนคติของผู้ป่วยในคลินิก ด้านสังคม และวัฒนธรรม และการสื่อสารทางการแพทย์การศึกษาแบบผสมผสานนี้ศึกษาเชิงปริมาณจากข้อมูลทุติยภูมิผู้ป่วยในคลินิกโรคเบาหวาน ทั้งหมด 216 คนวิเคราะห์ข้อมูลโดยใช้สถิติเชิงพรรณนา แจกแจงความถี่ ร้อยละ ค่าเฉลี่ย ส่วนเบี่ยงเบนมาตรฐาน การทดสอบไคสแควร์ และการทดสอบของฟิสเชอร์ และศึกษาเชิงคุณภาพจากการสัมภาษณ์เชิงลึกผู้ป่วยในคลินิกโรคเบาหวาน จำนวน 7 คน และทีมสหวิชาชีพ และภาคส่วนที่เกี่ยวข้อง จำนวน 15 คน โดยนำข้อมูลมาวิเคราะห์เชิงเนื้อหาผลการศึกษาพบว่า (1) ปัจจัยด้านเพศ อายุ การอาศัยในพื้นที่เขตรับผิดชอบของศูนย์บริการสาธารณสุข สิทธิการรักษาหลักประกันสุขภาพแห่งชาติ ปัจจัยเสี่ยงต่อการติดเชื้อโควิด-19 รุนแรง ระดับน้ำตาลในเลือด และการได้รับวัคซีนป้องกันโรคโควิด-19 ไม่มีความสัมพันธ์กับการรับบริการวัคซีนป้องกันโรคโควิด-19 (2) ผู้สูงอายุ 60 ปีขึ้นไป มีความสัมพันธ์กับการติดเชื้อโควิด-19 อย่างมีนัยสำคัญทางสถิติ และ (3) ปัจจัยความสำเร็จในการให้บริการวัคซีนป้องกันโรคโควิด-19 ได้แก่ การให้ความรู้เกี่ยวกับโรคและวัคซีน คำแนะนำจากครอบครัว เพื่อน และบุคลากรทางการแพทย์ ภาวะผู้นำขององค์กร ความเพียงพอและชนิดของวัคซีน ระบบการให้บริการที่สะดวก และการสื่อสารทางการแพทย์ที่ถูกต้องและเข้าใจง่าย

This study aimed to explore (1) factors associated with COVID-19 vaccine uptake of patients in the diabetes clinic at Public Health Center 49 Watchaiyaphruksamala, Bangkok, (2) factors related to COVID-19 infection, and (3) success factors of COVID-19 vaccination, focusing on organization, patient knowledge and attitudes, social and cultural aspects, and medical communication.Conducted as a mixed-methods study, the quantitative analysis involved the secondary data from 216 patients at the diabetes clinic. Descriptive statistics, frequency distribution, percentage, mean, standard deviation, Chi-square test, and Fischer's test were applied for data analysis. Furthermore, qualitative insights were gathered through in-depth interviews with 7 diabetes clinic patients and 15 multidisciplinary team members from various relevant sectors, with data subjected to content analysis.The results indicated that: (1) gender, age, residence within the catchment area of the public health service center, being covered by national universal health coverage scheme, blood sugar level and COVID-19 vaccination status were not associated with COVID-19 vaccine uptake; (2) elderly individuals aged 60 and above exhibited a statistically significant relationship with COVID-19 infection; and (3) success factors identified in the provision of COVID-19 vaccine services include imparting disease and vaccine-related knowledge, guidance from family members, friends, and healthcare professionals, effective organizational leadership, vaccine availability, a streamlined service delivery system, and clear and comprehensible medical communication.
