Abstract
การวิจัยนี้มีวัตถุประสงค์เพื่อศึกษา 1) การประกอบสร้างความหมายความตาย 2) กระบวนการสื่อสารความตาย และ 3) แนวทางการสื่อสารความตาย ระหว่างบุคลากรทางสุขภาพ ผู้ป่วยมะเร็งที่ได้รับการดูแลแบบประคับประคองและผู้ดูแลหลัก การวิจัยนี้เป็นการวิจัยเชิงคุณภาพด้วยการสัมภาษณ์เชิงลึก สนทนากลุ่ม และสังเกตการณ์แบบไม่มีส่วนร่วมในโรงพยาบาลกรณีศึกษาและที่บ้านผู้ป่วย ใช้วิธีเลือกผู้ให้ข้อมูลหลักแบบเจาะจงและแบบสโนบอลจากทีมสหวิชาชีพ ได้แก่ แพทย์ พยาบาล แพทย์รังสีรักษา นักสังคมสงเคราะห์ และนักจิตวิทยา รวมจำนวน 8 คน และผู้ป่วยมะเร็งที่ได้รับการดูแลแบบประคับประคอง 4 คนและผู้ดูแลหลัก 4 คน วิเคราะห์ข้อมูลโดยการสร้างข้อสรุป ผลการศึกษาพบว่า 1) การประกอบสร้างความหมายความตายของบุคลากรทางสุขภาพ ผู้ป่วยมะเร็งและผู้ดูแลหลักล้วนแตกต่างกันตามมุมมองและปัจจัยที่เกี่ยวข้อง (1) แพทย์เน้นมุมมองการแพทย์อย่างเดียว ความหมายความตายจึงเป็นเพียงภาวะหัวใจหยุดเต้น แต่แพทย์รังสีรักษาและแพทย์ผู้เชี่ยวชาญเน้นมุมมองสังคม ศาสนา การดูแลแบบประคับประคอง และประสบการณ์การทำงานที่คำนึงความต้องการผู้ป่วย ความหมายความตายจึงมากกว่าเพียงหัวใจหยุดเต้น (2) ขณะที่สหวิชาชีพอื่น ผู้ป่วยมะเร็งและผู้ดูแลหลักเน้นมุมมองศาสนา ความหมายความตายจึงมองว่าเป็นธรรมชาติ และเชื่อเรื่องภพภูมิหลังความตาย (3) แต่ผู้ป่วยมะเร็งยังมีปัจจัยเพื่อนผู้ป่วยข้างห้องที่บางคนอายุน้อยกว่า บางคนเป็นเพศหญิง จึงมีกำลังใจในการรักษาและค่อย ๆ ยอมรับความตายมากขึ้น (4) ส่วนผู้ดูแลหลักมีปัจจัยเรื่องระยะเวลา การดูแล อารมณ์ ความรู้สึก ความผูกพัน ความใกล้ชิด ความสัมพันธ์ในครอบครัว 2) กระบวนการสื่อสารความตายในแต่ละช่วง คือ (1) การแรกรับผู้ป่วยมะเร็งระยะสุดท้าย แพทย์เน้นให้ข้อมูลการรักษาผู้ป่วยตามอาการโดยไม่เน้นเครื่องมืออุปกรณ์การช่วยชีวิต โดยแพทย์ยอมรับว่ามีความลำบากในการสื่อสารและสร้างความสัมพันธ์กับผู้ป่วยมะเร็งระยะสุดท้ายเมื่อเทียบกับผู้ป่วยมะเร็งระยะแรก (2) การแจ้งอาการผู้ป่วยและการประชุมทีมสหวิชาชีพ แพทย์เน้นให้ผู้ป่วยร่วมวางแผนรักษาโรคด้วยตนเอง แพทย์เป็นความหวังเดียวของผู้ป่วยและผู้ดูแลหลักจึงเชื่อฟังแพทย์และยินยอมการรักษาเพื่อสร้างกำลังใจให้กับตนเองซึ่งถือเป็นการต่อรองลักษณะหนึ่งของผู้ป่วยที่เข้ามารักษาในพื้นที่กรณีศึกษา ขณะที่แพทย์เองก็ต่อรองกับผู้ป่วยและทีมสหวิชาชีพอื่นตามอำนาจความรู้วิชาชีพแพทย์ (3) การรักษาไประยะหนึ่งใกล้กลับบ้าน สหวิชาชีพอื่นมีความสำคัญให้การดูแลผู้ป่วย เข้าใจอารมณ์ความรู้สึกเห็นอกเห็นใจผู้ป่วยตามมุมมองศาสนา ให้ความสำคัญกับจิตวิญญาณตามความสนใจและความต้องการของผู้ป่วยและผู้ดูแลหลัก และ (4) การกลับไปรักษาตัวที่บ้านเท่ากับผู้ป่วยได้กลับสู่ความเป็นจริงในชีวิตประจำวันที่บ้านทำให้มีการต่อรองการสื่อสารความตายกับพื้นฐานความเชื่อเดิมอย่างเช่นเรื่องร่างทรง และ 3) แนวทางการสื่อสารความตาย สหวิชาชีพโดยเฉพาะแพทย์ควรเป็นผู้ให้คำปรึกษา ให้ความรู้การดูแลแบบประคับประคอง สร้างความสัมพันธ์ที่ดีกับผู้ป่วยตั้งแต่แรกรับผ่านการสื่อสารทั้งคำพูด (วัจนภาษา) และแววตา ท่าทาง (อวัจนภาษา) เป็นเพื่อนร่วมทางพร้อมช่วยแก้ปัญหา ให้เวลารับฟังเรื่องราวผู้ป่วย และควรเน้นการสื่อสารแบบมีส่วนร่วมทั้งการสื่อสารกลุ่มเป็นทางการและไม่เป็นทางการ ได้แก่ การประชุมครอบครัว การประชุมทีมสหวิชาชีพ การเยี่ยมบ้านผู้ป่วย ที่สำคัญแพทย์ควรบอกความจริงโรคอย่างตรงไปตรงมา เข้าใจอารมณ์ความรู้สึกเห็นอกเห็นใจผู้ป่วยด้วยความจริงใจอย่างแท้จริง
The objectives of this research were to study: 1) construction of the meaning of death; 2) the process of communicating about death; and 3) approaches for best practices for communicating about death for healthcare personnel, cancer patients in palliative care and primary caregivers. This was a qualitative research based on in-depth interviews, focus group discussions and non-participatory observation in the case study hospital and patients’ homes. The key informants were chosen through purposive sampling method and snowball technique from among allied health professionals including doctors, nurses, radiologists, welfare workers and psychologists (8 individuals) along with 4 palliative care cancer patients and their 4 primary caregivers. Data were analyzed by drawing conclusions. The results showed that 1) for the construction of the meaning of death, (1) doctors tended to emphasize only the medical perspective, that is, death is when the heart stops beating. However, radiologists and medical specialists also considered social and religious perspectives and the meaning for palliative caregivers as well as their personal experiences that were involved with the desires of dying patients, so the meaning for them was more than simply when the heart stops beating; (2) other allied health professionals, cancer patients and primary caregivers tended to emphasize the religious meaning of death as a natural change and they mostly believed in an afterlife or reincarnation; (3) hospital roommates were another factor that influenced the cancer patients; some had roommates who were younger or female, and often their presence gave the patients a morale boost and helped them gradually accept the idea of death; (4) primary caregivers were impacted by the factors of time, care, emotions, feelings of attachment and closeness as well as family relations. 2) The process of communicating about death consisted of several stages: (1) in the initial admittance of a terminal-stage cancer patients, doctors focused on giving information about treatment of various symptoms but did not focus on life-saving devices or equipment. Doctors acknowledged that it is more difficult to communicate with patients in terminal cancer cases than patients in early-onset cases. (2) In the next stage of describing the symptoms and meeting with allied professionals, the doctors tried to let the patients plan their own course of treatment, and because the doctors were the patients’ only hope, patients and primary caregivers tended to consent to whatever the doctors suggested because treatment can help boost morale and is a way of bargaining after the patients were transferred to palliative care. At the same time, the doctors bargained with the patients and the allied health team using the weight of their medical authority. (3) As treatment progressed and the patients were getting ready to move back home for hospice, the other allied health professionals played a greater role in caring for the patients and tried to understand their emotions and give them full sympathy according to their religious views, paying attention to the spiritual needs and interests of the patients and their primary caregivers. (4) Finally, when they went home for hospice, the patients returned to the reality of their former daily lives and negotiated through the communication about dying by relying on their fundamental beliefs, such as in spirit mediums. 3) Recommended approaches to communicating about death are that healthcare professionals, especially doctors, should consult and provide information about palliative care, and should build good relationships with patients from the very beginning by communicating verbally with language and nonverbally through facial expressions and body language. They should communicate that they are fellow travelers on the journey and can help solve problems. They should always take time to listen to what the patients want to say and should promote participatory communication, both formal and informal, such as family meetings, allied healthcare personnel team meetings, and home visits. Most importantly, doctors should tell the truth about the disease course and say it directly, while at the same time understanding the feelings of the patients and being sincerely sympathetic
