คู่มือโภชนาการสำหรับผู้ป่วยที่ได้รับการฟอกไตในศูนย์ไตเทียม โรงพยาบาลค่ายจิรประวัติ

Abstract

การดูแลผู้ป่วยโรคไตเรื้อรังด้วยการฟอกไต ต้องดูแลให้ไตเสื่อมลงในอัตราที่ช้าที่สุด เพื่อไม่ให้การดำเนินของโรคไปสู่ภาวะไตวายเรื้อรังระยะสุดท้าย ภาวะแทรกซ้อนสำคัญประการหนึ่ง จากการฟอกไตด้วยเครื่องไตเทียมคือการสูญเสียสารอาหารหลายชนิด ทำให้ผู้ป่วยเกิดภาวะขาดสารอาหาร การศึกษาค้นคว้าอิสระนี้ จึงมีวัตถุประสงค์เพื่อจัดทำคู่มือโภชนาการสำหรับผู้ป่วยที่ได้รับการฟอกไตในศูนย์ไตเทียมโรงพยาบาลค่ายจิรประวัติ วิธีการจัดทำคู่มือ ได้จากการทบทวนเอกสารและงานวิจัยที่เกี่ยวข้อง เพื่อจัดทำร่างคู่มือโภชนาการสำหรับผู้ป่วยที่ได้รับการฟอกไตในศูนย์ไตเทียม มีการตรวจสอบความตรงของเนื้อหาของร่างคู่มือโดยผู้เชี่ยวชาญ 3 คน ภายหลังการปรับแก้ไข ได้นำคู่มือไปทดลองใช้กับผู้ป่วยที่ได้รับการฟอกไตในศูนย์ไตเทียมโรงพยาบาลค่ายจิรประวัติจำนวน30 คน แล้วให้ประเมินความพึงพอใจต่อการใช้คู่มือโภชนาการดังกล่าว ผลการศึกษา พบว่า คู่มือโภชนาการสำหรับผู้ป่วยที่ได้รับการฟอกไตในศูนย์ไตเทียมโรงพยาบาลค่ายจิรประวัติที่จัดทำขึ้น แบ่งเป็น 2 ส่วน คือส่วนบทนำ และส่วนเนื้อหา ส่วนบทนำ ประกอบด้วย ความเป็นมา ความสำคัญและวัตถุประสงค์ในการจัดทำคู่มือ ส่วนเนื้อหามี 9 ตอน ได้แก่ 1) ความรู้เรื่องโรคไต 2) ประเภทของการบำบัดทดแทนไต 3) การฟอกไตด้วยเครื่องไตเทียม 4) ขั้นตอน ปฏิบัติในการเข้ารับการฟอกไตด้วยเครื่องไตเทียมในศูนย์ไตเทียมโรงพยาบาลค่ายจิรประวัติ 5) ภาวะโภชนาการของผู้ที่ได้รับการฟอกไต 6) คำแนะนำด้านโภชนาการสำหรับผู้ป่วยที่ได้รับการฟอกไต 7) การคำนวณสารอาหารเพื่อมาประยุกต์ใช้ในการรับประทาน 8) เมนูอาหารแนะนำและอาหารที่ควรหลีกเลี่ยง และ 9) วิธีทำและสูตรสำหรับการทำอาหารปั่นผสมหรืออาหารทางสาย สำหรับผลการประเมินความพึงพอใจในการทดลองใช้คู่มือ พบว่า ผู้ป่วยที่ใช้คู่มือ 30 ราย มีความพึงพอใจในระดับมากที่สุดในทุกด้านโดยเฉพาะอย่างยิ่งในด้านความน่าเชื่อถือของเนื้อหา

The treatment of chronic kidney disease (CKD) patients involves the nanagement to slow the disease progression to the end-stage. One of the complications from hemodialysis treatment is essential nutrients losses that may make patients have malnutrition. The objective of this study was to develop a nutrition handbook for the patients who were on dialysis treatment at the Hemodialysis Center of Fort Chiraprawat Hospital. The process for developing this handbook started from researching and reviewing relevant documents for preparing a draft nutrition handbook, followed by verifying the content validity by 3 specialists. Upon revision, the draft handbook was tried out with 30 hemodialysis patients at the hospital’s hemodialysis center; and their satisfaction with the handbook was assessed. As a result of this effort, the prepared nutrition handbook contained two parts: introduction and content. The introduction part was composed of background and objectives. Moreover, the content part contained nine chapters meluding 1) Knowledge about kidney disease; 2) Type of renal replacement therapy; 3) Hemodialysis, 4) Steps for receiving hemodialysis at Fort Chiraprawat Hospital, 5) Nutritional status of hemodialysis patients; 6) Dietary guideline for hemodialysis patients; 7) How to calculate nutritional needs and apply them to the meal plan; 8) Recommended foods to eat and avoid; and 9) Blenderized diet recipes. The satisfaction evaluation of 30 patients showed their strong satisfaction with all aspects, especially the reliability of the content in the handbook
