Abstract
การวิจัยครั้งนี้ เป็นการวิจัยพรรณนา มีวัตถุประสงค์เพื่อ (1) วิเคราะห์สภาพการณ์จัดการดูแล ผู้ป่วยโรคกล้ามเนื้อหัวใจตายเฉียบพลัน โรงพยาบาลชะอำ (2) สร้างรูปแบบการจัดการดูแลผู้ป่วยโรคกล้ามเนื้อหัวใจตายเฉียบพลัน และ (3) ประเมินความเหมาะสมของรูปแบบการจัดการดูแลผู้ป่วยโรคกล้ามเนื้อหัวใจตายเฉียบพลันที่สร้างขึ้น
ผู้ให้ข้อมูลหลัก ได้รับการคัดเลือกแบบเจาะจง มี 2 กลุ่ม ได้แก่ 1) ผู้ให้บริการ จำนวน 9 คน สำหรับวิเคราะห์สภาพการณ์จัดการดูแลผู้ป่วยกล้ามเนื้อหัวใจตายเฉียบพลัน และ 2) ผู้ประเมินความเหมาะสมของรูปแบบ จำนวน 13 คน ประกอบด้วย ตัวแทนผู้ให้ข้อมูลในกลุ่มแรก ผู้บริหารและกลุ่มผู้ปฏิบัติ และ ผู้ทรงคุณวุฒิภายนอก เครื่องมือวิจัย มี 2 ชุด ชุดแรก มี 2 ส่วน คือ ประเด็นสัมภาษณ์เจาะลึก ผู้วิจัยพัฒนาขึ้น ตามแนวคิดของโดนาบีเดียล และแนวคิดการจัดการรายกรณีของพาวเวลล์ ชุดที่สอง คือแบบประเมินความเหมาะสมของรูปแบบการจัดการดูแลผู้ป่วย โรคกล้ามเนื้อหัวใจตายเฉียบพลัน ผ่านตรวจสอบความตรงตามเนื้อหาทั้งสองชุดจากผู้ทรงคุณวุฒิ 5 ท่าน ได้ค่าดัชนีความตรงเนื้อหาเฉลี่ยเท่ากับ 0.98 และ 0.96 ตามลำดับ การวิเคราะห์ข้อมูลประกอบด้วยการแจกแจงความถี่ ค่าเฉลี่ย ร้อยละ และการวิเคราะห์เนื้อหา ผลการวิจัยพบว่า (1) สภาพการณ์การจัดการดูแลผู้ป่วยโรคกล้ามเนื้อหัวใจตายเฉียบพลัน ยังมีปัญหาทั้งด้านโครงสร้าง ด้านกระบวนการและด้านผลลัพธ์ จำเป็นต้องได้รับการพัฒนา และ (2) รูปแบบ มี 3 องค์ประกอบ ได้แก่ ด้านโครงสร้าง ประกอบด้วย ทรัพยากรบุคคล เครื่องมือ/วัสดุอุปกรณ์ และมาตรฐานการปฏิบัติงาน ด้านกระบวนการ ประกอบด้วย กระบวนการดูแลผู้ป่วยอย่างต่อเนื่อง 6 ขั้นตอน โดยมีผู้จัดการรายกรณีเป็น ผู้ประสานการจัดการดูแลและติดตามประเมินผล ด้านผลลัพธ์ประกอบด้วย ความพึงพอใจของผู้ ให้บริการ ผู้รับบริการและคุณภาพการบริการของโรงพยาบาลและหน่วยงาน และ 3) รูปแบบการจัดการดูแลผู้ป่วยโรคกล้ามเนื้อหัวใจตายเฉียบพลันที่สร้างขึ้น มีความเหมาะสมกับการนำไปใช้กับโรงพยาบาลชะอำ ร้อยละ 91.14
The objectives of this descriptive research were: (1) to analyze the situation of nursing care management for patients with Acute Myocardial Infarction (AMI) at Cha Am Hospital in Phetchaburi Province, (2) to develop the care management model for patients with AMI, and (3) to evaluate the appropriateness of the care management model for patients with AMI. The key informants were selected by purposive sampling technique and included 2 groups. The first group consisted of 9 health care providers and they analyzed the situation of nursing care management for patients with AMI. The latter consisted of 13 persons, comprising 4 representatives of the first group, 5 administrators and practitioners, and one external expert. This group evaluated the appropriateness of the developed model. Two research tools were developed as follows. (1) Semi-structure interview was developed based on the concept of Donabedian’s model and Powell’s case management model. The content validity index of the tools was verified by 5 experts, and each was 0.98 and 0.96 respectively. The data was analyzed by mean, percentage, and content analysis. The research findings were as follows. (1) The results of the situation analysis of care management for patients with AMI revealed some problems in terms of structure, process, and outcome, and all aspects needed to be improved. (2) The developed model comprised 3 components: 1) structure: human resources, instruments and materials, and standards of practice, (2) process: 6 steps of continuing care which needed the case manager for coordinating, monitoring, and evaluating, (3) outcome: satisfactions of health care providers and the patients, as well as quality of hospital and unit services. 3) The model was appropriate to implement on the Cha Am hospital (91.14%).
