Abstract
การวิจัยนี้มีวัตถุประสงค์เพื่อศึกษาผลลัพธ์การบริการผู้สูงอายุโรงพยาบาลส่งเสริมสุขภาพตำบล อำภออุทุมพรพิสัย จังหวัดศรีสะเกษ และเปรียบเทียบผลลัพธ์การบริการผู้สูงอายุโรงพยาบาลส่งเสริมสุขภาพตำบล ที่มีแพทย์ตรวจกับโรงพยาบาลส่งเสริมสุขภาพตำบลที่ไม่มีแพทย์ตรวจ ประชากรที่ศึกษาคือผู้สูงอายุที่ป่วย ด้วยโรคเรื้อรังตามนโยบายกระทรวงสาธารณสุข ประกอบด้วย โรคเบาหวาน โรคความดันโลหิตสูง โรคหลอดเลือดหัวใจ และโรคหลอดเลือดสมอง อาศัยอยู่ในอำเภออุทุมพรพิสัย และรับบริการที่โรงพยาบาลส่งเสริมสุขภาพตำบลในช่วง 1 ปี ที่ผ่านมาจำนวน 1,818 คน
กลุ่มตัวอย่างได้จาก การสุ่มแบบหลายขั้นตอนจำนวน 180 คน เครื่องมือที่ใช้คือ แบบสอบถามซึ่งมีค่าความเที่ยงเท่ากับ 0.88 วิเคราะห์ข้อมูลโดยการแจกแจงความถี่ ร้อยละ ค่าเฉลี่ย ส่วนเบี่ยงเบนมาตรฐาน และ Mann-Whitney U test ผลการวิจัยพบวา ผลลัพธ์การบริการผู้สูงอายุโรงพยาบาลส่งเสริมสุขภาพตำบล อำเภออุทุมพรพิสัย จังหวัดศรีสะเกษ อยู่ในระดับสูง (X̅ =4.12, SD =0.40) และการเปรียบเทียบผลลัพธ์การบริการผู้สูงอายุ โรงพยาบาลส่งเสริมสุขภาพตำบล ที่มีแพทย์ตรวจกับโรงพยาบาลส่งเสริมสุขภาพตำบล ที่ไม่มีแพทย์ตรวจโดยรวมไม่แตกต่าง (p > .05) แต่เมื่อพิจารณาเป็นรายด้านพบว่าด้านสรีรวิทยาและด้านสุขภาพครอบครัว โรงพยาบาลส่งเสริมสุขภาพตำบลที่มีแพทย์ตรวจกับโรงพยาบาลส่งเสริมสุขภาพตำบลที่ไม่มีแพทย์ตรวจแตกต่างกันอย่างมีนัยสำคัญทางสถิติ (p < .05) และพิจารณาเป็นรายข้อมี 11 ข้อที่แตกต่างอย่างมีนัยสำคัญทางสถิติ (p < .05) ได้แก่ ข้อ 2. ท่านรู้สึกตนเองไม่มีเรี่ยวแรงหรืออ่อนเพลีย ข้อ 14. ท่านถ่ายอุจาระน้อยกวา 3 ครั้งต่อสัปดาห์อุจจาระแข็ง หรือมีอาการเจ็บทวารหนักเวลาถ่าย ข้อ 15. ท่านมีอาการหลงลืม เช่น ลืมเพื่อนสนิทหรือญาติสนิทที่พบกันประจำ ข้อ 16. ท่านได้ยินเสียงหวีดก้องในหูโดยหาที่มาของเสียงไม่่ได้มักมีเสียงดังอยู่ตลอดเวลา และมีเสียงดังมากขึ้นเมื่ออยู่ที่เงียบ ข้อ 25. ท่านมีปัญหาในการมีเพศสัมพันธ์ ข้อ 27. ท่านมีความรู้เกี่ยวกับโรคที่กำลังเจ็บป่วย ข้อ 28. ท่านกินอาหารครบ 5 หมู่และควบคุมสัดส่วนปริมาณให้พอเหมาะ ข้อ 37. ท่านใช้ยาตามที่แพทย์แนะนำ และไปตรวจตามนัด ข้อ 45. สมาชิกในครอบครัวของท่านมีปัญหาสุขภาพ ข้อ 47.ท่านรู้ว่าครอบครัวมีความเครียดในการดูแลท่าน ข้อ 49. ผู้ดูแลสามารถปรับตัวดูแลท่านได้ทุกสภาวะการเจ็บป่วย
The objectives of this research were: (1) to study the outcomes of the elderly services in sub-district health promoting hospitals, UthumphonPhisai District, Srisaket Province and (2) to compare outcomes of the elderly services in sub-district health promoting hospitals between two types of hospital: with and without doctors in medicine. The population comprised 1,818 elderly who had chronic diseases: diabetes, hypertension, cardio-vascular, and cerebro-vascular diseases, based on the policy of the Ministry of Public Health. They lived in UthumphonPhisai District and received services at sub-district health promoting hospitals during one year. The sample included 180 and was selected by the multi-stage stratefied sampling technique. Questionnaires were used as a research tool, and the Cronbach’s alpha reliability coefficient was 0.88. Data were analyzed by frequency, percentage, mean, standard deviation, and Mann-Whitney U test. The results were as follows. (1) The elderly rated the outcomes’ quality of the elderly services in sub-district health promoting hospitals, UthumphonPhisai District, Srisaket Province at the high level (X̅ =4.12, SD =0.40). (2) Totallty, there was no significant difference between the outcomes of the elderly services in subdistrict health promoting hospitals between two types of hospital: with and without doctors in medicine (p < .05). However, there were significant differences between the outcomes of the elderly services in the following 11 items. Item 2: You felt weak or exhausted. Item 14: You had bowel movement less than 3 times a week. Item 15: You often forgot your closed friends or relatives. Item 16: You always hear sound which was not knew the source and this sound was louder when you stay in a silent place. Item 25: You had problems when having sexual intercross. Item 27: You had knewledge of your disease. Item 28: You ate healthy food and control the amount of food. Item 37: You took your medicine according to your doctor ordered and went to hospital for follow up. Item 45: Your family members had health problems. Item 47: You knew that your family felt stress for taking care of you. Item 49: Your care givers could adapt themselves to take care of you for all of your ailment conditions.
