Abstract
การศึกษาครั้งนี้ มีวัตถุประสงค์เพื่อ 1) ศึกษาสภาพปัจจุบันและความคาดหวังในการดำเนินงานประกันคุณภาพการพยาบาลระดับหน่วยงานและ 2) เปรียบเทียบสภาพปัจจุบันและความคาดหวังของการดำเนินการประกันคุณภาพการพยาบาลระดับหน่วยงาน ของพยาบาลวิชาชีพ สังกัดกลุ่มการพยาบาล โรงพยาบาลหนองม่วงไข่
จังหวัดแพร่ กลุ่มตัวอย่างคือ พยาบาลวิชาชีพ สังกัดกลุ่มการพยาบาล โรงพยาบาลหนองม่วงไข่ จังหวัดแพร่ มีประสบการณ์ทำงานตั้งแต่ 1 ปี ขึ้นไป จำนวน 38 คน เลือกแบบเจาะจงเครื่องมือที่ใช้ คือแบบสอบถามเกี่ยวกับสภาพปัจจุบันและความคาดหวังในการดำเนินงานประกันคุณภาพการพยาบาลระดับหน่วยงาน 10 ขั้นตอนได้แก่
1) การมอบหมายผู้รับผิดชอบ 2)การกำหนดขอบเขตบริการ 3) การกำหนดประเด็นสำคัญและมาตรฐาน/แนวทางปฏิบัติ 4) การกำหนดตัวชี้วัดคุณภาพ 5) การกำหนดเกณฑ์ระดับคุณภาพที่ยอมรับได้ 6) การเก็บรวบรวมและจัดการข้อมูล 7) การวิเคราะห์และประเมินผลคุณภาพการพยาบาล 8) การปรับปรุงและพัฒนาคุณภาพ9)การเฝ้า
ระวังและพัฒนาคุณภาพอย่างต่อเนื่องและ 10) การรายงานผลการประกันคุณภาพ ตรวจสอบความตรงตามเนื้อหา โดยผู้เชี่ยวชาญ จำนวน 5 คน มีค่าดัชนีความสอดคล้องระหว่างข้อคำถามกับจุดประสงค์ อยู่ระหว่าง 0.6-1.0 และตรวจสอบความเชื่อมั่น ด้วยการคำนวณหาค่าสัมประสิทธิ์อัลฟาของครอนบาค มีค่าความเชื่อมั่น 0945
วิเคราะห์ข้อมูลด้วยสถิติเชิงพรรณนาและการวิเคราะห์เปรียบเทียบค่าเฉลี่ยด้วยค่าสถิติ ที แบบจับคู่ผลการศึกษาพบว่า 1) สภาพปัจจุบันในการดำเนินงานประกันคุณภาพการพยาบาล โดยรวมอยู่ในระดับมาก (ค่าเฉลี่ย=3.72, ส่วนเบี่ยงเบนมาตรฐาน=0.56) ความคาดหวังที่มีต่อการดำเนินงานประกันคุณภาพการพยาบาลโดยรวมอยู่ในระดับมาก (ค่าเฉลี่ย-4.32,ส่วนเบี่ยงเบนมาตรฐาน=0.49) และ 2)ผลการเปรียบเทียบ
ความต่างของค่าเฉลี่ย ของการดำเนินงานประกันคุณภาพการพยาบาล ระหว่างสภาพปัจจุบันและความคาดหวังพบว่า ทุกขั้นตอนของการดำเนินงานประกันคุณภาพการพยาบาล คะแนนเฉลี่ยความคาดหวังมีค่ามากกว่า สภาพปัจจุบันอย่างมีนัยสำคัญทางสถิติที่ระดับ 0.01
The purposes of this study were 1) to study the current situation of nursing quality assurance and expectations and 2) to compare current nursing quality assurance and the expectation situation at Nongmuangkhai Hospital, Phrae Province. The participants were 38 nurses who had at least one year experience and worked at Nongmuangkhai Hospital, Phrae Province. They were selected by using purposive sampling method. The study instrument was a questionnaire related to the current and expected situations of nursing quality assurance. This included 10 steps: 1) assign responsible persons, 2) scope of care, 3) specify important issues and standard/ guide for practice 4) specify indicators, 5) establish acceptance quality level, 6) data collection and management, 7) analysis and evaluation of nursing care, 8) Take action to solve Problems, 9) Assess the Action and Document Improvement, and 10) Report all Information to organization. Its contents validity had been approved by 5 experts and the Index of Item-Objective Congruence were 0.6 – 1.0 as well as the Cornbrash’s Alpha Coefficient were 0.945. The data were used interferential statistical analysis and Paired-Samples T Test. The 2 result showed that 1) in overall, the quality assurance activities were in a high level (Mean = 3.72, SD=0.56). Nursing quality assurance expectations was in a high level (Mean = 4.32, SD=0.49). Moreover, 2) nursing quality assurance expectations was significantly higher than the concurrent situation (p<0.01)
