Abstract
การวิจัยครั้งนี้เป็นการวิจัยแบบผสมผสาน มีวัตถุประสงค์เพื่อศึกษา (1) การพัฒนาโรงพยาบาลส่งเสริมสุขภาพตำบลติดดาว (2) ปัจจัยที่เกี่ยวข้องกับความสำเร็จในการพัฒนาโรงพยาบาลส่งเสริมสุขภาพตำบลติดดาว และ (3) ปัญหา อุปสรรค การแก้ไขปัญหา และแนวทางการพัฒนาโรงพยาบาลส่งเสริมสุขภาพตำบลติดดาว ในจังหวัดนครศรีธรรมราช ประชากร คือ โรงพยาบาลส่งเสริมสุขภาพตำบลในจังหวัดนครศรีธรรมราช จำนวน 253 แห่ง กลุ่มตัวอย่างคือ โรงพยาบาลส่งเสริมสุขภาพตำบลที่ผ่านเกณฑ์คุณภาพติดดาวสูงที่สุดของแต่ละโซนอำเภอ จำนวน 6 แห่ง เครื่องมือคือ แบบบันทึกผลการพัฒนาโรงพยาบาลส่งเสริมสุขภาพตำบลติดดาวและแบบสัมภาษณ์เพื่อค้นหาปัจจัยความสำเร็จ การวิเคราะห์ข้อมูลใช้สถิติเชิงพรรณนา ร้อยละ ค่าเฉลี่ย และการวิเคราะห์เนื้อหา ผลการวิจัยพบว่า (1) การพัฒนาโรงพยาบาลส่งเสริมสุขภาพตำบลติดดาวในจังหวัดนครศรีธรรมราช ปี 2561 โรงพยาบาลส่งเสริมสุขภาพตำบลมีผลการประเมินตนเองผ่านเกณฑ์คุณภาพติดดาว จำแนกรายหมวดผ่านเกณฑ์ร้อยละ 80 จำนวน 64 แห่ง (2) ปัจจัยที่เกี่ยวข้องกับความสำเร็จในการพัฒนาโรงพยาบาลส่งเสริมสุขภาพตำบลติดดาวในจังหวัดนครศรีธรรมราชประกอบด้วย 2 กลุ่ม 11 ปัจจัย คือ ปัจจัยหลัก 4 ปัจจัยได้แก่ ปัจจัยด้านภาวะผู้นำ ปัจจัยด้านบุคลากร ปัจจัยด้านทรัพยากรและปัจจัยการมีส่วนร่วมของภาคีเครื่อข่าย และปัจจัยรอง 7 ปัจจัย ได้แก่ ปัจจัยด้านนโยบายผู้บริหาร ปัจจัยด้านวัฒนธรรมองค์กร ปัจจัยด้านการสื่อสาร การสนับสนุนจากองค์กร ปัจจัยด้านแรงจูงใจ ปัจจัยความพร้อมโครงสร้างพื้นฐาน และปัจจัยกลยุทธ์ 3) ปัญหา อุปสรรค การพัฒนาโรงพยาบาลส่งเสริมสุขภาพตำบลติดดาวในจังหวัดนครศรีธรรมราช ได้แก่ การขาดงบประมาณสนับสนุน ความครอบคลุมด้านระบบสนับสนุนทั้งด้านทรัพยากรบุคคล เงิน สิ่งของที่ใช้ในการจัดบริการ และระบบการจัดทำฐานข้อมูล แนวทางการพัฒนาควรมีการวางแผนบริหารจัดการให้เป็นระบบและมีความครอบคลุมทั้งทรัพยากรบุคคล เงินและสิ่งของการสร้างความเข้มแข็งของทีมและมีเวทีแลกเปลี่ยนเรียนรู้เพื่อให้เกิดการพัฒนา
This mixed methods research aimed : (1) to review the status of starred Sub-District or Tambon Health Promoting Hospitals (THPHs) development program; (2) identify critical success factors for developing starred THPHs; and (3) identify problems in program implementation and make recommendations for further program improvements in Nakhon Si Thammarat Province. The study was conducted in a sample of 6 THPHs selected from all 253 THPHs as the 6 hospitals had met all the criteria of being highest quality 5-star hospitals for 6 district zones of the province. Data were collected using a starred hospital development record form; and then descriptive (percentages and means) and content analyses were performed. The results showed that: (1) in the province, 64 THPHs had a passing score of 80% for all aspects of starred THPHs development criteria in 2018. (2) the critical success factors for being starred hospitals consisted of 2 groups of 11 factors: 4 main factors (leadership, personnel, resources and network participation) and 7 secondary factors (executive policies, organizational culture, communications, corporate support, motivation, infrastructure readiness, and strategy). (3) the identified problems and obstacles in program development included the lack or inadequacy of budget, human resources and materials for service delivery and database establishment. It is thus recommended that, for further program development, a management plan should be drawn up systematically; and human, financial and material resources should be adequately allocated for teamwork strengthening. And a knowledge sharing forum should also set up to enhance development efforts
