ความสัมพันธ์ทางสังคมระหว่างผู้สูงอายุกับบุคคลที่เกี่ยวข้องหลักกับพฤติกรรมการดูแลสุขภาพตนเองต่อโรคความดันโลหิตสูง จังหวัดเพชรบุรี

Abstract

การศึกษาในครั้งนี้เป็นการศึกษาวิจัยแบบเชิงพรรณนา โดยมีวัตถุประสงค์เพื่อศึกษา (1) ปัจจัยภายในบุคคลด้านชีวภาพและด้านจิตสังคมของผู้สูงอายุที่ป่วยด้วยโรคความดันโลหิตสูง (2) ปัจจัยความสัมพันธ์ทางสังคมครอบคลุมมิติการได้รับการสนับสนุนทางสังคมกับการบูรณาการทางสังคมระหว่างผู้สูงอายุดังกล่าวกับบุคคลที่เกี่ยวข้องหลัก (3) พฤติกรรมของผู้สูงอายุดังกล่าวในการดูแลสุขภาพตนเองต่อโรคความดันโลหิตสูง และ (4) ความสัมพันธ์ระหว่างปัจจัยภายในบุคคลและปัจจัยความสัมพันธ์ทางสังคมกับพฤติกรรมการดูแลสุขภาพตนเองต่อโรคความดันโลหิตสูงของผู้สูงอายุที่ป่วยด้วยโรคความดันโลหิตสูง กลุ่มตัวอย่างที่ใช้ในการศึกษาได้รับการสุ่มจากประชากรผู้สูงอายุที่ป่วยด้วยโรคความดันโลหิตสูงในจังหวัดเพชรบุรี โดยวิธีแบ่งชั้นภูมิ ได้จำนวน 377 ราย เก็บข้อมูลโดยการใช้การสัมภาษณ์ตามแบบฟอร์มวิเคราะห์ข้อมูลโดยสถิติ จำนวน ร้อยละ ค่าเฉลี่ย และส่วนเบียงเบนมาตรฐาน การทดสอบสมมติฐาน ใช้สถิติไคสแควร์ และสัมประสิทธิ์สหสัมพันธ์แบบสเปียร์แมน ผลการศึกษาพบว่า กลุ่มตัวอย่างมีค่าเฉลี่ยรวมด้านพฤติกรรมการดูแลสุขภาพตนเองในระดับปานกลาง และมีค่าเฉลี่ยรวมทุกมิติความสัมพันธ์ทางสังคมกับบุคคลที่เกี่ยวข้องหลักในระดับมาก โดยมีค่าเฉลี่ยตามลำดับ ได้แก่ บุคคลในครอบครัวเจ้าหน้าที่สาธารณสุข และอาสาสมัครสาธารณสุขประจำหมู่บ้าน และพบว่าปัจจัยภายในบุคคลด้านชีวภาพของผู้สูงอายุที่มีความสัมพันธ์อย่างมีนัยสำคัญกับพฤติกรรมการดูแลสุขภาพตนเอง ได้แก่ ระยะเวลาที่ป่วย และระดับความดันโลหิต แต่ไม่พบว่าปัจจัยภายในบุคคลด้านจิตสังคมปัจจัยใดที่มีความสัมพันธ์กับพฤติกรรมการดูแลสุขภาพตนเอง ปัจจัยความสัมพันธ์ทางสังคมกับบุคคลที่เกี่ยวข้องหลักทุกกลุ่มมีความสัมพันธ์ทางสถิติอย่างมีนัยสำคัญกับพฤติกรรมการดูแลสุขภาพตนเอง โดยความสัมพันธ์ทางสังคมกับบุคคลในครอบครัวมีค่าความสัมพันธ์ทางสถิติอย่างมีนัยสำคัญมากที่สุด ตามด้วยอาสาสมัครสาธารณสุขประจำหมู่บ้าน และเจ้าหน้าที่สาธารณสุข

This descriptive research aimed to study: (1) personal profiles of elderly patients with hypertension consisting of biological, psychological and social aspects; (2) social relations regarding social support and social integration between elderly patients and main concerned parties; (3) self-care behaviors of elderly persons related to hypertension; and (4) the relationships between personal profiles as well as social relations and elderly persons’ hypertension self-care behaviors. The study was conducted in a sample of 377 patients, selected from elderly persons with hypertension in Phetchaburi province, using the stratified random sampling technique. Data were collected using the interview technique and form, and then analyzed to determine numbers, percentages, and standard deviations, and perform hypothesis testing using chi-square test and Spearman correlation. The results indicated that, among the elderly participants, the overall mean score for self-care behaviors was at a moderate level, and for social relations with concerned parties it was at a high level. Among concerned parties, the patients had a high level of social relations with family members, followed by health personnel and village health volunteers. The elders’ biological aspects significantly associated with self-care behaviors were illness duration and blood pressure level, while psychological and social aspects were not. The elders’ social relations with all concerned parties were significantly associated with self-care behaviors – being highest with family members (r = 0.481), followed by with village health volunteers and health personnel.
