ปัจจัยที่มีความสัมพันธ์กับผลการพัฒนาระบบคุณภาพห้องปฏิบัติด้านการแพทย์และสาธารณสุขของหน่วยบริการระดับปฐมภูมิตามมาตรฐานโรงพยาบาลส่งเสริมสุขภาพตำบลติดดาว จังหวัดกระบี่

Abstract

การวิจัยเชิงสำรวจแบบตัดขวางครั้งนี้มีวัตถุประสงค์เพื่อ 1) ศึกษาปัจจัยด้านองค์การและการบริหารจัดการองค์การของหน่วยบริการระดับปฐมภูมิ จังหวัดกระบี่ (2) ประเมินผลการพัฒนาระบบคุณภาพห้องปฏิบัติการด้านการแพทย์และสาธารณสุขตามมาตรฐานโรงพยาบาลส่งเสริมสุขภาพตำบลติดดาวของหน่วยบริการระดับปฐมภูมิ จังหวัดกระบี่ และ (3) ศึกษาความสัมพันธ์ระหว่างปัจจัยด้านองค์การ การบริหารจัดการองค์การ และผลการพัฒนาระบบคุณภาพห้องปฏิบัติการด้านการแพทย์และสาธารณสุขตามมาตรฐานโรงพยาบาลส่งเสริมสุขภาพตำบลติดดาวของหน่วยบริการระดับปฐมภูมิในจังหวัดกระบี่ กลุ่มตัวอย่าง คือ หน่วยบริการระดับปฐมภูมิในจังหวัดกระบี่ ทั้งหมด 75 แห่ง โดยให้บุคลากรที่รับผิดชอบงานเป็นผู้ให้ข้อมูลแห่งละ 1 คน เครื่องมือในการศึกษาเป็นแบบประเมิน ซึ่งมีค่าสัมประสิทธิ์แอลฟาของครอนบรากเท่ากับ 0.97 วิเคราะห์ข้อมูลเชิงปริมาณด้วยค่าสถิติ ร้อยละ ค่าเฉลี่ย ค่าเบี่ยงเบนมาตรฐาน และการวิเคราะห์การถดถอยเชิงพหุ ผลการศึกษาพบว่า (1) ปัจจัยด้านองค์การ ได้แก่ ด้านขนาดโครงสร้าง ส่วนใหญ่เป็นโรงพยาบาลส่งเสริมสุขภาพตำบลขนาดใหญ่ บุคลากรตำแหน่งนักวิชาการสาธารณสุข และบุคลากรตำแหน่งข้าราชการ การบริหารจัดการองค์การทั้ง 7 ด้าน มีระดับความเป็นอยู่ในระดับเห็นด้วยมาก และด้านโครงสร้างองค์กรมีค่าเฉลี่ยมากที่สุด (2) ผลการพัฒนาคุณภาพห้องปฏิบัติการทางการแพทย์และสาธารณสุขตามมาตรฐานโรงพยาบาลส่งเสริมสุขภาพตำบลติดดาว อยู่ในระดับดีมาก และ (3) ผลการพัฒนาระบบคุณภาพห้องปฏิบัติการด้านการแพทย์และสาธารณสุขของมาตรฐานโรงพยาบาลส่งเสริมสุขภาพตำบลติดดาว มีความสัมพันธ์กับปัจจัยด้านขนาดโครงสร้างของโรงพยาบาลส่งเสริมสุขภาพตำบล เเละด้านทักษะความสามารถบุคลากร ซึ่งทั้ง 2 ปัจจัยสามารถร่วมกันอธิบายการแปรผันได้ร้อยละ 46.30

The cross-sectional survey research aimed (1) to study organizational and management factors of primary care units (PCUs); (2) to evaluate the qualitative development program for medical and public health laboratories; and (3) to study the relationship between organizational and management factors, the results of the quality development program of PCUs’ medical and public health laboratories, all according to the Standards of Starred Sub-district or Tambon Health Promoting Hospitals (THPHs), or PCUs, in Krabi Province. The study involved a sample of 75 PCUs in Krabi -1 person each responsible for primary care being data provider. The study instruments was an evaluation form with a Cronbach’s alpha coefficient of 0.97. Analyses were performed for quantitative data to determine frequencies, percentages, means, standard deviations, and multiple linear regression. The results indicated that, for the quality development program for medical and public health laboratories at the PCUs according to the Starred THPH Standards in Krabi province: (1) regarding organizational factors, most of the PCUs in the province were large-sized THPHs and staffed mostly by public health technical officers who were government officials; all seven aspects of organizational management factors were at a high level – highest for organizational structure, on average; (2) the results of the lab quality development program were at a very good level; and (3) the factors related to the lab quality development were THPH’s size and personnel’s working skills, both of which could explain 46.30% of the variations.
