Abstract
การวิจัยและพัฒนานี้ มีวัตถุประสงค์เพื่อ 1) วิเคราะห์สภาพการณ์บันทึกทางการพยาบาล กลุ่มงานสูติ-นรีเวชกรรม โรงพยาบาลอุทัยธานี 2) สร้างรูปแบบบันทึกทางการพยาบาลกลุ่มงานสูติ-นรีเวชกรรม โรงพยาบาลอุทัยธานี และ 3) ประเมินผลการนำรูปแบบการบันทึกทางการพยาบาลไปปฏิบัติจริงในหอผู้ป่วย กลุ่มตัวอย่าง เลือกแบบเจาะจง ประกอบด้วย ผู้ให้ข้อมูลหลัก 10 คน พยาบาลวิชาชีพหอผู้ป่วยสูติ-นรีเวชกรรม 10 คน และแบบบันทึกทางการพยาบาลของผู้ป่วยที่พักรักษาในหอผู้ป่วยสูติ-นรีเวชกรรม โรงพยาบาลอุทัยธานี 40 แฟ้ม สำหรับก่อนและหลังทดลองจำนวนเท่า ๆ กัน เครื่องมือวิจัยพัฒนาโดยผู้วิจัยมี 6 ชุด ได้แก่ (1) แนวคำถามการสนทนากลุ่ม (2) แบบวิเคราะห์แบบฟอร์มบันทึกทางการพยาบาล (3) แบบสอบถามความคิดเห็นเกี่ยวกับวิธีการบันทึกทางการพยาบาล (4) โครงการอบรมเชิงปฏิบัติการ เรื่อง รูปแบบบันทึกทางการพยาบาล และคู่มือการเขียนบันทึกทางการพยาบาลแบบโฟกัส (5) แบบประเมินคุณภาพบันทึกทางการพยาบาล และ (6) แบบสอบถามความพึงพอใจของพยาบาลวิชาชีพต่อการใช้รูปแบบบันทึกทางการพยาบาล เครื่องมือผ่านการตรวจสอบความตรงของเนื้อหาจากผู้ทรงคุณวุฒิจำนวน 5 ท่าน มีค่าความตรงตามเนื้อหาเท่ากับ 0.98 และเครื่องมือชุดที่ 5) และ 6) มีค่าสัมประสิทธิ์อัลฟาครอนบาคค่าความเที่ยงเท่ากับ 0.86 และ 0.89 ตามลำดับ สถิติที่ใช้ ได้แก่ ร้อยละ ค่าเฉลี่ย ส่วนเบี่ยงเบนมาตรฐาน ทดสอบความแตกต่างด้วยสถิติวิลค็อกซัน ผลการวิจัย พบว่า 1) รูปแบบการบันทึกทางการพยาบาลที่พัฒนาขึ้น ประกอบด้วย แบบบันทึกและวิธีการบันทึกทางการพยาบาลแบบโฟกัส โดยปรับปรุงใหม่ จำนวน 6 แบบฟอร์ม โดยปรับลดลงเหลือ 5 แบบฟอร์ม ได้แก่ (1) แบบประเมินการเลี้ยงลูกด้วยนมแม่ (2) แบบประเมินการให้นมแม่โดยวิธีแลซ (3) ตารางแสดงภาวะลิ้นติดและลักษณะของหัวนมแม่ (4) แบบบันทึกการจำหน่ายออกจากโรงพยาบาล (5) แบบบันทึกติดตามผลการพยาบาล และ (6) ใบบันทึกอาการเปลี่ยนแปลงที่สำคัญ 2) คุณภาพบันทึกทางการพยาบาลของเวชระเบียนโดยรวมทุกด้านค่าเฉลี่ยหลังการพัฒนาสูงกว่าก่อนการพัฒนาอย่างมีนัยสำคัญทางสถิติ (p < .05) และ 3) ความพึงพอใจของพยาบาลวิชาชีพต่อการใช้รูปแบบบันทึกทางการพยาบาลที่พัฒนาขึ้นใหม่ อยู่ในระดับมาก (M = 4.12, SD =.51).
The purposes of this research and development were: 1) to Analyze the problems of a Nursing Document Model for the Obstetrics and Gynecology Department. 2) to develop the nursing document model for the Obstetrics and Gynecology Department at Uthai Thani Hospital and 3)to evaluate the results of implementing the developed model at ward. The sample comprised 10 key informants, 10 professional nurses, and 40 nursing recorders of patients admitted in the Obstetrics and Gynecology Department for using pre and post experimental equally and they were selected by purposive sampling. The research tools composed of (1) a focus group guideline, (2) a form for nursing document analysis, (3) questionnaires asking for opinions on how to write nursing documents, (4) a workshop project on the nursing document record, and a handbook for writing nursing documents; (5) an evaluation form for evaluating the quality of nursing documents; and 6) satisfaction questionnaires asking for satisfaction on using the nursing document model. These tools were developed by the researcher, and were verified by 5 experts. Content validity index and the Cronbach’s alpha reliability coefficients of the 5th and 6th questionnaires were 0.86 and 0.89 respectively. Data were analyzed by percentage, mean, standard deviation, and Wilcoxon Signed Ranks Test. Research findings were as follows. 1) The developed model comprised a form and how to record nursing documents were which used the “Focus charting”. The documents form of developed nursing included (1) an evaluation form for breast feeding, (2) an evaluation form for LATCH breast feeding, (3) a table showing intolerance and the characteristics of mother's nipples, (4) a discharge plan form, and (5) a nursing monitoring form. 2) After the model was implemented, the mean score of quality on recording nursing documents were statistically significantly higher than before (p < .05). 3) Satisfaction of nursing document model was at the high level (M = 4.12, SD =.51).
