แนวปฏิบัติการดูแลผู้ป่วยไทยมุสลิมระยะสุดท้ายตามการรับรู้ของแพทย์และพยาบาล จังหวัดสงขลา

Abstract

การศึกษานี้เป็นการวิจัยเชิงพรรณนา เพื่อประเมินแนวปฏิบัติการดูแลผู้ป่วยไทยมุสลิมระยะสุดท้ายให้สอดคล้องกับวิถีความเชื่อในประเพณีและวัฒนธรรมของผู้ป่วยประชากรและผู้ให้ข้อมูลหลัก ประกอบด้วยแพทย์ 12 คน และพยาบาลวิชาชีพ 28 คน เครื่องมือศึกษาพัฒนามาจากกรอบการประเมินการดูแลผู้ป่วยของสำนักงานหลักประสุขภาพแห่งชาติ 2553 เป็นแบบสอบถาม ประกอบด้วย 2 ส่วน 1) ข้อมูลส่วนบุคคล และ 2) แบบประเมินความเป็นไปได้ของแนวปฏิบัติในการดูแลผู้ป่วยไทยมุสลิมระยะสุดท้าย 6 ด้าน รวม 49 ข้อ ได้แก่ (1) ด้านการแสดงเจตนาเกี่ยวกับเป้าหมายการดูแลรักษา 6 ข้อ (2) ด้านความเชื่อเกี่ยวกับการเจ็บป่วย 17 ข้อ (3) ด้านความสามารถในการทำกิจวัตรประจำวัน 3 ข้อ(4) ด้านความรู้สึกและการปรับตัวของผู้ป่วยและครอบครัว 10 ข้อ (5) ด้านความไม่สุขสบายทางกายและอาการต่างๆ 9 ข้อ และ (6) ด้านผลกระทบด้านสังคมและเศรษฐกิจ 4 ข้อ ตรวจสอบความตรงของเนื้อหา โดยผู้เชี่ยวชาญ5 คน ได้ค่าดัชนีความตรงของเนื้อหาเท่ากับ 0.94 และค่าสัมประสิทธิ์อัลฟาของ ครอนบาค เท่ากับ 0.97 วิเคราะห์ข้อมูลส่วนบุคคลและแบบสอบถามโดยใช้การวิเคราะห์ข้อมูลด้วยการแจงแจงความถี่ ร้อยละ ค่าเฉลี่ย และส่วนเบี่ยงเบนมาตรฐาน

This descriptive study aimed to develop the palliative clinical guidelines related to Thai Muslim client beliefs, traditions, and cultures. Participants included 12 doctors and 28 nurses, and they were selected by the purposive sampling technique. The research instrument was questionnaire which was applied from the Bureau of National Health, 2010 Framework. Questionnaires included two parts: 1) personal information and 2) the assessment form for the possibility of palliative care practice. This form comprised six sections: (1) expression of goals of treatment and care, (2) illness beliefs, (3) ability of doing daily life activities, (4) emotion and adaptation of clients and their family, ( 5) physical discomfort and symptoms, and (6) social and economic affects. Contents validity index was verified by 5 experts and it was 0.94. Cronbach’s alpha coefficient was 0.97. Data were analyzed by frequency, percent, mean, and standard deviation. The study results showed as follows. Doctors and nurses rated the possibility of palliative care practice at the highest level (Mean = 4.30, SD = 0.76). They rated 5 sections at the highest level: (1) physical discomfort and symptoms (Mean = 4.42, SD = 0.73), (2) emotion and adaptation of clients and their family (Mean = 4.36, SD = 0.72), (3) ability of doing daily life activities (Mean = 4.34, SD = 0.76), (4) illness beliefs (Mean = 4.27, SD = 0.82), and (5) social and economic affects (Mean = 4.21, SD = 0.74). They rated expression of goals of treatment and care at the high level (Mean = 4.15, SD = 0.74)
