ประสิทธิผลของการใช้รูปแบบการดูแลผู้ป่วยระยะสุดท้ายแบบบูรณาการโรงพยาบาลสมเด็จพระยุพราชเวียงสระ สุราษฎร์ธานี

Abstract

การศึกษานี้เป็นการวิจัยกึ่งทดลอง มีวัตถุประสงค์เพื่อเปรียบเทียบ 1) ความสามารถในการดูแลผู้ป่วยแบบองค์รวมตามการรับรู้ของบุคลากรทีมสุขภาพระหว่างก่อนและหลังการใช้รูปแบบ การดูแลผู้ป่วยระยะสุดท้ายแบบบูรณาการโรงพยาบาลสมเด็จพระยุพราชเวียงสระ จังหวัดสุราษฎร์ธานี 2) ความพึงพอใจในงานของบุคลากรทีมสุขภาพระหว่างก่อนและหลังการใช้รูปแบบการดูแลผู้ป่วยระยะสุดท้ายแบบบูรณาการและ 3) ความพึงพอใจในบริการของญาติผู้ดูแลระหว่างก่อนและหลังการใช้รูปแบบการดูแลผู้ป่วยระยะสุดท้ายแบบบูรณาการ กลุ่มตัวอย่างได้รับการคัดเลือกแบบเฉพาะเจาะจง ประกอบด้วย 2 กลุ่มตัวอย่าง คือ 1) บุคลากรทีมสุขภาพ ที่มีประสบการณ์ในการดูแลผู้ป่วยระยะสุดท้ายอย่างน้อย 1 ปี โรงพยาบาลสมเด็จพระยุพระราชเวียงสระ และโรงพยาบาลส่งเสริมสุขภาพตําบล อําเภอเวียงสระ จังหวัดสุราษฎร์ธานี รวม 20 คน และ 2) ญาติผู้ดูแลผู้ป่วยระยะสุดท้าย จํานวน 20 คน เครื่องมือที่ใช้ในการเก็บรวบรวมข้อมูล ประกอบด้วย แบบประเมินความสามารถในการดูแลผู้ป่วยแบบองค์รวม และแบบสอบถามความพึงพอใจในงาน และแบบสอบถามความพึงพอใจของญาติผู้ดูแลซึ่งมีค่าดัชนีความตรงเชิงเนื้อหา เท่ากับ 0.88, 0.92, และ 0.89 ตามลำดับ และมีค่าสัมประสิทธิ์อัลฟาของครอนบาชเท่ากับ 0.87, 0.95, และ 0.85 ตามลำดับ วิเคราะห์ข้อมูลด้วยสถิติเชิงพรรณนา ได้แก่ ความถี่ ร้อยละ ค่าเฉลี่ย และส่วนเบี่ยงเบนมาตรฐาน สถิติทดสอบทีและสถิติวิลคอกซัน ผลการวิจัยสรุปได้ดังนี้ 1) บุคลากรทีมสุขภาพรับรู้ว่ามีความสามารถในการดูแลผู้ป่วย แบบเป็นองค์รวมหลังทดลองสูงกว่าก่อนทดลองอย่างมีนัยสำคัญทางสถิติที่ระดับ .001 2) บุคลากร ทีมสุขภาพมีความพึงพอใจในงานหลังทดลองสูงกว่าก่อนทดลองอย่างมีนัยสำคัญทางสถิติที่ระดับ .001 และ 3) ญาติผู้ดูแลมีความพึงพอใจในบริการหลังทดลองสูงกว่าก่อนทดลอง อย่างมีนัยสำคัญ ทางสถิติที่ระดับ .001

The purposes of this quasi-experimental research were to compare:-1) holistic care competency between before and after using the integrated palliative care model(IPCM), as perceived by health care personnel at Weingsra Crown Prince Hospital, Surat Thani Province, 2) the job satisfaction of the health care team between before and after using the IPCM, and 3) the servicesatisfaction of caregivers between before and after using the IPCM for the end of life patients. The purposive sample was composed of 2 groups: 1) twenty personnel of health care team who had experienced in caring patients more than one year and 2) twenty caregivers of the end of life patients. Three research tools included (1) the Holistic Practice Evaluation, (2) Job Satisfaction Survey, and 3) Caregiver’sSatisfaction Survey. The content validity indexes (CVI) of those research tools were .88, .92, and .89 respectively. The Cronbach’ s alpha reliability coefficients of those research tools were .87, .95, and .85 respectively. Data were analyzed by descriptive statistics (frequency, percentage, mean, and standard deviation), dependent t-test, and Wilcoxon’s matched pairs signed-ranks test. The major findings were as follows. (1) Health care personnel rated their holistic care competency after using IPCM statistically higher than before using IPCM. (p.<.001). . (2) Health care personnel rated their job satisfaction after using IPCM statistically higher than before using IPCM. (p.<.001). (3) Caregivers of the end of life patients rated their satisfaction after using IPCM statistically higher than before using PCM (p.<.001).
